        인지행동치료를 통한 치과불안 감소에 대한 질적 연구

        본 연구는 치과 의료진에 의해 실시된 치과불안 감소에 대한 인지행동치료의 효과를 검증하고 이를 통해 치과 진료 과정에서 적용 가능한 효과적인 개입방법 제안을 연구의 목적으로 하였다. 연구대상과 방법은 임상치과의원에 내원한 치과불안 증세를 보이는 환자중, 연구에 동의한 1인을 대상으로 DFS, 심층인터뷰, 그리고 SUD를 이용하여 환자의 치과불안 정도를 측정하고 불안의 종류와 수준에 따라 인지행동치료를 3월 2일부터 5월 20일까지 2주 간격으로 5회기 실시하였으며 연구의 결과는 다음과 같다. 첫째, 치과에 관련하여 왜곡된 인지의 변화는 이와 연결된 불안들과 함께 짧은 기간 동안 급성호전 현상을 나타내며 감소하였다. 둘째, 환자 개개인의 특성과 심리적 상태에 따라 주요 개입법과 보조적인 개입법을 함께 사용하는 것이 효과적이며, 호흡이 완법은 신체적 긴장감과 심리적 안정화를, 수신호 사용은 진료과정 중의 소통에 효과적으로 나타났다. 공감적 관계 맺기는 의료진과의 신뢰성 형성을 통한 원활한 소통과 지속적인 치료에 대한 동기부여 및 치료적 퇴보를 완화시켰다. 불안목록 작성, 점진적인 둔감법, 직접적인 노출법, 진료과정 리허설은 문제에 대한 직접적이고 점진적인 도전을 경험함으로써 적응력강화 및 자기 조절 능력을 향상시키고 왜곡된 인지 변화를 유도하였다. 셋째, 인지행동치료를 훈련한 치과 의료진이 치료과정 중에 실시한 단시간 심리적 개입이 환자의 치과불안 감소에 유의미한 효과가 있음을 도출하였다. 본 연구는 단일사례연구라는 제한점에도 불구하고 단기간의 인지행동치료 개입법이 치과와 연계된 왜곡된 인지를 재구조화함으로써 환자의 치과진료 회피행동과 치과불안을 감소시키는 것에 효과적인 접근법이며 지속적인 연구가 필요함을 확인하였다. Many people in Korea suffer from a fear of dental treatments; however, only a few studies have focused on this population. This study assessed the effectiveness of cognitive behavioral therapy (CBT) and its interventions in reducing dental anxiety, especially when administered by trained dental staff. Using case studies, the authors analyzed the content of each session and examined the process of reducing dental anxiety. First, the authors observed that the results of both dental fear scale and subjective unit of disturbance decreased significantly with changes in cognitive distortions related to dental anxiety and that rapid improvements were noted after two therapy sessions. Second, the study reported that practicing relaxation (deep-breathing) and muscle relaxation techniques; using hand signals to indicate anxiety or discomfort; establishing an empathic relationship with the therapist; making an anxiety list; and graded insensitive and exposure along with rehearsal, were effective in coping with dental anxiety and in changing negative thoughts towards the dentist. Third, a simple cognitive behavioral intervention can be effective in reducing dental anxiety. In summary, a short-term, five-session psychological intervention with CBT produced a sustained decrease in the symptoms of dental anxiety, allowing the patients to receive the required procedures. The authors discussed the study limitations and the implication of their results on future research.

