        갑상선 양성 종양으로 갑상선 부분절제술을 시행한 환자에서 수술 전후 갑상선 기능의 변화

        Purpose: Lobectomy or subtotal thyroidectomy in patients with a benign thyroid tumor or goiter can give rise to hypothyroidism due to the reduced volume of the hormone-secreting thyroid gland. This study investigated the incidence of hypothyroidism in such patients and the clinical risk factors. Methods: One hundred seven patients who underwent partial thyroidectomy for benign thyroid tumor or goiter from January 2003 to February 2005 in our institution were reviewed retrospectively. Patients who had been preoperatively diagnosed with hyper- or hypothyroidism preoperatively were excluded. Postoperative hypothyroidism was defined as an elevated serum thyroid stimulating hormone (TSH) level >6.5ՌIU/L at about 6 months post- thyroidectomy. Results: The mean age of the 107 patients was 42.2 years. Ninety patients (84.1%) were female. Subtotal thyroidectomy was performed in 20 patients, lobectomy in 83 patients and enucleation in four patients. The most common pathologic diagnosis was nodular hyperplasia (86.0%). Postoperative hypothyroidism developed after surgery in 19 (21.8%) patients. Patients were evaluated for age, gender, preoperative TSH level, tumor size, tumor number, extent of the resection, thickness of thyroid isthmus and the presence of pathologic thyroiditis or thyroid autoantibody. Advanced age, elevated preoperative TSH level and extensive resection of the thyroid gland were significantly associated with postoperative hypothyroidism. Conclusion: Since many patients with a benign thyroid nodule can maintain a normal thyroid function even after thyroidectomy, preservation of more thyroid tissue during the operation is desirable, especially in young patients with a low-normal TSH level, unless the possibility of disease recurrence is high.         수도권 중소규모 종합병원 간호사 이직의도 영향요인

        The purpose of this study was to investigate the factors influencing nurse turnover intention in small and medium sized hospitals in the metropolitan area. Methods: A cross-sectional survey design was used. A questionnaire was distributed to the nurses in small and medium sized hospitals. The data of 209 nurses was analyzed using t-test, ANOVA, Pearson correlation coefficient and multiple regression. Results: Organizational commitment and age were identified as factors influencing turnover intention. These factors explained 56.8% of variance of turnover intention. Conclusion: Results suggest that establishment of organizational commitment needs to be discussed and continuously developed. Further study is also necessary to identify the key mechanism in nurse turnover intention.

        임상연구코디네이터의 역할수행에 대한 연구자와 임상시험모니터요원의 인식비교

        본 연구는 임상연구코디네이터와 함께 임상시험을 수행하는 연구자 및 임상시험모니터요원이 인식하는 임상연구코 디네이터의 역할수행 정도를 비교분석하기 위한 조사연구이다. 본 연구의 자료는 2011년 3월 7일부터 11월 30일까지 4개 병원의 연구자 39명과 임상시험모니터요원 68명을 대상으로 구조화된 설문지를 활용하여 수집하였고, 기술통계와 t-test를 이용하여 분석하였다. 연구자와 임상시험모니터요원 모두 임상시험에서의 임상연구코디네이터의 역할수행이 매우 중요하다 고 인식하고 있었고, 그 중에서도 대상자 관리 업무에의 역할 수행 비중이 가장 높다고 인식하고 있었다. 그러나 각 항목별 역할수행 정도에 관해 연구자 집단과 임상시험모니터요원 집단 간 인식 정도를 비교해 보면 차이가 나타났다. 따라서 임상연 구코디네이터의 역할수행에 대한 인력 간 인식 차이를 줄일 수 있는 보다 객관적인 임상연구코디네이터의 업무기술 및 지침 개발이 요구된다. The aim of this study was to compare the perceptions of the role performance of a clinical research coordinator (CRC) between the investigators and clinical research associates (CRAs). Data was collected using a structured questionnaire from 39 investigators and 68 CRAs in 4 hospitals from March 7 to November 30, 2011 and analyzed using descriptive statistics and a t-test. The investigators and CRAs were aware of the importance of CRC`s role and they all gave the highest values for patients care management. On the other hand, the score of the role performance items showed perceptional differences between investigators and CRAs. Therefore, the development of detailed job descriptions and manuals will be needed to reduce the perceptional differences in the role performance of the CRC.

