봄철각결막염 환자의 Shield Ulcer에서 양막이식 1예

Purpose: Human amniotic membrane has been used to variable ocular surface disorder. We report a case of the usefulness of amniotic membrane transplantation (AMT) for vernal keratoconjunctivitis "Shield ulcer". Methods: A 12-year-old male patient was diagnosed to have vernal keratoconjunctivitis "Shield ulcer" and treated with medical therapy for four weeks. But he showed severe itiching, foreign body sensation, decreased visual acuity and continued to corneoconjunctival lesion and then, Amniotic membrane transplantation with stromal side down was performed after debridement of the ulcer bed and therapeutic contact lens was applied. Results: At tenth day after AMT, ulcer size was decreased and corneal state was generally improved with restoration of visual acuity. One month after, he has been comfortable with improved visual acuity, giant papillae of upper tarsal conjunctiva was nearly complete remission. Conclusions: AMT is an effective method for managing vernal keratoconjunctivitis "Shield ulcer", that are refractory to conventional medical treatment. 목적 : 인간 양막은 다양한 각막표면 질환에 사용되어 왔다. 이에 저자들은 봄철각결막염의"shield ulcer"가 있는 환자에서 양막 이식술로 성공적인 치료를 시행한 1예를 보고하는 바이다. 대상과 방법 : 봄철각결막염의 shield ulcer 진단 하에 4주 동안 약물치료를 받은 12세 남자환자에서 시력저하와 함께 심한 이물감과, 소양감 및 각결막 병변이 지속되어, 궤양부위를 긁어낸 후 양막 간질부위를 각막 쪽으로 양막 이식을 시행하고, 콘택트렌즈를 착용케 하였다. 결과 : 수술 후 10일째 시력의 호전과 함께, 궤양의 크기가 감소하였으며, 전반적인 각막의 상태가 좋아졌고 한 달 후에는 눈꺼풀판결막의 거대유두가 완전히 사라졌으며, 증상의 소실과 함께 시력도 정상으로 회복되었다. 결론 : 봄철각결막염의 shield ulcer에 대한 양막이식은 약물치료에 반응을 하지 않는 경우 좋은 치료법이 될 것으로 사료된다.

In this study the structure analysis of tapping m/c is developed to improve dynamic characteristics. To this purpose a tapping m/c is mathematically modelled and dynamic response analysis of the model are investigated by use of Finite Element Method(FEM). The investigation procedures are as follows. The finite element modelling of the tapping m/c is done with triangular and rectangular plate elements and each node of the elements has 6 degrees of freedom. And this model is idealized as a forced vibration system under impulse load by cutting force. Model parameters, natural frequency and mode shape, and dynamic response for displacement and stress are analyzed by use of mode superposition method. To reduce computing time of dynamic response analysis, lower vibration mode is analyzed with master-DOF and mass participation factor. In this case maximum relative displacement is occured at 17.483 Hz in the right column.