      • 가토심장동방결절 및 방실결절세포의 전기적 특성에 미치는 Taurocholate 및 Cholate의 효과

        The present study was designed to investigate effects of taurocholate (TCA) and cholate (CA) on transmembrane potentials of pacemaker cells of the rabbit heart. And also the electrophysiological effect of these bile acids on sinoatrial node was compared with on atrioventricular node. The results obtained were as follows: 1. Following superfusion of central sinus node cell (CSN) with CA (o.023-0.05%) action potential amplitude (APA) increased, while rate of spontaneous firing (RSF) decreased markedly. 2. When the peripheral sinus node cell (PSN) was superfused with TCA (O.05%), RSF of pacemaker was decreased, whereas action potential duration (APD60) was prolonged. On the other hand APA of PSN was increased by CA (0.05%). 3. Superfusion of atrioventricular mode cells (AVN) with CA (0.025-0.05%) and TCA (0.025-0.05%) resulted in a decrement in RSF and a prolongation of APD60. And also an increment in APA was observed during superfusion of the cells with TCA (0.05%). 4. To compare effects of bile acids on transmembrane potentials of the CSN and PSN, APA and RSF of CSN pacemaker cells were altered by CA while RSF and APD60 of PSN were changed by TCA. 5. When effects of bile acids on cellular potentials were compared between the CSN and the AVN, it was found that only Rsf and APD60 of AVN were decreased after wuperfusion with CA. 6. The minimun concentration of CA that reduced RSF of both sinoatrial and AVN cells was 0.025% and the lowest concentration of TCA that altered APD60 of AVN was 0.025% also. However, APA of all these pacemaker cells were increased following superfusion with 0.05% TCA or CA.

      • Effect of Dieckol, a Component of <i>Ecklonia cava</i> , on the Promotion of Hair Growth

        <P>This study was conducted to evaluate the effect of <I>Ecklonia cava</I>, a marine alga native to Jeju Island in Korea, on the promotion of hair growth. When vibrissa follicles were cultured in the presence of <I>E. cava</I> enzymatic extract (which contains more than 35% of dieckol) for 21 days, <I>E. cava</I> enzymatic extract increased hair-fiber length. In addition, after topical application of the 0.5% <I>E. cava</I> enzymatic extract onto the back of C57BL/6 mice, anagen progression of the hair-shaft was induced. The treatment with <I>E. cava</I> enzymatic extract resulted in the proliferation of immortalized vibrissa dermal papilla cells (DPC). Especially, dieckol, among the isolated compounds from the <I>E. cava</I> enzymatic extract, showed activity that increased the proliferation of DPC. When NIH3T3 fibroblasts were treated with the <I>E. cava</I> enzymatic extract and the isolated compounds from the <I>E. cava</I> enzymatic extract, the <I>E. cava</I> enzymatic extract increased the proliferation of NIH3T3 fibroblasts, but the isolated compounds such as eckol, dieckol, phloroglucinol and triphlorethol-A did not affect the proliferation of NIH3T3 fibroblasts. On the other hand, the <I>E. cava</I> enzymatic extract and dieckol significantly inhibited 5α-reductase activity. These results suggest that dieckol from <I>E. cava</I> can stimulate hair growth by the proliferation of DPC and/or the inhibition of 5α-reductase activity.</P>

        Background: The procedure that combines crystal-free microdermabrasion using an abrading tip with the pneumatic application of chemical peeling could be effective than chemical peeling. Rare basic data about the recovery of skin barrier function after combined treatment of chemical peels and dermabrasion have been investigated. Objectives: We aimed to investigate the changes in skin barrier function after chemical peeling combined with physical effects. Methods: In this prospective, randomized and split-face controlled study, 13 Asian women were enrolled. We measured the degree of acute skin damage and the time required for the skin barrier function to recover including transepidermal water loss(TEWL), erythema index. We also assessed the skin blood flow in order to investigate the impact of negative pressure on vasculature of the dermis. Results: TEWL in chemical peeling combined with pneumatic dermabrasion was increased after 30 minutes and a significantly decreased TEWL was observed at 2 dayscompared with that in the chemical peeling treated site. The erythema index was the highest at 30 minutes, and it gradually decreased after 7 days. However, the changes in skin erythema index were not significant. After 30 minutes, skin blood flow increased in the chemical peeling combined with pneumatic dermabrasion treated site. Conclusion: In conclusion, skin barrier function following a single application of AHA combined with negative pressure was recovered after 2 days.

      • Use of non-melanocytic HEK293 cells stably expressing human tyrosinase for the screening of anti-melanogenic agents.

