Purpose: This study examined the effects of swimming exercise and Achyranthes Radix extracts on the inflammatory and behavioral responses in type II collagen-induced arthritic rats for 28 days. Methods: Sprague-Dawley rats were allocated randomly to one of the following four groups: only type II collageninduced (group Ⅰ), application of swimming exercise after type II collagen-induced (group II), application of Achyranthes Radix ointment after type II collagen-induced (group III), application of swimming exercise and Achyranthes Radix ointment after type II collagen-induced (group IV). Arthritis was established in SD rats by an intradermal injection of Chick type II collagen plus incomplete Freund's adjuvant at the base of the tail of the animals. The swimming exercise program consisted of a 25 min swimming session/day with a load corresponding to 5.5% of the weight bearing, three days/week for four weeks. The Achyranthes Radix ointment (0.1g) was applied twice a day for five days. The changes in behavior, H & E stain, and cyclooxygenase-2 (COX-2) level in the knee joint were assessed. Results: The gross and histological examination, after RA induction showed reddening, edema and erythema. The H & E stain revealed the destruction of articular cartilage, bony erosion and the infiltration of inflammatory cells after RA induction. The mechanical allodynia test results were significantly higher in group I than in groups II, III and IV (p<0.01). The immunohistochemistrical response of COX-2 in the knee joint showed that groups II, III, IV had a lower response effect than group I. Conclusion: Swimming exercise training and Achyranthes Radix ointment decreased the inflammatory responses and enhanced the behavioral responses in the arthritic rats.
수중운동이 허혈성 뇌졸중 유발 백서의 하지근육 및 운동기능회복에 미치는 영향

Purpose: This study is intended to examine the aquatic exercise on the improvement of muscle atrophy and motor function in an ischemic stroke model induced by middle cerebral artery occlusion. Methods: We used 60 Sprague-Dawely rats which were divided into 4 groups; the subjects were divided into group of 5 rats. Group I was a group of high dose aquatic exercise after inducing ischemic stroke; Group II was a group of low dose aquatic exercise after inducing ischemic stroke; Group III was a control group, Group IV was a sham group without ischemic stroke. Results: Muscle weight of gastrocnemius muscle was significantly difference in Group II compared to Group III on 8 weeks(p<0.05). For the changes in relative muscle weight of gastrocnemius muscle, there was significant increase in Group II compared to Group III on 8 weeks(p<0.05). For neurologic exercise behavior test, Group II generally had the highest score, compared to other groups. The results of behavior test that Group II improved in degeneration and inflammation of muscle fiber and decreased in destruction of nerve cells and cerebral infarction, indicating a similar state of muscle fiber and brain to Group III. Conclusion: Based on these results, aquatic exercise may improve muscle atrophy and contribute to the improvement of motor function.
뇌졸중 환자의 가상현실 프로그램이 상지기능에 미치는 영향 : 메타분석

본 연구의 목적은 메타분석을 통해 가상현실 프로그램이 뇌졸중 환자의 상지기능에 미치는 효과크기를 알아보고자 한다. 데이터베이스는 한국교육학술정보원(RISS), 한국학술정보(KISS), 국립중앙도서관, 한국학술지인용색인(KCI), 국가과학기술정보센터(NDSL)를 활용하였다. 2010년 1월부터 2019년 6월까지 가상현실 프로그램을 실시한 선행연구들을 조사하였다. 본 연구는 PICO(Patient, Intervention, Comparison, Outcome, PICO)기준에 의거하여 최종 14편을 선정하여 메타분석을 실시하였다. 질 평가도구로 RCT(Randomized Control Trials) 연구는 RoB(Risk of Bias) 도구, NRCT(Non-Randomized Control Trials) 연구는 RoBANS(Risk of Bias Assessment tool for Non-randomised Study) 도구를 사용하였다. 선정된 각 연구들의 효과크기를 산출하기 위해 CMA 3.0프로그램을 사용하였다. 추가적으로 하위집단분석과 메타회귀분석 및 출판편의 분석을 실시하였다. 가상현실 프로그램이 상지기능에 대한 전체 효과크기는 Hedges’s=0.390 (95 % CI: 0.192~0.587)으로 확인되었다(p<.05). 가상현실 프로그램은 뇌졸중 환자의 상지기능에 긍정적인 영향을 미친다. 따라서, 4차 산업에 발맞추어 뇌졸중 환자를 위한 다양한 가상현실 프로그램과 산학 협력 기술 개발이 요구된다. 향후 가상현실 프로그램에 대한 RCT연구들과 세부적인 상지기능에 대한 연구가 질적 및 양적으로 확보되어야 할 것이다. This study examined the effect size of virtual reality programs on the upper extremity function of stroke patients through a meta-analysis. Databases, such as the Research Information Sharing Service, the Korean Studies Information Service System, the National Library of Korea, the Korean Citation Index, and the National Digital Science Library, were used. Previous articles were surveyed for virtual reality programs between January 2010 and June 2019. A meta-analysis was performed by selecting the final 14 studies based on the PICO standard. The RoB and RoBANS tools were used as quality assessment tools for randomized and non-randomized control trials, respectively. The CMA 3.0 program was used to calculate the effect size of each study. Sub-group analysis, meta-regression analysis, and publication bias were performed. The total effect size of the virtual reality programs on the upper extremity function was Hedges"s g=0.390 (95% CI: 0.192~0.587) (p<.05). The virtual reality program positively affects the upper extremity function of stroke patients. Therefore, the development of various virtual reality programs and industry-academia cooperation technology for stroke patients is required in accordance with the fourth industry. Randomized control trials and detailed upper extremity function studies for virtual reality programs will be needed in follow-up studies.
