        뇌졸중 환자의 회전 보행 시 회전 방향이 보행 특성에 미치는 영향

        Purpose: The purpose of this study was to determine the effects of rotation direction during curved walking on gait parameters in stroke patients. Methods: A group of thirty subjects with stroke (Berg Balance Scale score${\geq}41$ were fifteen, Berg Balance Scale score${\leq}40$ were fifteen) were enrolled in this study. Testing indications included two directions for rotation in each subject. These indications were for rotation toward the affected and unaffected side in stroke patients. The gait speed, affected side single support duration, affected side double support duration were recorded. The obtained data were analyzed by using paired t-test and Wilcoxon signed rank test in the group that are below and above 40 points of Berg Balance Scale score. Results: There was significant increase affected side single support duration was turned the affected side in stroke patients that presented a Berg Balance Scale score${\geq}41$ (p<.05). There were significant increase gait speed, affected side single support duration, and significant decrease affected side double support duration while subjects were turned the affected side in stroke patients that presented a Berg Balance Scale score${\leq}40$ (p<.05). Conclusion: This result may be effective to rotate in the paralyzed direction to improve the ability of the paralyzed lower limb to gain weight during gait training for stroke patients with a Berg Balance Scale score<40. Therefore, walking training program for hemiplegic patient needs to be suggested in the direction of turning for suitable balance ability.

        Effects of Core Stability Exercises on Energy Expenditure During Gait in Subacute Stroke Patients

        Purpose This study aimed to evaluate the effect of core stability exercises on energy expenditure during gait in subacute stroke patients. Methods The study included 24 subacute stroke patients randomly allocated to the experimental group (n=12) and control group (n=12). Patients in the experimental group performed the core stability exercises (20 min) and conventional physical treatment (20 min), while those in the control group performed conventional physical treatment (40 min) only. Both groups performed training five times a week for 4 weeks. Outcome assessments evaluated energy cost, physiological cost index, and walking speeds. The patients were assessed before and after intervention. The Shapiro-Wilk test was used to assess normalization. Within-group differences were analyzed using the paired t-test, while between-group differences were analyzed using an independent t-test. Results After 4 weeks of exercise, both the groups showed significant increased walking speed and significant decreased energy cost, physiological cost index (p < .05). However, the experimental group was more effective than control group was increasing in walking speed (p < .05) and decreasing in energy cost (EC), physiological cost index (PCI) (p < .05). Conclustion We suggest that core stability exercises effectively improve walking speed and energy efficiency during gait in subacute stroke patients.

