Prazosin 수술전 처치후 갈색세포종 마취관리에 대한 임상적고찰

Perioperative management of patients with pheochromocytoma is challenging. Accordingly, proper preoperative preparation is important. Prazosin, a selective alpha I blocker, may offer a potential advantage. This 54-year-old woman was treated with prazosin 2 mg, b.i.d. for 15 days and also with propranolol 20 mg, b.i.d. for a few days intermittently before the proposed surgery. Both symptoms and blood pressure were well controlled effectively. Induction of anesthesia was accomplished with nitrolingual spray, fentanyl 100 μg, 1% idocaine 50 mg, 2.5% thiopental sodium 200 mg, vecuronium 6 mg and 100% O₂-enflurane. During the surgical and anesthetic procedure, the patient showed a reduced incidence of excessive blood-pressure variations and no arrhythmia was present except for supraventricular ectopic beats.

최근 휴대폰을 이용해 URL을 입력하여 PC와 동일한 웹사이트에 접근할 수 있는 ‘풀 브라우징(Full Browsing) 서비스’가 전 세계적으로 각광받으며 급성장하고 있다. 그러나 급격한 시장 성장 수준에도 불구하고 서비스에 대한 평가기준이나 평가척도의 개발은 느리게 진행되고 있어, 사용자의 요구사항과 서비스 만족도에 대한 평가기준의 마련이 요구되고 있다. 따라서 본 연구에서는 풀 브라우징 서비스 사용자에 대한 실증연구를 통하여 풀 브라우징 서비스에 대한 고객 만족도 평가체계를 제시하였다. 이는 사용자가 풀 브라우징 서비스에서 중요하게 인식하는 요소들이 무엇인지 제시하고, 향후 고객 유지 및 신규 고객 창출에 영향을 미치는 마케팅 전략 수립에 활용할 수 있을 것으로 기대된다.

It has been reported that risk of death caused by cardiovascular diseases (CAD) in women markedly increases with the onset of menopause. Low levels of cardiorespiratory fitness are associated with high CAD and all-cause mortality. However, the effects of circuit exercise training in sedentary postmenopausal women with prehypertention are not studied. Therefore, the purpose of this study was to investigate whether the cardiovascular responses to increasing exercise intensity were differ in postmenopausal women participating in an exercise program. Fourteen prehypertensive postmenopausal women were recruited for this study. All subjects underwent a maximal graded exercise testing. Following at least 48 hours, all subjects reported to the laboratory and were instrumented to measure the cardiovascular responses at rest and during exercise. The experimental procedures repeated after exercise training program. Systolic blood pressure were significantly decreased at rest and 40% of VO2max and diastolic blood pressure at 60% of VO2max, and mean arterial pressure at 40% and 60% of VO2max, after exercise training program. There were no differences in both stroke volume and cardiac output responses, where heart rate was significantly decreased at 40% if VO2max. Total vascular conductance was significantly increased at 60% of VO2max. Thus, attenuated blood pressure responses were mainly mediated via increases in total vascular conductances. This study suggests that the decreases in blood pressure response in postmenopausal women after exercise program may be due to improvement in function of exercise presser reflex.

본 논문에서는 3축 공작기계의 디지털 트윈을 구성하고, 이를 이용하여 가공 시간 또는 생산 원가를 줄이기 위한 최적의 가공 조건을 도출하는 방법을 제안하였다. 제어기, 이송계의 모델과 가공 중 발생하는 절삭부하를 예측하기 위한 모델을 도출하고, 이를 통합하여 디지털 트윈을 구축하였으며, 가공실험을 통해 그 정확도를 검증하였다. 검증된 디지털 트윈을 기반으로 유전 알고리즘을 이용하여 제한된 절삭력을 넘기지 않으며 가공 시간 또는 생산 원가 저감을 위한 최적 가공 조건을 도출하고, 실험을 통해 도출된 가공 조건이 적절함을 판단하였다. 또한 디지털 트윈을 이용하여 절삭력 한계 값과 공구 비용 등 최적화 목적 함수에 포함되는 변수가 최적 가공 조건에 미치는 영향을 분석하였다. This study proposes a new method to optimize machining conditions based on a digital twin of the 3-axis machine tool. A digital twin composed of the physical models of a controller, feed drive systems, and cutting force is constructed and evaluated based on machining experiments. A genetic algorithm is utilized to find machining conditions to minimize machining time and production cost based on the digital twin. The optimal feed rate and spindle speed are obtained for each line of the part program when the cutting force is limited. Machining results are presented to demonstrate the effectiveness of the proposed method. Furthermore, the effects of limited cutting force and tool cost on optimal machining conditions are investigated by using the digital twin.

The anesthetic experience in 20 years was evaluated by statistically analyzing the total of 17,595 cases which were performed at S.N.U.H. from 1960 through 1979 according to age, sex, anesthetic agents and methods. To simplify the analysis, the authors selected the anesthetic cases of 1960, 1965, 1970, 1975, and 1978. The results were as follows: 1) General anesthesia has been used with increasing frequencies and local anesthesia with decreasing frequencies during the period. 2) More than half of the totaI cases were performed for the patients in their second or third decades. 3) General surgery cases were the majority of the total. 4) The usage of halothane has been increased evei since its introduction into clinical anesthesia in this hospital. 5) For intravenous induction, thiopental has been used as the main agent, and succinylcholine and pancuronium as the major musce relaxants since 1975. 6) For anesthetic technique, semiclosed circle absorption system has been employed in almost all cases, and non-rebreathing system has been used recently with increasing frequencies in pediatric anesthesia.
