폐 종양 FOG PET 영상의 다양한 추적자 역학 분석 방법 개발과 유용성 고찰

Purpose: The purpose of this study was to assess the diagnostic accuracy of various quantitation methods using F-18-fluorodeoxyglucose (FDG) in patients with malignant or benign lung lesion. Materials and Methods: 22 patients (13 malignant including 5 bronchoalverolar cell cancer; 9 benign lesions including 1 hamartoma and 8 active inflammation) were studied after overnight fasting. We performed dynamic PET imaging for 56 min after injection of 370 MBq (10 mCi) of FDG. Standardized uptake values normalized to patients body weight and plasma glucose concentration (SUVglu) were calculated. The uptake rate constant of FDG and glucose metabolic rate were quantified using Patlak graphical analysis (Kpat and Mrpat), three compartment-five parameter model (K5p, MR5p), and six parameter model taking into account heterogeneity of tumor tissue (K6p, MR6p), Areas under receiver operating charac-teristic curves (ROC) were calculated for each method. Results: There was no significant difference of rate constant or glucose metabolic rate measured by various quantitation methods between malignant and benign lesions. The area under ROC curve were 0.73 for SUVglu, 0.66 for Kpat, 0.77 for Mrpat, 0.71 for KSp, 0.73 for MRSp, 0.70 for K6p, and 0.78 for MR6p, No significant difference of area under the ROC curve between these rne- thods v;as observed except the area between Kpat vs. Mrpat (p<0.05). Conclusion: Quan- titative methods did not improve diagnostic accuracy in comparison with nonkinetic methods. However, the clinical utility of these methods needs to be evaluated further in patients with low pretest likelihood of active inflammation or bronchoalveolar cell carcinoma. Purpose: The conventional gamma camera is not ideal for scintimammography because of its large detector size ( 500 mm in width) causing high cost and low image quality. We are developing a small gamma camera dedicated for breast imaging. Materils and Methods: The small gamma camera system consists of a NaI (Tl) crystal (60 mm X60 mm X6 mm) coupled with a Hamamatsu R3941 Position Sensitive Photomultiplier Tube (PSPMT), a resister chain circuit, preamplifiers, nuclear instrument modules, an analog to digital converter and a personal computer for control and display. The PSPMT was read out using a standard resistive charge division which multiplexes the 34 cross wire anode channels into 4 signals (X, X, Y, Y ). Those signals were individually amplified by four preamplifers and then, shaped and amplified by amplifiers. The signals were discriminated and digitized via triggering signal and used to localize the position of an event by applying the Anger logic. Results: The intrinsic sensitivity of the system was approximately 8,000 counts/.'ec/pCi. High quality flood and hole mask images were obtained. Breast phantom containing 2-7 mm diameter spheres was successfully imaged with a parallel hole collimator. The image displayed accurate size and activity distribution over the imaging field of view. Conclusion: We have succesfully developed a small gamma camera using NaI(T1)-PSPMT and nuclear instrument modules. The small gamma camera developed in this study might imprnve the diagnostic accuracy of scintimammography by optimally imaging the breast. (Korean J Nucl Med 1998;32:365-73)
N - ( 4 - [ 18F ] Fluoromethylbenzyl ) spiperone : 유력한 도파민 D2 수용체 선택성 방사성리간드