        정비사업의 건설사업관리(CM) 적용을 위한 프로세스 개발 연구

        Despiste the enactment of the 「Urban and Living Environment Improvement Act」 in July 1, 2003, due to abscense of official undertakings, privatization of profits by developments, no guarantee for civil participations, inefficiency of businesses, non-professionalism of business promoters, etc., the business of housing improvements has become the disadvantage to the owners of the lands or such with postponement of the business term and deterioration of its profitability. In order to solve these problems, the government revised the law on March 2, 2016, which opened the way for the independent maintenance of the trust companies, but from a constructive point of view, the structure that can not but lean on the contractor remains remains. To solve the problem, the research has been done in order to seek for a new direction of maintenance business by contemplating on the business and CM, comparing each processes and combining them. The research method employed advanced researches with literature, legal and institutional approaches towards data from 1st July 2013 to 31st December 2016. Applying CM to maintenance business 1) improves transparency of the business and solves local disputes, 2) is expected to bring the decrease in business period and efficient administration, 3) is expected to bring decrease in cost, and 4) would contribute to the local publicity by preventing slumism through normal conduct of private businesseIn this study, it is expected that the maintain business development model that utilizes the characteristics of the construction management(CM) should consider the position of housing policy of Korea and a better alternative will be studied. 2003년7월1일 「도시 및 주거환경정비법」이 제정됨에도 불구하고 주택정비사업은 공공의 역할부재, 개발이익의 사유화, 주민참여 미보장, 사업의 비효율성, 사업추진 주체의 비 전문성 등으로 사업기간 지연과 수익성 악화로 토지등소유자에게 손실이 되고 있다. 이러한 문제점을 해소하고자 정부에서는 2016년3월2일 법 개정을 통해 신탁사의 정비사업 단독 시행의 길을 열어 주었으나 건설적인 측면에서 보면 시공사에 전적으로 기댈 수 밖에 없는 구조는 여전히 남아 있어 이에 대한 해결방안을 찾고자 하였으며 이를 위해 연구 방법은 선행연구와 문헌적, 법·제도적 접근방법을 연구의 시간적 범위는 2003년7월1일부터 2016년12월31일까지, 내용적 범위는 정비사업 및 건설사업관리(CM)에 대한 고찰과 각각의 프로세스를 비교하고 이를 융합해 정비사업의 새로운 방향을 모색해 보고자 하였다. 정비사업에 건설사업관리(CM)를 활용하면 첫째. 정비사업의 투명성 강화와 주민 갈등이 해소되고, 둘째. 정비사업의 공기단축과 원활한 공정관리가 기대 되며, 셋째. 사업경비 절감이 예상되며, 넷째. 정비사업에 민간사업자의 정상적 진행으로 도시 슬럼화가 방지 되어 지역의 공공성에도 기여를 할 것이다. 본 연구에서 논의된 건설사업관리(CM)의 특성을 활용한 정비사업 개발 모델이 우리나라 주택정책에서 어떤 위치를 가질지 고민해야 할 것이며 더 좋은 대안이 연구되길 기대 한다.

        휠체어 몸통 훈련이 급성기 뇌졸중 환자의 낙상 효능감, 낙상 위험도, 일상생활활동에 미치는 영향: 무작위 대조군 예비 연구

        Background: The aim of this randomized controlled pilot study was to determine the effect of trunk training in wheelchair on fall efficacy, fall risk and activities of daily living in acute stroke patients. Design: Randomized controlled pilot trial. Methods: The study included 18 patients with acute stroke who were randomly allocated to an experimental group (EG) (n=9) and a control group (CG) (n=9). Patients in the EG group received general rehabilitation therapy combined with trunk training in wheelchair for 20min, whereas CG group received general rehabilitation therapy combined with bicycle training for 20min. Both groups performed the exercise 5 times a week for 3 weeks. Outcomes were assessed using Korean-Fall Efficacy Scale (K-FES), Fullerton Advanced Balance Scale (FAB) and Korean-Modified Barthel Index (K-MBI). Results: After 3 weeks of training, both groups showed significantly improved K-FES, FAB and K-MBI (p<.05 in both groups). However, the K-FES, FAB and K-MBI in the experimental group was significantly improving than in the control group (p<.05). Conclusion: These findings indicate that trunk training in wheelchair may be effective at decreasing fall risk and improving activities of daily living in acute stroke patients. Therefore, trunk training in wheelchair may be recommended as an intervention in reducing the incidence of fall risk in acute stroke patient.