        해당화 추출물의 주요물질 분석에 따른 폴리페놀 함량 및 항산화 활성 탐색

        해당화로부터 초음파 추출을 이용한 기능성 물질 생산 및 이를 통한 화장품용 소재 발굴을 위해 HPLC와 HPLC-MS/MS를 이용하여 추출물의 주요물질을 탐색하고 항산화 활성에 기반한 기능성을 평가하였다. 해당화 추출물의 HPLC 분석을 통해 해당화 추출물 내 갈산과 에피갈로카테킨 갈레이트가 각각 17.4와 4.35 mg/g 포함됨을 확인하었으며 HPLC-MS/MS 분석을 통해 해당 물질이 해당화 추출물 내 주요물질임을 재확인하였다. 항산화 활성을 평가하기 위해 TPC, TFC와 RSA를 분석한 결과 72.3±1.5 mg GAE/g DM, 11.2±0.7 mg QE/g DM과 87.9±0.3%로 확인되었으며 HPLC 분석을 통해 확인된 갈산과 에피갈로카테킨이 해당화의 TPC와 TFC에 기여하는 주요물질임이 확인되었다. 주요물질의 RSA를 측정한 결과 갈산과 에피갈로 카테킨 갈레이트가 해당화 추출물의 항산화 활성에 기여하는 주요물질임이 재차 확인되었다. 따라서, 해당화 추출물이 우수한 생리활성 효능을 가지는 갈산과 에피갈로카테킨 갈레이트의 함량이 높고 이에 따른 높은 폴리페놀 함량과 함께 우수한 항산화 활성이 확인됨에 따라 건강기능성 식품과 화장품 천연 항산화제 소재로써 부가가치를 창출할 수 있을 것으로 사료된다. The purpose of this study was to evaluate the antioxidant properties of Rosa rugosa extract and to identify which of its components are responsible for these properties. Reactive oxygen species play an important role in diseases such as cancer, arteriosclerosis, and heart disease as a consequence of increased metabolic rates, gene mutations, and relative hypoxia. Therefore, the antioxidant effect of R. rugosa extract was confirmed by HPLC, HPLC-MS/MS, the total polyphenol content, the total flavonoid content, and the radical scavenging activity. HPLC and HPLC-MS/MS analyses were conducted to identify and quantify the main components of the R. rugosa extract. Gallic acid and epigallocatechin gallate were identified as the main components, with 17.4 and 4.35 mg/g dry matter (DM), respectively. The antioxidant activity of R. rugosa extract was evaluated based on its total polyphenol content, total flavonoid content, and radical scavenging activity, which were 72.3 mg gallic acid equivalent/g DM, 11.2 mg quercetin equivalent/g DM, and 87.9%, respectively. The radical scavenging activities of the main components, gallic acid and epigallocatechin gallate, were 80.5% and 89.7%, respectively. Therefore, R. rugosa has a high polyphenol content and antioxidant activity, and it can be used as a natural antioxidant in food, cosmetics, and pharmaceuticals.

        임상연구코디네이터의 역할수행과 관련 요인

        Purpose: The purpose of this study was to analyze the role performance and related factors of the Clinical Research Coordinator (CRC). Methods: A quantitative, cross-sectional design was used. Data were collected using a structured questionnaire from 134 Clinical Research Coordinators in 4 hospitals and analyzed using descriptive statistics, t-test, ANOVA, and Pearson correlation coefficient. Results: The scores for role performance of CRC were high (average 336.51 points of a possible 420 points, mean average 5.74±0.67 of a possible 7) and the degree of role performance in the management task sub dimension (mean average 6.12±0.77 of a possible 7) was higher than any other sub dimension. Role performance of the CRC was also related to job satisfaction (r=.198, p=.023). Conclusion: The study results indicate that most CRC are nurses and perceive their role as very important. Further the CRC performance of activities of management task and the Coordination/Advocacy task sub dimension were high and higher levels of job satisfaction were related to higher levels of role performance of CRC was. Therefore, strategies related to improvement in the fringe benefit system and work environment should be developed to increase job satisfaction and thus increase job performance.