We evaluated the in vivo kinetics, distribution, and pharmacology of N-(4-[F] fluorornethylbenzyl)spiperone ([F]FMBS), a newly developed derivative of spiperone, as a potentially more selective #radiotrar.er for the dopamine (DA) Dz receptors. Mice received 1.9-3.7 MBq (1.8-3.6 nmol/kg) of [F]FMBS by tail vein injectivn. The time course and regional distribution of the tracer in brain were assessed. Blocking studies were carried out by intravenously preinjecting DA Dp receptor blockers (spiperone, butaclamol) as well as drugs with high affinity for DA Dr lSCH 23390), DA transporter (GBR 12909), and serotonin Sp (5-HTz) (ketanserin) sites. After injection of the tracer, the radioactivity in striatum increased steadily over time, resulting in a striatal-to-cerebellar ratio of 4.8 at 120 min postinjection. By contrast, the radioactivity in cerebellum, frontal cortex, and remaining cortex washed out rapidly. Preinjection of unlabe1ed FMBS (1 rng/kg) and spiperone (1 mg/kg) reduced [F] FMBS striatal-to-cerebellar ratio by 41Zo and 80Ya, respectively. (+)-Butaclamol(1 mg/kg) blocked 80Yo of the striatal [F]FMBS binding, while (-)-butaclamol (1 rng/kg) did not. Preinjection of SCH 23390 (1 mg/kg) and GBR 12909 (5 mg/kg) had no significant effect. On [F]FMBS binding. Ketanserin (1 mg/kg), a ligand for the 5-H1g receptors, did not cause significant inhibition either in striatum, in frontal cortex, or the remaining cortex. The results demonstrate that [F]FMEtS labels DA Dz receptors selectively in vivo in the mouse brain. It may hold promise as a selective radiotracer for studying DA Dz receptors in vivo by PET.

[ $[^{123}I]{\beta}$ ]-CIT [$2{\beta}$-carbomethoxy-$3{\beta}$-(4-iodophenyl)tropane]는 도파민 운반체 (dopamine transporter)에 특이결합하며 $[^{123}I]{\beta}$-CIT의 도파민 운반체 결합정도는 파킨슨병에서 도파민 뉴우런의 변성정도를 반영하는 것으로 제안되어 왔다. 이 연구의 주요 목적은 파킨슨병 환자에서 $[^{123}I]{\beta}$-CIT SPECT를 이용하여 측정된 $[^{123}I]{\beta}$-CIT의 선조체 결합지표들이 질병의 임상적 진행정도를 반영하는지를 검토하고, 간편화된 조직방사능비가 $[^{123}I]{\beta}$-CIT의 결합정도를 나타내는 정량적 지표로 이용될 수 있는지를 검증하는 것이었다. 파킨슨병 환자 30명($59{\pm}9$세, 평균${\pm}$표준편차: Hoehn-Yahr stage 1-3)과 정상인 6명 ($58{\pm}5$세)을 대상으로 $[^{123}I]{\beta}$-CIT SPECT 영상을 얻었다. $[^{123}I]{\beta}$-CIT 선조체 결합의 정량적 지표로서 (선조체 방사능-소뇌방사능)/소뇌방사능 비(specific binding ratio, SBR)와 추적자역학모델을 이용하여 측정한 선조체 결합능(binding potential)($k_3/k_4$)을 구하였다. 파킨슨병 환자에서 $[^{123}I]{\beta}$-CIT의 선조체 결합역학은 정상인에 비하여 현저하게 느렸으며 그 결합지표들은 정상인에 비하여 뚜렷하게 낮았다. 한편, 편측파킨슨병 환자에서 $[^{123}I]{\beta}$-CIT 결합은 증상 반대쪽 선조체 뿐만 아니라 같은 쪽 선조체에서도 정상인에 비해 유의하게 감소되어 있었다. 파킨슨병 환자에서 $[^{123}I]{\beta}$-CIT 투여 후 24시간의 선조체 SBR 및 최대 SBR, 선조체 결합능은 모두, 유병기간, Hoehn-Yahr stage, UPDRS(Unified Parkinson's Disease Rating Scale) 총점, UPDRS 운동점수, UPDRS 일상활동점수와 유의한 상관관계를 나타내었다. 24시간 선조체 SBR과 최대 SBR은 선조체 결합능과 우수한 상관관계를 보였다. 이상의 결과로부터 $[^{123}I]{\beta}$-CIT의 선조체 결합은 파킨슨병의 진행정도를 나타내는 지표로 이용될 수 있다. 또 $[^{123}I]{\beta}$-CIT 투여 후 24시간 영상으로부터 얻은 간편화된 조직방사능 비는 $[^{123}I]{\beta}$-CIT의 결합정도를 정량적으로 반영한다. $[^{123}I]{\beta}$-CIT SPECT는 파킨슨병의 조기진단 및 진행 추적에 임상적으로 유용할 것으로 판단된다.
알쯔하이머병에서 양전자방출단층촬영을 이용한 국소뇌포도당대사의 변화에 관한 연구