        화엄사 서오층석탑 발견 「무구정광다라니」의 서지적 연구

        Hwaeomsa dharani is the only material with which we can reconstruct the overall aspects of Mugujeonggwang dharani distributed during unified Silla. Despite its value as significant material, it has not been much noticed so far unlike other 「Mugujeonggyeong」 materials transmitted from unified Silla. Therefore, this author aims to reveal its value as significant material by examining the contents, patterns of transcription, marks of correction, and the New Characters of Wu Zetian. Regarding it, this study has found some characteristics of Hwaeomsa dharani and summed them up as follows: first of all, only the dharani was transcribed, excluding the Buddhist scriptures of 「Mugujeonggyeong」. The characters needed to recite the dharani were not transcribed. Six dharanis form a set and are transcribed repetitively, and they are transcribed 28 times at least. Also, in the beginning of each session, the title of a dharani, ‘最勝無垢清淨光明大根夲陁羅 ’, is transcribed. After transcribing six dharanis once, one or two blank lines are left for division before transcribing the next ones. There is space between Chinese characters to distinguish a dharani from another within a session. With these characteristics, we can see one of the concrete ways to produce mugujeonggwang dharani for enshrinement used during unified Silla. Hwaeomsa dharani was transcribed not for reciting but purely for enshrinement. Meanwhile ‘卜’ was used for deletion marks. This is the earliest example of using ‘卜’ for a deletion mark in Korea. In addition that, marks for reversal or repetition are also found. As in other Silla scriptures, ‘埊’, the new characters of Wu Zetian, was also used. 화엄사 다라니는 통일신라시대에 유통된 무구정광다라니 전체 모습을 재구할 수 있는 유일한 자료로, 그 자료적 가치에도 불구하고 그동안 통일신라시대의 다른 「무구정경」 자료에 비해 큰 관심을 받지 못하였다. 이에 본고는 화엄사 서오층석탑 출토 다라니의 내용과 필사 양식, 부호, 측천무후자의 사용 등을 살펴 그 자료적 가치를 드러내고자 한 것이다. 이와 관련하여 본고에서 확인된 화엄사 다라니의 특징 몇 가지를 정리하면 다음과 같다. 먼저, 화엄사 다라니에는 「무구정경」의 경문을 제외한 다라니만 필사되어 있으며, 다라니 내 다라니의 암송을 위한 표시들은 필사되어 있지 않다. 여섯 개의 다라니가 한 세트가 되어 반복적으로 필사되었고 최소 28회 필사되었다. 또 매회의 시작 부분에 다라니명 ‘最勝無垢清淨光明大根夲陁羅 ’를 필사하였다. 여섯 개의 다라니를 한 차례 필사하고 다시 다음 차례의 다라니를 필사할 때에는 空行을 두어 구분하는 경향을 보였고, 한 회 안에서 다라니와 다라니를 구별할 때에는 隔字를 두는 경향을 보였다. 이와 같은 사실을 통해 통일신라 시대에 납탑용 무구정광다라니를 제작하는 하나의 구체적인 방식을 확인할 수 있었으며, 화엄사 다라니가 암송을 위한 것이 아니라 순수하게 납탑을 위해 필사된 것임을 알 수 있었다. 한편, 삭제부로는 ‘卜’이 사용되었는데 이는 우리나라에서 삭제부 ‘卜’이 쓰인 매우 이른 시기의 용례이다. 이 외에도 전도부, 반복부 등의 부호가 확인되었으며, 다른 신라 사경처럼 측천무후자 ‘埊’도 사용했음이 확인되었다.

        중환자 재활에서 물리치료사의 역할과 필요성

        In the past, patients in the intensive care unit were mostly managed by the intensive care unit's own protocol through bed rest and sedation. The number of hospitalized patients in intensive care units in Korea is steadily increasing, but the mortality rate is higher than in developed countries and the perception of intensive care rehabilitation is lowered. The reality of intensive care rehabilitation in korea has many problems due to low fee structure, lack of manpower, and lack of policy establishment and will of related institutions. In addition, intensive carerehabilitation should consist of a multidisciplinary tema, and the team composition should include professionals such as dedicated doctors, dedicated nurses, and physical therapists. Although the effect of intensive care unit rehabilitation has been confirmed through previous foreign studies, the reality of intensive care unit rehabilitation in Korea is currently lagging behind due to a system of low medical fees and the will of government agencies. This study was attempted to find out the role and necessity of physical therapists in intensive care rehabilitation so that rehabilitation medical fees suitable for reality can be set when domestic intensive care rehabilitation is institutionalized and activated in the future.

