        Pubmed로 검색한 소아 천식의 침술 및 혈위약물요법 치료에 관한 문헌고찰

        Objectives Asthma is an allergic disease that frequently occurs in children. This study aims to research acupuncture and acupoint-application treatment of pediatric asthma. Methods We got 53,653 search results with searching word 'child' 'children' 'pediatric' 'juvenile' 'adolescence' 'baby' 'infant' 'asthma' in Pubmed and got 147 results among them with searching word 'acupuncture' 'acupoint application'. We selected 13 articles among them which were seem to be related to this study. We classified these studies according to the type of study, the treatment methods and treatment outcomes. Results There were few studies about acupuncture and acupoint application on pediatric asthma. According to current research, conducting acupuncture and acupoint application treatment on pediatric asthma patient is recommended. Especially, acupoint application and laser acupuncture treatment were important to consider as treatment options on pediatric asthma patients for non-invasive way and less painful to children. Conclusions For the study on acupuncture and acupoint application treatment in pediatric asthma, it is necessary that we conduct research on various merit of acupuncture. More clinical data would be needed to prove the effects of acupuncture and acupoint application treatment in pediatric asthma.

        Hydroxylapatite 보형물($VoCoM^{(R)}$)을 이용한 제1형 갑상연골성형술

        Background and Objectives: $VoCoM^{(R)}$ is a set composed of prefabricated hydroxylapatite implants and shims of various sizes that are designed for the type I thyroplasty, Authors aimed to evaluate the efficacy of $VoCoM^{(R)}$ System in type I thyroplasty. Materials and Method: Twenty three patients with unilateral vocal cord palsy were included in the study, who received type I thyroplasty with $VoCoM^{(R)}$ between May 2000 and May 2007 in St. Mary's Hospital, The Catholic University of Korea, Seoul, Korea, Acoustic, aerodynamic and stroboscopic analyses were performed pre-and post-operatively, Subjective voice improvement was analysed by Voice handicap index, Results : Preoperative jitter was $4.68{\pm}2.46%$ and improved to $3.19{\pm}1.94%$(P<0,05), Preoperative NHR was $0.26{\pm}0.1$ and improved to $0.18{\pm}0,07$(P<0.05), Preoperative MPT was $6.16{\pm}4.9$secs improved to $9.55{\pm}4.67$secs(p<0.05), The postoperative stroboscopy revealed an effective medialization of vocal fold of all patients, Conclusion: Type I thyroplasty using $VoCoM^{(R)}$ is a efficient, safe and convenient way of vocal fold medialization at the expense of its high cost and difficulty in removal.

        일본에서 소아질환에 적용하는 한약치료에 대한 문헌고찰

        Objectives : This research aimed to analyze studies on pediatric disease treated by kampo drug, kind of kampo drug used in children, treatment period, and the result of kampo drug treatment for children in Japan. Methods : We got 263 search result with searching word 'kampo medicine' and '小兒', 'children', '乳兒' in J-stage. We selected 34 articles among them which were related to objective of research to analyze studies by type of pediatric disease treated with kampo drug, kinds of kampo drug for each disease, treatment period and result of kampo drug treatment for children. We considered frequency of kampo drug use & pediatric disease treated with kampo drug and significance of research. Results : According to analyzed results, respiratory diseases are the most frequent diseases that are healed by kampo drug. Next sequenced diseases are skin disease. In Kind of kampo drug for pediatric disease. Goreisan and Shosaikoto (柴胡桂枝湯), Shokenchuto (小建中湯) are used frequently. Also, various disease treated with kampo drug were improved. Conclusions : Japanese Doctors consider the Kampo drug is safe and has a lot of merit compared to modern medication. Especially for symptom with unknown origin & immune diseases such as upper respiratory tract infections. Referring to clinical cases of kampo drug in Japan, we will use kampo drug for various pediatric diseases in future.

