세계는 부조리한 연극의 무대다 그것은 너무나 부조리한 것이어서 베케트 나 이오녜스코 의 인극도 오늘날의 세계에서 실제로 이루어지고 있는 것 과 비교하면 오히려 사회주의리얼리즘 의 작품처럼 보인다. 이 말은 체코 의 한 신문기자가 부르짖었던 것이다.1968년도의 미국 대통령 선거전을 종식하고 새 내각 조직하는 중요한 시기가 박두하게되었다. 닉슨 신행정부는 다양화한 국제사회에서 70년대를 향한 관점은 그와 대외정책에서새로운 기원을 수립 하려고 안간힘을 쓰고 있다. 닉슨은 외교분야에서 「존슨행정부는 세계가 변천하여 우리들이 새로운 세계에 살고 있다는 것을 인식하지 못하였다. 우리는 새로운 정책이 필요하다. 우리 국가가 생존하려면 새로운 리더쉽이 긴요하다. 」 고 새로운 외교정책수립의 필요성을 관명하고 있다. 68년 대통령 선거전에서 힘프리 와 닉슨은 뚜렷한 상호대차없이 도남문제 동서관계 중동문제등에 있어서 힘프리 와 거의 흡사한 입장을 취해 왔던 것이다. 그러나 이디에다 중점을 두느냐에 관한 대체점은 조금 달리한는 경향이다.

Anti-TNF-α 치료를 받은 강직성 척추염 환자들의 체중과 골밀도 변화

Background/Aims: To determine the changes in body weight and bone mineral density in patients with ankylosing spondylitis (AS) receiving anti-tumor necrosis factor-α (TNF-α) treatment. Methods: Thirty-one patients with AS (25 males and 6 females) who fulfilled the Modified New York Criteria for AS were included in this retrospective study. All patients had active disease that eventually required anti-TNF-α treatment. Each patient received anti-TNF-α treatment (etanercept 25 mg twice weekly or adalimumab 40 mg twice monthly) for more than 2 years. Body weight, disease activity as Bath ankylosing spondylitis disease activity index (BASDAI), C-reactive protein, erythrocyte sedimentation rate (ESR), lumbar bone mineral density (LBMD), and femoral bone mineral density (FBMD) were measured at baseline and at 1 and 2 years after initiating anti-TNF-α treatment. Results: There was a significant increase in mean body weight at 1 year (1.1 ± 3.8 kg) and at 2 years (1.7 ± 4.8 kg) compared with baseline. The gains in mean BMD of the lumbar spine were significant at 1 year (0.4 ± 0.4) and 2 years (0.5 ± 0.7) compared with baseline. Mean BMD of the femur was also increased at 1 year (0.08 ± 0.7) and 2 years (0.1 ± 0.8) compared with baseline, but these differences were not statistically significant. There were significant decreases in BASDAI at 1 year (-3.3 ± 2.8) and at 2 years (-3.6 ± 2.8) compared with baseline. Conclusions: This study showed significant increases in body weight, lumbar BMD, and BASDAI at 1 year and 2 years in patients with ankylosing spondylitis after receiving anti-TNF-α treatment. 고분자에 고정화된 효소는 생성물로부터 효소의 분리가 용이하고, 재사용이 가능하며, 고온 반응과 다양한 pH에서도 안정성을 유지할 수 있기 때문에, 다양한 생화학 반응 시스템에서 응용가능하다. 본 연구에서는 오존을 이용하여 폴리스티렌 비드에 퍼록사이드기를 도입하고 redox 중합 반응을 통해 PS-sg-PGMA나 PS-sg-P(GMA-co-MAM)등과 같이 표면이 그래프트된 폴리스티렌 계열의 고분자 담체들을 합성하였고, W/O 에멀전 방법에 의에 키토산 고분자 담체를 합성하여, 각각을 효소 고정화 반응에 응용해 보았다. 또한 고정화된 효소의 활성과 안정성을 각각 비교하여 고정화 담체가 가져야 할 특성이 무엇인지 도출하고자 하였다.

Gastric bezoars are concretion of undigested material in the gastrointestinal tract. In the past, gastric bezoars were generally treated with surgical management. Recently, the efficacy of oral intake or endoscopic injection therapy with Coca-Cola has been reported. We report a case of a 47-year-old-man with huge gastric bezoar (4×2.5cm) that was successfully removed by endoscopic fragmentation with Coca-Cola injection. Compared with a single endoscopic fragmentation therapy, the combination therapy with Coca-Cola injection shortened the procedure time and reduced the complication associated with fragmented bezoar.
