The Wenckebach phenomenon is recognized as the progressive prolongation of the PR interval and the eventual transmission failure of an atrial beat to reach the ventricles. Although this phenomenon is usually regarded as a disease at the AV-node level, It may occur in any level of cardiac conduction system. We describe a case of Wenckebach phenomenon at the ventricular myocardium which was demonstrated to be due to propafenone overdose.

Hyperthyroidism is associated with increased plasma levels of procoagulant and antifibrinolytic clotting factor, including fibrinogen and plasminogen activator inhibitor-1 (PAI-1) (1,2). Arterial thrombosis in thyrotoxicosis is mainly resulted from atrial arrhythmias and the overall incidence of thromboembolism varies from 8% to 40% (3). There are many reports about venous thrombosis related to the thyrotoxicosis, however, there are few cases about arterial thrombosis combined with thyrotoxicosis independent of atrial arrhythmias. Herein, we report a case of Graves` disease who presented with renal artery infarction independent of atrial fibrillation.

리튬나이오베이트 결정에 Ti를 열확산시켜 광도파로를 만드는 방법에서 문제가 되는 out-diffusion(삼출) 문제를 해결하는 방안으로 확산과정 이전에 SiO₂ 박막을 시료 위에 입히고 Ti을 열확산시켜 광도파로를 제조하는 방법을 제안하였으며, 기존의 방법에 의해서 만들어진 광도파로와 본 연구의 방법에 의해서 만든 광도파로의 근시야상(mear field pattern)을 비교하였다. We report a new method of fabricating a Ti:LiNbO₃ waveguide with no out-diffusion by coating the waveguide with SiO₂ thin film. It was coated before diffusion process, and the Li₂O out-diffusion was prevented in the diffusion process. We compare the near field patterns of the guided modes between the typical waveguide and the waveguide fabricated by new method proposed here.

Hemorrhage fever with renal syndrome induced by hantavirus usually has the clinical manifestations such as renal impairment and fever. The mechanism of hemorrhage fever with renal syndrome is uncertain, but is speculated due to vasculitis and immune system. This immune system can damage to endothelial cells and the renal parenchymal. In this paper, We report a case of hemorrhage fever with renal syndrome with antineutrophilic cytoplasmic antibody-positive and antiglomerular basement membrane antibody-positive and try to discuss about path ogenesis by the immune system.

40 Gbit/s 속도의 시분할 다중화(OTDM)된 광펄스 신호열로부터 반도체 광증폭기의 4광파 혼합 신호에 포함된 위상정보를 이용하여 10㎓로 위상 동기된 진동자 신호를 추출하였다. 제작된 위상 동기 회로는 5시간이상 안정되게 동작되었고, 위상 동기 주파수의 작동범위는 입력 광펄스의 기본 주파수의 10㎑ 이내로 측정되었다. A new phase lock loop (PLL) is proposed and demonstrated for clock recovery from 40 Gbit/s time-division-multiplexed (TDM) optical pulse train. The proposed clock recovery scheme improves the jitter effect coming from the clock pulse laser of harmonically-mode locked fiber laser. The cross-correlation frequency component between the optical signal and an optical clock pulse train is detected as a four-wave-mixing (FWM) signal generated in SOA. The lock-in frequency range of the clock recovery is found to be within 10 KHz.

3준위 레이저 rate equation 및 overlap integral로부터 파장 1.48 ㎛에서 여기된 Er³+ 첨가 광섬유 광증폭기를 위한 광섬유 매개 변수의 최적 조건을 계산하였다. 이 계산으로부터 Er³+ 첨가 광섬유 광증폭기의 소신호 이득(small signal gain) 특성을 개구수 (N.A.), V값, 광섬유 길이, 차단 파장(cutoff wavelength) 등의 함수로 알아 보았으며 또한, 최대 소신호 이득을 갖기 위한 광섬유 매개 변수를 결정하였다. We carried out the useful theoretical calculation for the optimum design of a 1.48 ㎛ diode laser pumped Er³+ doped fiber amplifier. The model we established is based on the rate equations of three level laser system and the overlap integral between fundamental mode LP_(01) and Er³+ doped area.<br/> We determined several fiber parameters (N.A., V value, fiber length, Er³+ concentration, cutoff wavelength etc.) for the optimum design of a high optical gain. We found that our theoretical results are very useful to the design of Er³+ doped fiber used in EDFA.

Percutaneous renal biopsy is a relatively safe procedure; however, about 10% to 30% of patients experience major or minor bleeding complications. Such complications are associated with age, sex, prolonged blood coagulation profiles, hypertension and renal dysfunction. In this report, we present the perirenal hematoma after renal biopsy in uncooperative patients. Clinicians should bear in mind that lack of cooperation during procedures is a risk factor for bleeding complications, and the importance of close observation for 24 hours after procedures should not be overlooked.
만성변비 환자에서 직장항문 기능검사의 진단적 응용 가치

Background/Aims: The pathophysiology of chronic constipation is more complex than just simple disorders of the pelvic floor muscle or colon. This study was designed to assess the diagnostic usefulness of physiologic investigation in patients with chronic constipation and to analyze the characteristics of physiologic findings of obtained diagnosis. Methods: Anal manometry (n=82), cinedefecography (n=106), anal EMG/PNTML (pudendal nerve terminal motor latency; n=27) and colonic transit time study (n=35) were performed on the patients with chronic constipation. Physiologic findings of patients who recieved at least two items of physiologic tests (n=106, 68 females and 38 males) were retrospectively evaluated. Age matched control data (n=33) in anal manometry and sex matched control data (n=20) were selected from volunteers. Results: Patients were categorized as rectocele (Group I: n=36), nonrelaxing puborectalis syndrome (Group II: n=14), anal dyschezia (Group III: n=14), rectoanal intussusception (Group IV: n=8), encopresis (Group V: n=7) and others (n=27). In anal manometry, the patients with group III showed higher mean and maximum resting pressures (p$lt;0.001) and higher mean squeeze pressures and maximal voluntary contraction (p$lt;0.01) than the patients with other groups. In cinedefecography, patients with group II showed smaller anorectal angle at strain (p$lt;0.001) and at dynamic change between rest and strain (p$lt;0.05) than the patients with other groups. In neurologic study, mean value of PNTML for all patients was 2.35±0.44 (range, 1.66-3.66) msec. The value of PNTML was increased in proportion to the ages of patients (r=0.46, p=0.003). Conclusions: These results suggest that anorectal physiologic studies are vital to the functional diagnosis for the patients with constipation. Current data provide the important diagnostic ramification, and can be used for clinical appraisal.
