        시각장애인의 안전한 독립보행을 위한 통합 서비스 플랫폼 연구 -대중교통 환승 경험 시나리오를 중심으로-

        오늘날 정보화와 기술의 발달로 다양한 모빌리티 서비스가 확산 중이며 기능과 편리성의 향상으로 대중들의 이동의 질이 상승하고 있다. 그러나 공공편의시설과 교통시설은 여전히 일반인들의 경험에 기반하여 확충된 곳이 많으며 장애인 중 특히 독립적인 보행에 어려움이 큰 시각장애인이 체감하는 현실적인 시설은 아직까지 부족하다고 할 수 있다. 길을 찾아갈 때 장애물 파악과 경로 파악 등 정보 파악이 용이하지 않은 등 여전히 장애물 등 정보 파악에 어려움이 있다. 또한 그로인한 심리적 불안감으로 독립적 보행이 어려운 환경임을 심층인터뷰를 통해 확인할 수 있었다. 이러한 어려움을 해결하기 위해 청각과 촉각 정보로 정보전달을 할 수 있는 방법을 중심으로 시각장애인 보행 보조기기 시스템에 대한 꾸준한 선행연구가 이루어지고 있으나 아직까지 보행보조기기 제품의 상용화와 활용이 잘 이루어지고 있지 않고 있다. 하지만 하나의 통합된 보행 보조기기 사용을 중심으로 청각과 촉각 정보를 주는 통합된 시스템 방식은 시각장애인이 보행 보조기기의 보조를 받아 길을 찾아갈 때 환경 정보 파악에 도움이 되는 것을 확인할 수 있었다. 이에 본 연구는 국내 시각장애인의 대중교통 환승 경험에서의 문제점에 기반하여 안전하고 독립적인 이동을 위해 통합된 보조시스템의 기능과 역할 활용에 관해 제안을 하는데 연구 목적을 두었다. 본 연구의 진행 과정은 보행보조기기 및 서비스 사례분석을 통해 시각장애인의 환승 경험 도중 개선이 필요한 기능과 요소를 확인한 뒤 발견하기(Discover), 정의하기(Define), 발전하기(Develop), 전달하기(Deliver) 서비스 디자인 프로세스 4단계의 방법론을 활용하여 진행한다. 결과적으로 본 연구는 시각장애인 중심의 환승 경험 시나리오 및 통합 보조시스템의 다섯 가지 서비스 컨셉들을 제안하였으며, 이는 시각장애인이 대중교통환승 시 독립적인 이동을 경험하는 데 유용하며 심리적 안정감을 주는데 적합함을 확인하였다. 이를 통해 최종적으로 시각장애인이 환승 전반의 단계에서 안전한 이동을 경험할 수 있는 통합 서비스플랫폼의 서비스 블루프린트와 상황별 맞춤 서비스를 제공받을 수 있는 앱 프로토타입을 제안하였다. 본 연구는 시각장애인 중심의 환승 경험 시나리오 검증을 통해 그 유용성을 확인했다는 것에 의의가 있으며, 이는 향후 사용자 경험을 중심으로 한 보행 보조기기 시스템 개발에 활용 될 수 있다. Today, various mobility services are spreading due to the development of informationization and technology, and the quality of public movement is increasing due to the improvement of function and convenience. However, public convenience facilities and transportation facilities are still expanded based on the experience of the general public, and there is still a lack of realistic facilities felt by the blind, especially those with independent walking difficulties. There are still difficulties in identifying information such as obstacles, such as identifying obstacles and identifying routes, as it is not easy to grasp information. It was also confirmed through an in-depth interview that it was difficult to walk independently due to psychological anxiety. In order to solve these difficulties, there has been a steady prior study of the blind walking aid system, focusing on ways to convey information with hearing and tactile information, but the commercialization and utilization of walking aid products has not been done well yet. However, an integrated system method that provides auditory and tactile information centered on the use of a single integrated walking aid could be found to help blind people understand environmental information as they navigate their way with a walking aid. The purpose of this study was to propose the use of the functions and roles of integrated auxiliary systems for safe and independent movement based on problems in the transit experience of public transportation for blind people in Korea. The process of this study is conducted using the methodology of the four stages of the Discover, Define, Develop, and Deliver service design process after identifying features and factors that need improvement during the transit experience of the blind through walk assistants and service case analysis. As a result, this study proposed five service concepts for blind-oriented transit experience scenarios and integrated assistive systems, which are useful for blind people to experience independent movement during transit and are suitable for psychological stability. Through this, we propose a service blueprint on an integrated service platform that allows blind people to finally experience safe travel throughout the transfer phase, and an app prototype that allows them to receive contextual customized services. This study is significant that it has confirmed its usefulness through the verification of blind-oriented transit experience scenarios, which can be utilized in the development of walking aid systems centered on future user experiences.

