        Numerical analysis of vertical drains accelerated consolidation considering combined soil disturbance and visco-plastic behaviour

        Soil disturbance induced by installation of mandrel driven vertical drains decreases the in situ horizontal hydraulic conductivity of the soil in the vicinity of the drains, decelerating the consolidation rate. According to available literature, several different profiles for the hydraulic conductivity variation with the radial distance from the vertical drain, influencing the excess pore water pressure dissipation rate, have been identified. In addition, it is well known that the visco-plastic properties of the soil also influence the excess pore water pressure dissipation rate and consequently the settlement rate. In this study, a numerical solution adopting an elastic visco-plastic model with nonlinear creep function incorporated in the consolidation equations has been developed to investigate the effects of disturbed zone properties on the time dependent behaviour of soft soil deposits improved with vertical drains and preloading. The employed elastic visco-plastic model is based on the framework of the modified Cam-Clay model capturing soil creep during excess pore water pressure dissipation. Besides, nonlinear variations of creep coefficient with stress and time and permeability variations during the consolidation process are considered. The predicted results have been compared with V$\ddot{a}$sby test fill measurements. According to the results, different variations of the hydraulic conductivity profile in the disturbed zone result in varying excess pore water pressure dissipation rate and consequently varying the effective vertical stresses in the soil profile. Thus, the creep coefficient and the creep strain limit are notably influenced resulting in significant changes in the predicted settlement rate.

        An Improved Technique for Pulmonary Endarterectomy

        We report a modified technique for pulmonary endarterectomy (PEA) on a 67-year-old man with chronic thromboembolic pulmonary hypertension (CTEPH) who presented with dyspnea. He was referred to our medical center for coronary artery bypass grafting. CTEPH had not been detected in his first visit to another medical center, but upon re-evaluation, the diagnosis was confirmed. PEA was performed with a modified method, which seems to be safe and suitable for the removal of clot and fibrotic materials. Iatrogenic dissection was performed with normal saline injection in the pulmonary artery, and then, the clot was removed completely. Although the technique may not be applicable for all cases, it can be used as an alternative to using an aspirating dissector and a pair of forceps.

        Ketone ester supplementation of Atkins type diet prolongs survival in an orthotopic xenograft model of glioblastoma

        Heavy reliance on glucose metabolism and a reduced capacity to use ketone bodies makes glioblastoma (GBM) a promising candidate for ketone-based therapies. Ketogenic diet (KD) is well-known for its promising effects in controlling tumor growth in GBM. Moreover, synthetic ketone ester (KE) has demonstrated to increase blood ketone levels and enhance animal survival in a metastatic VM-M3 murine tumor model. Here, we compared the efficacy of a KE-supplemented Atkins-type diet (ATD-KE) to a classic KD in controlling tumor progression and enhancing survival in a clinically relevant orthotopic patient-derived xenograft GBM model. Our findings demonstrate that ATD-KE preserves body weight (percent change from the baseline; 112±2.99 vs. 116.9±2.52 and 104.8±3.67), decreases blood glucose (80.55±0.86 vs. 118.6±9.51 and 52.35±3.89 mg/dl), and increases ketone bodies in blood (1.15±0.03 mM vs. 0.55±0.04 and 2.66±0.21 mM) and brain tumor tissue (3.35±0.30 mM vs. 2.04±0.3 and 4.25±0.25 mM) comparable to the KD (results presented for ATD-KE vs. standard diet [STD] and KD, respectively). Importantly, the ATD-KE treatment significantly enhanced survival compared to the STD and was indistinguishable from the KD (47 days in STD vs. 56 days in KD and ATD-KE), suggesting that a nutritionally balanced low carbohydrate ATD combined with KE may be as effective as the KD alone in reducing brain tumor progression. Overall, these data support the rationale for clinical testing of KE-supplemented low-carb diet as an adjunct treatment for brain tumor patients.

        The Effect of Agglomeration on Urban Productivity in Zhejiang, China

        The urban productivity not only determine with the traditional inputs such as the physical capital, human capital and labour but also the density and agglomeration of output and labour could determine part of urban productivity. This study attempts to highlight the role of spatial agglomeration on urban productivity in Zhejiang, China. For measuring the effect of agglomeration on economic performance, we use the employment as a proxy of productivity and its effect of agglomeration. The data of 11 major cities of Zhejiang province from 1999 to 2008 were chosen to estimate regressions for the aim of this study. The results show that part of the growth of urban productivity in Zhejiang could be induced by the growth of output and labour density. Hence, agglomeration is an important type of externality mechanism in determining the urban economic performance.

