Background/Aims: Both liver stiffness (LS) values measured by transient elastography and enhanced liver fibrosis(ELF) test can assess fibrotic burden accurately. Recently, a sequential combination of LS value and ELF test has been proposed to ensurea higher chance to avoid liver biopsy (LB). In this study, we assessed the diagnostic performance of LS and ELF, validated LS-ELF diagnosticalgorithm,and investigated whether sequential combinationof LS and ELF performs better than their concomitant combinationin treatment-naive patients with chronic hepatitis B (CHB). Methods: In a 4-year period (2009-2013), a total of 290 patients with CHB who underwent LB, along with LS measurment and ELF test, were recruited for retrospective analysis. External cutoff value from Hong Kong study to predict both F3-4 and F4 were 6.0 and 7.5 for LS and 8.4 and 8.8 for ELF, respectively. Results: The median age of the study population (men 144 and women 78) was 48 years. The median alanine aminotransferase level, HBV DNA level, LS value, and ELF test were 42 IU/L, 616,000 IU/L, 10.2 kPa, and 9.7, respectively. Advanced liver fibrosis (F3-4) and cirrhosis (F4) were identified in 23 (10.4%) and 118 (53.2%), respectively. Areas under the receiver operating characteristiccurve of LS value to predict F3-4 (0.887 vs. 0.703) and F4 (0.853 vs. 0.706) were significantly higher than those of ELF test (all p<0.001). The internal cutoff values to predict F3-4 and F4 were 9.0 kPa and 11.0 kPa for LS and 8.4 and 8.8 for ELF, respectively. Based on LS-ELF diagnostic algorithm, 60.4% (n=49) and 55.7% of patients (n=58) could avoid LB to exclude F3-4 and F4 using external cutoffs, respectively, whereas 71.6% (n=58) and 66.3% (n=69) of patients could avoid LB to exclude F3-4 and F4 using internal cutoffs, respectively. In addition, 78.7% (n=111) and 63.5% of patients (n=75) could avoid LB to confirm F3-4 and F4 using external cutoffs, respectively, whereas 68.0% (n=96) and 66.1% (n=78) of patients could avoid LB to confirm F3-4 and F4 using internal cutoffs, respectively. When LS-ELF diagnostic algorithm including confirmation and exclusion strategy was applied, 67.1-67.6% patients and 61.3-66.2% patients could avoid LB to diagnose F3-4 and F4, respectively, according to internal and external cutoff values, respectively. When the proportion of patients with correctly avoided LB in predicting F3-4 according to sequential LS-ELF diagnostic algorithm (69.4% by internal cutoffs and 72.5% by external cutoffs) was significantly higher than the proportionaccording tothe strategy of concomitant first-line use of LS and ELF which was proposed by Castera, et al (42.3% by internal cutoffs and 59.0% by external cutoffs) and Boursier et al (57.7%) (all p<0.05). Similar phenomenon was observed in predicting F4 (all p<0.05). Conclusion: The performance of LS value by TE to predict F3-4 and F4 was significantly higher than those of ELF test. The diagnostic performance of LS-ELF algorithm was validated in our study and the sequential LS-ELF diagnostic algorithm performed significantly better than their concomitantfirst-line use in terms of higher chance to avoid LB in patients with CHB.
Interim result of autologous bone marrow cell infusion in patients with advanced Liver cirrhosis

Background The goal of induction therapy for multiple myeloma (MM) is to achieve adequate disease control. Current guidelines favor triplet (bortezomib-lenalidomide-dexamethasone; VRd) or quadruplet regimens (daratumumab, bortezomib-thalidomide-dexamethasone; D-VTd). In the absence of a direct comparison between two treatment regimens, we conducted this study to compare the outcomes and safety of VRd and D-VTd. Methods Newly diagnosed MM patients aged >18 years who underwent induction therapy followed by autologous stem cell transplantation (ASCT) between November 2020 and December 2021 were identified. Finally, patients with VRd (N=37) and those with D-VTd (N=43) were enrolled. Results After induction, 10.8% of the VRd group showed stringent complete remission (sCR), 21.6% showed complete response (CR), 35.1% showed very good partial response (VGPR), and 32.4% showed partial response (PR). Of the D-VTd group, 9.3% showed sCR, 34.9% CR, 48.8% VGPR, and 4.2% PR (VGPR or better: 67.6% in VRd vs. 93% in D-VTd, P=0.004). After ASCT, 68.6% of the VRd group showed CR or sCR, while 90.5% of the D-VTd group showed CR or sCR (P=0.016). VRd was associated with an increased incidence of skin rash (P=0.044). Other than rashes, there were no significant differences in terms of adverse events between the two groups. Conclusion Our study supports the use of a front-line quadruplet induction regimen containing a CD38 monoclonal antibody for transplant-eligible patients with newly diagnosed MM.
Alagille syndrome and a JAG1 mutation: 41 cases of experience at a single center