        Cricopharyngeal dysphagia(CPD), a common condition in the dysphagic patient, refers to the dysfunction of the upper esophageal sphincter complex(UESC), which is composed of the cricopharyngeus, inferior pharyngeal constrictor and the upper segment of the cervical esophagus. Primary CPD is the disease entity solely confined to dysfunctional UESC, while secondary CPD encompasses various conditions that accompany UESC dysfunction. For proper diagnosis and treatment of such entity, a thorough understanding of the complex anatomy and physiology of the upper esophageal sphincter. Adequate relaxation of the cricopharyngeal muscle in conjunction with anterosuperior excursion of the larynx by suprahyoid muscles and propulsion of food bolus are prerequisite for normal swallow, mechanisms of which if altered result in cricopharyngeal dysfunction. Of the various methods used for the diagnosis of cricopharyngeal dysphagia, videofluoroscopy remains the method of choice. Mechanical dilatation of the cricopharayngeus, cricopharyngeal myotomy and botulinum toxin injection and head-lift exercise have been used in clinical practice to relieve dysphagia in such patients. Such procedures have therapeutic effect in primary CPD, but so often fail to relieve swallowing dysfunction in patient with secondary CPD. We herein explain ancillary procedures that support these primary treatment options, which lead to successful treatment of dysphagia.

        Polymer Solar Cells based on a Furan-containing Asymmetric Nonfullerene Acceptor

        고분자 태양전지의 어셉터로 비접합된 전자 주는 thiophene-furan 중심과 전자 끄는 옥틸로다닌 말단 그룹으로 이루어진 비대칭 비풀러렌 단분자 TF-ORH가 설계되었다. TF-ORH은 Knoevenagel 축합과 스즈끼 짝지음 반응을 통해 합성되었다. TF-ORH는 소자 제작에 충분한 열 안정성 및 용해도를 나타냈다. 용액과 비교하여, TF-ORH 필름은 596 nm에서 최대 흡수를 가지는 장파장 이동된(J-aggregation) UV-Vis 흡수를 나타내었고, 이는 저밴드갭 고분자 도너인 PTB7-Th의 특성과 상호보완적이다. 퓨란의 도입으로 thiophene 유사체인 T2-ORH와 비교하여 분자 궤도 에너지 준위들은 높아졌다. 활성층으로서 PTB7-Th:TF-ORH 블렌드 필름을 사용하여 제작된 고분자 태양전지 소자는 0.88V의 개방전압과 0.70 eV의 낮은 에너지 손실로 1.21%의 전력 변환 효율을 나타냈다. An asymmetric nonfullerene small molecule, TF-ORH, containing a nonfused electron-donating core of thiophene, furan and electron-withdrawing octylrhodanine (ORH) end groups was designed as an acceptor for polymer solar cells (PSCs). TF-ORH was synthesized via Knoevenagel condensation and the Suzuki coupling reaction. TF-ORH exhibited sufficient thermal stability and solubility for device fabrication. Compared to solution, TF-ORH film showed a redshifted UV–Vis absorption with an absorption maximum at 596 nm (J-aggregation), which is complementary with that of the low bandgap polymer donor of poly[4,8-bis(5-(2-ethylhexyl)thiophen-2-yl)benzo[1,2-b:4,5-b"]dithiophene-co-3- fluorothieno[3,4-b]thiophene-2-carboxylate] (PTB7-Th). The introduction of furan elevated the molecular orbital energy levels of TF-ORH compared to the thiophene analog, T2-ORH. Finally, a PSC fabricated using a PTB7-Th:TF-ORH blend film as the active layer exhibited a power conversion efficiency of 1.21% with an open-circuit voltage of 0.88 V and a small energy loss of 0.70 eV.

        Pilomatrixoma is an unusual benign cutaneous tumor, originating from precursor cells that normally differentiate into hair matrix cells. The majority of pilomatrixoma is seen in the first two decades of life, and they are extremely rare in older patients. Thus, pilomatrixoma is often not considered in the clinical differential diagnosis of solitary, firm nodules in older adults. Most present as firm, solid nodules in the head and neck region. We present two cases of a non-calcifying pilomatrixoma in older patients. One lesion arose from the upper neck in 54 year-old male and the other from the cavum concha of left ear in 35 year-old male.