        <P>Tyrosinase (TYR) from mushrooms has been inappropriately used in the screening assay for hypopigmenting agents even though its biochemical properties are different from those of human TYR. Cell-free extracts of human epidermal melanocyes (HEMs) could be another choice for the assay, but HEMs grow too slowly to get a sufficient amount of cell-free extracts. In the present study, human embryonic kidney (HEK) 293 cells were transfected with a human TYR construct to establish a cell line that grows rapidly and expresses human TYR constitutively. Cell-free extracts of the established cell line, HEK293-TYR, were tentatively used in the screening assays for 11 phenylpropanoids that have chemical structures similar to that of L-tyrosine, the substrate of TYR. Of the 11 compounds, the strongest inhibition of TYR activity was shown by p-coumaric acid (IC50, 3 μM), followed by 3-(4-hydroxyphenyl)propionic acid (50 μM) and 3-(4-hydroxyphenyl)lactic acid (70 μM). The results indicate that p-coumaric acid has an optimal chemical structure for the inhibition of TYR. The effects of these phenylpropanoids on melanin synthesis in HEMs correlated well with their effects on TYR activity in vitro. This study demonstrated that HEK293-TYR cells can be a good source of the human TYR enzymes needed in the screening assay of anti-melanogenic agents.</P>

        Apoptosis Inducing Activity of Fucoidan in HCT-15 Colon Carcinoma Cells

        <P>The antitumor activity of fucoidan from <I>Fucus vesiculosus</I> was investigated in human colon carcinoma cells. The crude fucoidan, a polysaccharide composed predominantly of sulfated fucose, markedly inhibited the growth of HCT-15 cells (human colon carcinoma cells). After HCT-15 cells were treated with fucoidan, several apoptotic events such as DNA fragmentation, chromatin condensation and increase of the population of sub-G1 hypodiploid cells were observed. In the mechanism of fucoidan-induced apoptosis, we examined changes in Bcl-2 and Bax protein expression levels and activation of caspases. Fucoidan decreased Bcl-2 expression, whereas the expression of Bax was increased in a time-dependent manner compared to the control. In addition, the active forms of caspase-9 and caspase-3 were increased, and the cleavage of poly(ADP-ribose) polymerase (PARP), a vital substrate of effector caspase, was observed. Furthermore, the induction of apoptosis was also accompanied by a strong activation of extracellular signal-regulated kinase (ERK) and p38 kinase and an inactivation of phosphatidylinositol 3-kinase (PI3K)/Akt in a time-dependent manner. These findings provide evidence demonstrating that the pro-apoptotic effect of fucoidan is mediated through the activation of ERK, p38 and the blocking of the PI3K/Akt signal pathway in HCT-15 cells. These data support the hypothesis that fucoidan may have potential in colon cancer treatment.</P>

        치과내원 환자의 치과공포 감소를 위한 중재법 적용

        The purposes of this study were to assess dental fear and anxiety level of patients attending a dental clinic using the dental fear survey (DFS) scale, to apply interventions chosen by patients for reducing dental fear and anxiety and measure their effects. This study surveyed 34 patients who visited a dental clinic in Seoul about their self-rated health and their experience of dental fear, and measured the level of dental fear using DFS. Trained dental hygienists applied interventions desired by the patients for reducing dental fear and anxiety and, for each intervention, examined the patients. satisfaction (very helpful [5 point]∼not helpful at all [1 point]). Collected data were analyzed using IBM SPSS Statistics ver. 21.0 through independent t-test or one-way ANOVA for difference in level of fear according to related characteristics, and through Wilcoxon signed rank test for comparison between before and after the intervention. The subjects. mean level of fear (DFS score) was 44.53, which was an average level, and the level of dental fear was relatively high for stimulus-response (2.72). The level of dental fear was higher in those who had experienced pains or indirect dental fear from dental treatment in the past, and those whose subjectively perceived health state was poor (p<0.05). With regard to the applied dental fear intervention, .Helpful (3.57). was the most common answer. Overall satisfaction before and after the application of dental fear intervention was generally high as 4.37 and 4.35, respectively, but it decreased slightly after the intervention. In order to lower the level of dental fear, it is considered effective to survey not only the level of dental fear but also patients. need of dental fear interventions and to apply a suitable intervention. It is also required to educate dental workers and to develop related manuals.