Purpose: Alagille syndrome is a complex hereditary disorder that is associated with cardiac, hepatic, skeletal, ocular, and facial abnormalities. Mutations in the Notch signaling pathway, such as in JAG1 and NOTCH2, play a key role in embryonic development. A cardiac or hepatic presentation is a critical factor for determining the prognosis. Methods: We conducted a retrospective study of 41 patients with Alagille syndrome or a JAG1 mutation between 1983 and 2013. Results: The first presentations were jaundice, murmur, cyanosis, and small bowel obstruction at a median age of 1.0 months (range, 0-24 months). The JAG1 mutation was found in 27 of the 28 genetically-tested patients. Cardiovascular anomalies were identified in 36 patients, chronic cholestasis was identified in 34, and liver transplantation was performed in 9. There was no significant correlation between the severity of the liver and cardiac diseases. The most common cardiovascular anomaly was peripheral pulmonary stenosis (83.3%), with 13 patients having significant hemodynamic derangement and 12 undergoing surgical repair. A total bilirubin level of >15 mg/dL with a complex surgical procedure increased the surgical mortality (P=0.022). Eight patients died after a median period of 2.67 years (range, 0.33-15 years). The groups with fetal presentation and with combined severe liver and heart disease had the poorest survival (P<0.001). Conclusion: The group with combined severe liver and heart disease had the poorest survival, and a multidisciplinary approach is necessary to improve the outcome.
A Case of Recurrent Superficial Acral Fibromyxoma

Superficial acral fibromyxoma (SAFM) is a rare myxoid tumor that was first described in 2001. The presence of a very slow growing solitary tender mass in the subungual area is the typical clinical feature at presentation. Histopathologically, SAFM is composed of stellate cells in a myxocollagenous matrix with a poorly circumscribed margin. This tumor is thought to be benign, but its natural course is not fully understood. We describe a 15-year-old patient with recurrent SAFM and discuss the proper treatment and follow up. Prevalence of Toxoplasma gondii Infection in Stray and Household Cats in Regions of Seoul, Korea

The principal objective of this study was to investigate the prevalence of toxoplasmosis in household and stray cats in Seoul, Republic of Korea. We collected blood samples from 72 stray and 80 household cats, and all samples were examined by ELISA and nested PCR. The overall positive rates of Toxoplasma gondii in stray cats were 38.9% (28/72), with 15.3% (11/72) in ELISA and 30.6% (22/72) in PCR. The positive rate in male stray cats was slightly higher than that of female stray cats. The highest positive rate of T. gondii infection was noted in Gangnam and Songpa populations in ELISA and in Gwangjin population in PCR. In household cats, however, we could not detect any specific antibodies or DNA for T. gondii. In conclusion, we recognized that the infection rate of toxoplasmosis in stray cats in Seoul was considerably high but household cats were free from infection.
Real-time In-situ Plasma Etch Process Monitoring for Sensor Based-Advanced Process Control

To enter next process control, numerous approaches, including run-to-run (R2R) process control and fault detection and classification (FDC) have been suggested in semiconductor manufacturing industry as a facilitation of advanced process control. This paper introduces a novel type of optical plasma process monitoring system, called plasma eyes chromatic system (PECSTM) and presents its potential for the purpose of fault detection. Qualitatively comparison of optically acquired signal levels vs. process parameter modifications are successfully demonstrated, and we expect that PECSTM signal can be a useful indication of onset of process change in real-time for advanced process control (APC).