        Splanchnic artery aneurysms, previously thought to be rare, are being diagnosed with incresed frequency be- cause of development of noninvasive imaging techniques. The pathogenesis of visceral aneurysms is varied and may be secondary to arteriosclerosis(30%), trauma(25%), inflammation(11%), gestational alteration, surgery, infection, medial necrosis, collagen vascular disease, arteritis, and congenital anomalies. Clinical manifestation of visceral artery aneurysms is vague and not specific. The diagnosis and treatmet for them are difficult. Preoperative arteriography may be helpful in emergency situation to detect the aneurysm and visualize multiple aneurysms. Surgical ligation, resection and/or reconstructive surgery are first choice of treatment for them. Otherwise, transarterial embolization has also been reported for them recently. We report a case of hemoperitoneum due to left gastric artery aneurysmal rupture and a rare complication of splanchnic artery aneurysm Double rupture. The patient was treated successfully with simple ligation of the ruptured vessel.

        복막 투석환자에서 출구 감염의 역학-단일 기관 성적

        Background : Catheter-related infection is one of the most important causes of technical failure in peritoneal dialysis patients. We have examined the incidence and etiology of exit-site infection (ESI) at Keimyung University Dongsan medical center. Methods : Between January 2001 and December 2003, 292 new patients received peritoneal dialysis using double cuffed straight Tenckhoff catheter and were reviewed retrospectively. Results : In 292 patients, 81 (27.7%) patients experienced ESI and a total of 97 episodes of ESI has occurred during study period. The overall incidence of peritonitis during peritoneal dialysis was 0.27 episodes/patient-year. According to Twardowski`s classification, equivocal infection in 14.4%, acute infection in 68.1% and chronic infection in 17.5% were noted. Staphylococcus aureus was the most frequently isolated organism, followed by Staphylococcus epidermidis, and Pseudomonas aeruginosa. The duration of treatment was significantly longer in chronic infection group than acute infection group (44.6 days vs. 18.5 days, p<0.05). The rates of Pseudomonas infection (p<0.05) and catheter replacement (p<0.05) were significantly higher in the chronic infection group than in the acute infection group. There was one catheter loss due to refractory peritonitis and three deaths unassociated with ESI during mean follow-up of 20.3 months. Conclusions : Exit-stie infection is still a major causes of peritonitis and catheter failure. Because of suboptimal quality of practice guideline, additional studies on the definition, prevention and treatment of ESI are required.(Korean J Med 69:395-401, 2005)

        피부화상치유에서 줄기세포 표지자의 면역조직화학적 발현에 관한 연구

        Purpose: A cutaneous wound healing requires a well-orchestrated integration of the complex biological and molecular events of cell migration and proliferation, extracellular matrix deposition, angiogenesis and remodeling. Finally, skin regeneration is the main goal. Stem cells are self-renewing multipotent progenitors with the broadest developmental potential in a given tissue at a given time. The aim of this study was to examine the role of stem cells during the wound healing process of cutaneous burn in hairless mice by using immunohistochemical stainings (nestin, cytokeratin 15 and CD31). Methods: Each mouse received 2 burns at the dorsal area by applying a metal stick heated in boiling water. Burn wound sites were dressed with duoderm. The mice were sacrificed at 0, 2, 7, 14 and 21 days after burn. Histological findings and immunohistochemical expression for stem cell markers were observed. Results: Nestin was expressed in the stromal cells beneath the epidermis, hair follices, dermal cysts and endothelial cells. Cytokeratin 15 was expressed in the epidermis except in basal cells. On 7 and 14 days after burn, the regenerated epidermis didn’t express cytokeratin 15. CD31 was expressed in the endothelial cells on 7 and 14 days after burn. The amount of nestin expression was the highest. Conclusion: Our results showed that nestin may have various effects on burn wound healing. Cytokeratin 15 was expressed before burn and after burn. It is likely that other cytokeratin may stimulate epithelial regeneration. CD31 may act in vascular regeneration during burn healing.

