천궁육계탕(川芎肉桂湯)을 투여한 요추 방출성 골절 환자 치험 1례

천궁육계탕(川芎肉桂湯)을 처방하여 요추 방출성 골절로 인한 극심한 요통 및 하지의 견인통이 호전된 1례를 통하여 신경학적 결손이 없는 요추 방출성 골절에 대해 천궁육계탕(川芎肉桂湯)이 환자의 자각적인 증세 및 NRS, ODI상으로 유효한 효과가 있었음을 확인하였다. 이에 저자는 요추 방출성 골절로 입원하여 천궁육계탕(川芎肉桂湯)을 복용한 후 호전된 1케이스를 보고하는 바이다. Objectives: The purpose of this study is to investigate the clinical application of Chungungyukgye-tang(Chuanxiongrougui-tang) to one patient with lumbar burst fracture. Methods: Patient is hopitalized at Dept. of Oriental Internal Medicine, Bu-Chun Jaseng Oriental Medicine Hospital, diagnosed as lumbar burst fracture and treated mainly with herbal medine ; Chungungyukgye-tang(Chuanxiongrougui-tang). This study was measured by ODI(Oswestry Disability Index) and NRS(Numeric Rating Scale) score. Results: After taking Chungungyukgye-tang(Chuanxiongrougui-tang), the patient's pain was controlled. NRS & ODI score were decreased. Conclusions: As seen in this one case of lumbar burst fracture, Chungungyukgye-tang(Chuanxiongrougui-tang) has a positive effect to control pain with lumbar burst fracture.

금출도담탕(芩朮導痰湯)을 처방하여 어깨 충돌증후군으로 인한 극심한 통증 및 가동범위의 제한이 호전된 3례를 통하여 금출도담탕(芩朮導痰湯)이 어깨 충돌증후군에 대해 환자의 자각적인 증세 및 NRS, 견관절 이학적 검사상 유효한 효과가 있었음을 확인하였다. 이에 저자는 어깨충돌증후군으로 입원하여 금출도담탕(芩朮導痰湯)을 복용한 후 호전된 3케이스를 보고하는 바이다. Methods: Patients were hospitalized at Dept. of Oriental Rehabilitation Medicine, Bu-Chun Jaseng Oriental Medicine Hospital, diagnosed as shoulder impingement syndrome and treated mainly with herbal medicine, Geumchuldodam-tang(Q$\acute{i}$nzhud$\check{a}$ot$\acute{a}$n-tang). This study was measured by Numeric Rating Scale(NRS) score. Results: After taking Geumchuldodam-tang(Q$\acute{i}$nzhud$\check{a}$ot$\acute{a}$n-tang), pain of the patients was controlled. NRS scores were decreased. Conclusions: As seen in these three cases of shoulder impingement syndrome, Geumchuldodam-tang(Q$\acute{i}$nzhud$\check{a}$ot$\acute{a}$n-tang) has a positive effect to control pain with shoulder impingement syndrome.

Muscle Energy Techniques 및 한의학적 치료를 이용한 후천성 회전형 사경의 치험 1례

후천성 회전형 사경으로 입원 치료한 환자 1례에 대하여 MET 기법을 위주로 한 한의학적 치료가 주소 증상에 유효한 효과가 있음을 확인하였다. 이에 저자는 본 증례를 보고하는 바이다. Objectives : The purpose of this study is to investigate the clinical application of Muscle Energy Techniques(MET) and Oriental Medicine to a patient who had Acquired Rotating Torticollis. Methods : A patient diagnosed as acquired torticollis was hospitalized at Buchun Jaseng hospital of Oriental Medicine for 28 days and she was treated by MET, herbal medicine and acupuncture. To measure the outcome of the patient's improvement, we observed Numerical Rating Scale(NRS), Neck Disablility Index(NDI), Tsui's Scale and the X-rays of Whole spine. Results : After being treated by MET and Oriental Medicine, the patient's pain was reduced and the head rotating was significantly improved. The improvement of patient in the NRS, NDI, Tsui's Scale was detected and the X-ray of Whole spine became normal. Conclusions : As seen in this case, complexed therapy with MET and Oriental Medicine is effective treating method to control pain and head rotating for patients with acquired rotating torticollis.