        An Unusual Biatrial Cardiac Myxoma in a Young Patient

        This is a report of a biatrial cardiac myxoma in a young man with a 10-month history of exertional dyspnea andpalpitation. The echocardiogram revealed biatrial myxoma prolapsing through the mitral and tricuspid valves duringdiastole. All cardiac chambers were enlarged and dysfunctional. The electrocardiogram revealed a rapid ventricularresponse with atrial flutter rhythm. The masses were resected and diagnosed as myxoma by a histologicalexamination. The follow-up echocardiogram revealed significant improvement in ventricular function and reduction inthe cardiac chambers’ volume. There was no evidence of myxoma recurrence. The most probable cause of thepatient’s heart failure was considered to be tachycardia-induced cardiomyopathy.

        The COVID-19 pandemic and healthcare utilization in Iran: evidence from an interrupted time series analysis

        Objectives: This study aimed to examine the effect of the coronavirus disease 2019 (COVID-19) outbreak on the hospitalization rate, emergency department (ED) visits, and outpatient clinic visits in western Iran.Methods: We collected data on the monthly hospitalization rate, rate of patients referred to the ED, and rate of patients referred to outpatient clinics for a period of 40 months (23 months before and 17 months after the COVID-19 outbreak in Iran) from all 7 public hospitals in the city of Kermanshah. An interrupted time series analysis was conducted to examine the impact of COVID-19 on the outcome variables in this study.Results: A statistically significant decrease of 38.11 hospitalizations per 10,000 population (95% confidence interval [CI], 24.93–51.29) was observed in the first month of the COVID-19 outbreak. The corresponding reductions in ED visits and outpatient visits per 10,000 population were 191.65 (95% CI, 166.63–216.66) and 168.57 (95% CI, 126.41–210.73), respectively. After the initial reduction, significant monthly increases in the hospitalization rate (an increase of 1.81 per 10,000 population), ED visits (an increase of 2.16 per 10,000 population), and outpatient clinic visits (an increase of 5.77 per 10,000 population) were observed during the COVID-19 pandemic.Conclusion: Our study showed that the utilization of outpatient and inpatient services in hospitals and clinics significantly declined after the COVID-19 outbreak, and use of these services did not return to pre-outbreak levels as of June 2021.

        Guided wave analysis of air-coupled impact-echo in concrete slab

        This study aims to develop a signal processing scheme to accurately predict the thickness of concrete slab using air-coupled impact-echo. Air-coupled impact-echo has been applied to concrete non-destructive tests (NDT); however, it is often difficult to obtain thickness mode frequency due to noise components. Furthermore, apparent velocity in concrete is a usually unknown parameter in the field and the thickness of the concrete slab often cannot be accurately measured. This study proposes a signal processing scheme using guided wave analysis, wherein dispersion curves are drawn in both frequency-wave number (f-k) and phase velocity-frequency (V_cp-f) domains. The theoretical and experimental results demonstrate that thickness mode frequency and apparent velocity in concrete are clearly obtained from the f-k and V_cp-f domains, respectively. The proposed method has great potential with regard to the application of air-coupled impact-echo in the field.

      • Literacy and Breast Cancer Prevention: a Population-Based Study from Iran

        Breast cancer is the most common cancer in women worldwide and the leading cause of death from cancer among women. Evidence suggests that early diagnosis and screening interventions might help to improve outcomes. This population-based study was conducted to determine breast cancer awareness and screening behavior among Iranian women and to examine its association with women's literacy. The study was carried out in two provinces, with 1,477,045 population, located in central and eastern part of Iran. Overall, 770 women were studied. Of these, 482 (62.7%) were literate and 287 (37.3%) were not. The results obtained from the data analysis indicated that there was a significant difference between literate and illiterate women. Further analysis of the data using logistic regression showed that literacy was an important contributing factor for breast cancer prevention behavior. The findings suggest that in order to improve women's health and breast cancer outcomes providing equal educational opportunities for women seems necessary.