        후방 보행훈련이 급성기 뇌졸중 환자의 균형, 균형 자신감, 낙상 효능에 미치는 영향: 무작위 대조군 예비연구

        Background: The requirements for postural and motor control in backward walking training (BWT) may improve balance and walking speed in patients with acute stroke. The aim of this study was to analyze the effect of BWT on balance, balance confidence, and fall efficacy in this population. Design: Randomized controlled pilot trial. Methods: This study included 14 subjects with acute stroke (onset of illness less than one month). They were randomly allocated to a BWT (n=7) or forward walking training (n=7) group and observed five times in a week for a period of two weeks. Measurements were taken before and after the experiment using the Berg balance scale (BBS), Activities-specific balance confidence scale (ABC), and Fall efficacy scale (FES). Results: The BBS, ABC and FES scores obtained in both groups after the experiment were significantly higher than those before the experiment (p<0.05). In addition, the BBS, ABC, and FES scores in the experimental group were significantly higher than those in the control group (p<0.05). Conclusion: These findings indicate that BWT improved balance and balance confidence and decreased the risks of fall in patients with acute stroke. Further study is needed to better understand the effects of backward walking in acute stroke patients.

        동작관찰 체간훈련이 급성기 뇌졸중 환자의 체간조절능력, 균형, 일상생활동작에 미치는 영향: 무작위 대조 예비 연구

        목적 : 본 연구는 급성기 뇌졸중 환자에서 동작관찰 체간훈련이 체간조절능력, 균형, 일상생활수행능력에 미치는 효과를 알아보고자 실시하였다. 연구방법 : 2017년 7월 3일부터 2018년 4월 30일까지 전북 익산 소재의 W 대학병원에 입원한 급성기 뇌졸중 환자 14명을 실험군과 대조군으로 무작위 할당하였다. 두 군 모두 일반적인 물리·작업치료를 실시하였다. 추가적으로 실험군에는 동작관찰 체간훈련을, 대조군은 풍경관찰 체간훈련을 각 1일 1회 30 분, 주 5회, 총 3주간 실시하였다. 치료 중재 전·후 체간조절능력을 측정하기 위해 체간 손상 척도 (Trunk Impairment Scale; TIS), 균형능력을 측정하기 위해 수정된 기능적 팔 뻗기 검사(Modified Functional Reach Test; M-FRT)와 버그균형척도(Berg Balance Scale; BBS)를 사용하였고, 일상생활활동 수행능력을 평가하기 위해 한국판 수정된 바델 지수(Korean version of Modified Barthel Index; K-MBI)을 사용하였다. 결과 : 3주 중재 후, 두 그룹에서 중재 전·후 체간조절능력, 균형, 일상생활수행능력에서 유의한 차이가 나타났으며 (p<.05), 중재 후 두 그룹 간 변화량의 차이는 TIS와 M-FRT에서 유의한 차이가 나타났다(p<.05). 결론 : 본 연구 결과 동작관찰 체간훈련이 급성기 뇌줄중 환자의 체간조절능력, 균형, 일상생활수행능력을 향상시킬 수 있는 중재 방법으로 나타났다. 따라서 기립 훈련기에서 동작관찰 체간훈련은 급성기 뇌졸중 환자에게 능동적이고 동적인 체간훈련을 제공할 수 있는 새로운 중재 방법으로 사용될 수 있을 것이다. Objective : This study investigated the effects of action observation trunk training on trunk control, balance, and activities of daily living in patients with acute stroke. Methods : Fourteen inpatients were randomly allocated to either the experimental group, who underwent action observation trunk training, or the control group, who underwent landscape observation trunk training(n = 7 each). Each intervention consisted of a 30-min session once a day, five times a week, for three weeks. Each intervention consisted of a 30-min session once a day, five times a week, for three weeks. To measure trunk control, balance, and performance capacity in activities of daily living, the Trunk Impairment Scale(TIS), Modified Functional Reach Test(M-FRT), Berg Balance Scale(BBS), and Korean-modified Barthel Index(K-MBI) were used before and after the intervention. Results : After the intervention, both groups showed a significant increase in TIS, M-FRT, BBS, and K-MBI scores (p<.05). Using change value comparison, the experimental group showed a greater increase in TIS and M-FRT scores (p<.05). Conclusion : Action observation trunk training was effective in facilitating trunk control, balance, and activities of daily living in patients with trunk control deficits after acute stroke. Therefore, action observation trunk training in standing may be used as a new intervention method to provide active and dynamic training for rehabilitating patients with acute stroke.