N/A The purpose of the present study was to validate the use of tissue radioactivity ratios instead of regional metabolic rates for the assessment of regional metabolic changes in Alzheimer's disease(AD) with [18F]FDG PET and to examine the correlation of ratio indices with the severity of cognitive impairment in AD. Thirty-seven AD patients(age 68±9 yrs, mean±s.d.; 36 probable and 1 definite AD), 28 patients with dementia of non-Alzheimer type(age 66±7 yrs), and 17 healthy controls(age 66±4 yrs) underwent [18F]FDG PET imaging. Two simplified radioactivity ratio indices were calculated from 37-66 min image: region-to-cerebellar radioactivity-ratio(RCR) and a composite radioactivity ratio(a ratio of radioactivity in the most typically affected regions over the least typically affected regions: CRR). Local cerebral metabolic rate for glucose(LCMRglu) was also measured using a three-compartment, five-parameter tracer kinetic model. The ratio indices were significantly lower in AD patients than in controls(RCR in temporoparietal cortex, 0.949±8.136 vs. 1.238±0.129, p=0.0004; PCR in frontal cortex, 1.027±0.128 vs. 1.361±0.151, p〈0.0001; CRR, 0.886±0.096 vs. 1.032±0.042: p=0.0024). On the RCR analysis, 86% of AD patients showed a pattern of bilateral temporoparietal hypometabolism with or without frontal involvement; hypometabolism was unilateral in 11% of the patients. When bilateral temporoparietal hypometabolism was considered to be suggestive of AD, the sensitivity and specificity of the RCR was analysis for the differential diagnosis of AD were 86% and 73%, respectively. The RCR was correlated significantly with the macroparameter K [K1k3/(k2+k3)] (r=0.775, p〈0.0001) and LCMRglu(r=0.633, p=0.0002) measured using the kinetic model. In patients with AD, both average RCR of cortical association areas and CRR were correlated with Mini-Mental Status Examination(r=0.565, p=0.0145; r=0.642, p=0.0031, respectively), Clinical Dementia Rating(r=-0.576, p=0.0124; r=-0.591, p=0.0077), and total score of Mattis Dementia Rating Scale (r=0.574, p=0.0648; r=0.737, p=0.0096). There were also significant correlations between memory and language impairments and corresponding regionl RCRs. The results suggest that the [18F]FDG PET ratio indices, RCR and CRR, reflect global and regional metabolic rates and correlate with the severity of cognitive impairment in AD. The simplified ratio analysis may be clinically useful for the differential diagnosis and seral monitoring of the disease.

N/A About one-third of radiologically indeterminate solitary pulmonary nodules (SPN) are eventually turned out to be malignant. It is very important to noninvasively determine whether the SPN is malignant or not for the decision of its way of management. PET imaging is highlighted by its unique ability of imaging the function and metabolism of cells. Glucose metabolism is increased in malignant transformed cells. We performed FDG-PET studies in patients who ha radiologi- cally indeterminate SPN and compared the findings with histologic diagnoses to assess the diagnostic accuracy in the detection of malignancy and to decide which parameter is the most suitable for clinical practice among peak SUV (pSUV), average SUV (aSUV), 50/10 ratio, and time-activity curve (TAC). Thirty patients were included in this study and the most useful parameter was pSUV. The sensitivity and specificity in the detection of malignant SPN using 3.5 as a cut off pSUV were both 87%. Interestingly, all 2 false-negative cases were bronch- ioloalveolar carcinoma on histologic examination. If these cases, which could be strongly suspected by CT findings, were excluded, the sensitivity of pSUV was 100%. In conclusion, PET imaging is very helpful for determining malignancy in indeterminate SPN and pSUV is a conveniently measurable parameter which is valuable for interpretation.