        무모쥐의 피부화상에 대한 실크피브로인 필름 피복제의 치유 효능

        Purpose: The aim of this study was to examine the effects of silk fibroin film on wound healing of cutaneous burn in hairless mice by using microscopic findings and stem cell markers (nestin, cytokeratin 15) and ki-67 proliferation marker. Methods: Each mouse received two burns at the dorsal area by applying a metal rod heated with boiling water. Burn wound sites were dressed with Silk Fibroin Film and duoderm (SF group), Aquacel hydrofiber and duoderm (AC group) and duoderm only (Control group). All groups were covered externally with duodermas adhesive bands. Those mice were sacrificed at zero, two, seven, fourteen and twenty one days after burn. Histological findings and immunohistochemical staining for stem cell markers were observed. Results: In SF group, inflammatory cell infiltration, formation of granulation tissue and inflammatory foci are greater than in AC and control group. Those factors appear to enhance mesenchymal stem cell markers such as nestin. Finally mesenchymal tissue regeneration was enhanced. In addition, the length of ki-67 expressed re-generating epithelium, which appeared to be associated with epithelial regeneration, was the longest in SF group. Conclusion: The results show that the wound healing effect of SF is the best among other treatment materials including AC in the experimental group and duoderm in the control group through mesenchymal regeneration and epithelial regeneration which are essential factors for wound healing.

        This study aimed to analyze the master’s and doctoral theses in social pharmacy field over the past 10 years in Korea. Theses were searched based on the Korean National Assembly Digital Library System, and descriptive analyses were conducted using SAS. Among 669 theses, pharmacy/pharmacist accounted for 24.8%, followed by drug misuse with 16.9%, and pharmaceuticals industry related theses with 16.6% . Graduate school of pharmacy was responsible for a share of 27.2%, suggesting that approximately a quarter of the entire social pharmacy related theses are studies at the school of pharmacy.

        Genetic Variations of Outer Membrane Protein Genes of Vibrio harveyi Isolated in Korea and Immunogenicity of OmpW in Olive Flounder, Paralichthys olivaceus

        Vibrio harveyi is a pathogenic marine bacterium causing systemic symptoms resulting in mass mortalities in fishes and shrimps in aquaculture. Outer membrane proteins(OMPs) are related to the pathogenicity and thus good targets for diagnosis and vaccination for Gram negative bacteria. Recently vaccination strategies using the OMPs have been suggested to control vibriosis in several fish species. In this study, we have isolated V. harveyi from diseased marine fishes from different regions of Korea and investigated genetic variations of four OMP genes including OmpK, OmpU, OmpV and OmpW. Consequently, OmpK and U genes could be divided into 3 subgroups of type I, II, III and type A, B, C, respectively, without any correlation with geographical regions and species while OmpV and W were highly homologous. OmpW gene of V. harveyi FP4138 was fully sequenced and predicted the deduced amino acid sequence to form β-barrel with hydrophobic channel. Indeed, the immunogenicity of recombinant OmpW produced in Escherichia coli was assessed by vaccinating flounder. As a result, the high antibody response with antibody titer of 4.2±0.7 and protection with relative percent survival of 60% against artificial infection of V. harveyi were demonstrated. This result indicates that OmpW is a virulence related factor and it can be a vaccine candidate to prevent a high mortality caused by V. harveyi infection in olive flounder, Paralichthys olivaceus.
