N/A In order to evaluate the effectiveness of the external peritoneovenous shunt in controlling the ascites, we have performed the procedure in three patients with intractable ascites. The bedside external peritoneovenous shunt had several advantages over the LeVeen shunt. The summary of the results were as follows; 1) A surgeon and operating room are not required. 2) Peritoneal dialysis catheter can be easily inserted and removed. 3) The abdominal binder is not required. 4) The infusion of the ascitic fluid is under precise control. 5) Definitive short-term therapy. Conclusively, the external peritoneovenous shunt is a very easy procedure to perform and its trial is of value in the medical treatment of intractable ascites.
Rifampin으로 유도된 Addisonian Crisis

Rifampin is an inducer of hepatic drug metabolizng enzymes. This results in interaction with several drugs metabolized through the liver which is known to be the major organ reponsible for steroid inactivation. Thus concurrent treatment of rifampin and glucocorticoid hormone can lead to decreased bioavailability of glucocorticoid hormone and to increased requirement. We report a case of rifampin-induced adrenal crisis who has received a physiologic maintenance dose of priednisolon and rifampicin. It is therfore recomended that the steroid replacement needs to be adjusted according to the clinical response and serum electrolyte concentrations. This may require three to fourforld increment of the daily dosage of steroids.
갑상선 기능 변화에 따른 갑상선 호르몬 수용체 c-erb A mRNA의 발현

To study whether thyroid hormone regulates triiodothyronine (T3) receptor at the gene level, total RNA was isolated from peripheral leukocytes in patients with hyperthyroidism and from the liver tissue of thyroxine induced hyperthyroid rat or propylthiouracil induced hypothyroid rat. mRNA expression was examined by Northern hybridization technique using c-erb A complementary DNA probe. C-erb A mRNA of approximately 2.0 kilobase size in length were expressed from peripheral leukocyte and from rat liver tissue. The degree of expression of c-erb A mRNA were negatively correlated with the level of thyroid hormone (T3 and T4 respectively) in patients with hyperthyroidism. In rat liver tissue the c-erb A expression were increased in both hyperthyroid and hypothyroid status as compared to euthyroid status. To explore the mechanism of biologic action of thyroid hormone, further studies to the relations between other c-erb A gene and thyroid hormone in various tissues will be needed.