고관절 내회전 각도와 단분절 요추 추간판 탈출증 발생 부위의 상관성 분석

Objectives : The purpose of this study is to find out the relationship between hip internal rotation angle and the level of herniation of lumbar intervertebral disc(HIVD) in single-segment. Methods : We investigated 314 patients (158 male, 156 female) who were diagnosed as herniation of lumbar intervertebral disc(HIVD) in single-segment. We measured 314 patients' hip internal rotation angle and analysed the relationship between the hip internal rotation angle and the level of herniation of lumbar intervertebral disc(HIVD). Results : 1. Among 314 cases, the hip internal rotation angle was different between male and female. Hip internal rotation angle of male was mainly limited and that of female was mainly excessive. 2. Among 314 cases, the normal group, defined as patients who have specific range of hip internal rotation angle(male : $35^{\circ}{\pm}10^{\circ}$, female : $45^{\circ}{\pm}10^{\circ}$), tends to be occurred HIVD at L4/5 level. The limited group, defined as patients who have less angle than normal group, the excessive group, defined as patients who have more angle than normal group, and the complex group, defined as patients who have more angle of one leg and less angle of the other leg than normal group, tend to be occurred HIVD at L5/S1 level(p<0.05). Conclusions : In single-segment lumbar HIVD patients, The normal hip internal rotation angle mainly leads to L4/5 HIVD, while the limited and excessive hip internal rotation angle mainly lead to L5/S1 HIVD.

한방 치료와 자하거약침을 병행하여 치료한 극상근건염 및 삼각근하 점액낭염 환자 4례 증례보고

Objectives : The purpose of this case series is to investigate and report the effectiveness of oriental medicine treatment and Hominis placenta pharmacopuncture treatment for supraspinatus tendinosis and subdeltoid bursitis. Methods : 4 patients are admitted at Dept. of Oriental Rehabilitation Medicine, Bu-Cheon Jaseng Oriental Medicine Hospital, diagnosed as supraspinatus tendinosis and subdeltoid bursitis and treated with oriental medicine treatment and Hominis placenta pharmacopuncture. Each case is measured and assessed daily by NRS(Numeric Rating Scale) score and shoulder physical exam. Results : After treatment, patients' shoulder pain are decreased and they showed nearly full ROM(range of movement). In patients with supraspinatus tendinosis and subdeltoid bursitis, oriental medicine treatment and Hominis placenta is good method for pain relief and better movement. Conclusions : As seen in these 4 cases of supraspinatus tendinosis and subdeltoid bursitis, oriental medicine treatment and Hominis placenta pharmacopuncture appears to be effective. 극상근건염 및 삼각근하 점액낭염으로 견관절의 통증과 이학적 검사 양성반응 및 ROM 제한을 호소하는 환자 4례에 대하여 한방 치료와 자하거약침 치료를 병행하였으며 그 결과 통증의 감소, 이학적 검사 소견의 개선 및 ROM 호전을 확인하였다.

PubMed를 통해 살펴 본 요통에 대한 추나 치료 연구의 최신 동향

Objectives : The purpose of this study is to explore the latest trends of Chuna(chiropractic) treatment on low back pain. Methods : We investigated the latest studies on Chuna(chiropractic) treatments for low back pain via searching PubMed. 19 research papers were analyzed and the journals, the authors, countries, methods, periods, techniques of chiropractic, instruments of assessment, and ethics approvals of those research papers were examined. Results : Studies on Chuna(chiropractic) treatments were mainly published via J Manipulative Physiol Ther. The technique of chiropractic that most frequently adopted in the researchs was high-velocity low-amplitude thrust technique(HLVA). Numeric rating scale(NRS), oswestry disability questionnaire(ODI), and Roland-Morris disability index(RMDQ) were used as primary outcome assessments. There were no ethical problems found on the studies investigated. Conclusions : Reviewing the latest trends of studies on Chuna(chiropractic) treatments for low back pain and figuring out the strong points and weak points of those treatments are necessary to future studies. It is anticipated that this review would benefit the in-depth treatments for low back pain in terms of Korean medicine.
