Pouchitis, a non-specific acute inflammation occurring in the ileal pouch, is one of the most common complications developed after the restorative proctocolectomy and ileal pouch-anal anastomosis (IPAA) performed for the treatment for the patients with ulcerative colitis and familial adenomatous polyposis. The prevalence of pouchitis is known to range from 20% to 50%. One to two percent of the cases are chronic and resistant to the drug therapy. The effective treatment for this chronic resistant pouchitis is to remove the ileal pouch and perform the permanent ileostomy. Hereby, we report one case of chronic pouchitis resistant to multiple drug therapy developed after IPAA performed for the treatment of ulcerative colitis in a patient. Among malignant tumors of the stomach, adenocarcinoma takes up about 95% and the remaining are mostly lymphomas, being less than 5%. The majority of lymphomas are B cell lymphomas, and the most common types are low-grade B cell lymphoma of mucosa-associated lymphoid tissue and diffuse large B cell lymphoma (DLBL). The synchronous occurrence of adenocarcinoma and lymphoma in the stomach is being reported rarely. Especially the concurrence of adenocarcinoma and DLBL is very scarce and less than 10 cases have been reported inside and outside this country. In the past, the general treatment for cases of concurrence of adenocarcinoma and DLBL when surgery is possible according to cancer stages was gastrectomy, followed by single or combined chemotherapy and radiation treatment. However, when considering that most cases of concurrent adenocarcinoma were early gastric cancer which is limited to the mucosa, endoscopic submucosal dissection (ESD) can become an alternative treatment method for gastrectomy. We report the experience with chemotherapy and ESD done together instead of surgery, in patients concurrently diagnosed with early gastric cancer and gastric lymphoma. Background: Transplantation of primary hepatocytes (PH) has been shown to provide metabolic support during acute liver failure. However, PH are known to be subject to necrosis in the peritoneal cavity. This is because cell-cell interaction plays an important role in their survival, but the peritoneal cavity can not provide such an environment. We tried to improve the survival of PH by simultaneously transplanting nonparenchymal liver cells (NPL). Methods: PH from normal Wistar rats, either alone (10(9) cells/kg, group 1, n=10) or mixed with NPL (5×10(8) cells/kg, group 2, n=10) were transplanted into the peritoneal cavity of hyperbilirubinemic Gunn rats which are congenitally devoid of bilirubin glucuronidation. Liver cells from Gunn rats were transplanted as a control. Results: Bilirubin glucuronides (BG) were detected in the bile of both group 1 and 2 rats collected at 6 hours after transplantation, and reached peak levels in 4 days. However, in the third and fourth week, BG could be detected only in group 2 animals. The serum bilirubin levels were decreased by 12.1~18.9% of basal levels in the second and third week for group 2 rats, but decreased by 15.1% only in the second week for the group 1 rats. Using in situ hybridization, albumin mRNA positive cells could be detected until the fourth week for the group 2 animals, but only until the second week for the group 1 rats. Conclusions: PH start functioning in a short time after intraperitoneal transplantation and simultaneous transplantation of NPL with PH can prolong the survival and function of transplanted hepatocytes. A Study on Integrated Tourism Platform for supporting Smart Tourism

Purpose - The existing ICT based services for tourism are limited to supporting to service only partial functions among overall tourism processes which consists of tourism planning process, tourism activities and tourism experience sharing. The ICT based service system that supports tourism services necessary in the overall tourism process is known not to be yet introduced in domestic tourism area. It is necessary to conceptualize and design schematically the ICT based tourism service system for supporting the overall tourism process. In this paper, we propose the concept and skeleton functions of the integrated tourism platform, which would support services that tourists and tourism suppliers might require in smart tourism environment. Research, design, data, and methodology – The concept and skeleton functions of the integrated tourism platform might be derived in a qualitative research method. We proposed a concept of tourism life cycle proposed first. We investigated and analyzed the service functions of the existing ICT based tourism service systems that support each step in the tourism life cycle so that the concept and skeleton functions of the integrated tourism platform were derived with results that experts in tourism and ICT areas discussed in brainstorming ways. Results - We proposed the concept of tourism life cycle needed to derive the functions of the integrated tourism platform. The tourism life cycle contains six phases, which consists of planning, research, booking, travel, reviews and inspiration phases. The tourism life cycle is to departmentalize overall tourism process. The integrated tourism platform consists of data platform and service platform. The data platform is a kind of large scale database with integrating various types of data. The various data would be collected by web crawlers or inputted manually by tourism providers. The Ontology and Mash-up technologies should be applied to integrate the various types of data effectively to make data with good quality. Conclusions - The integrated tourism platform could support the overall tourism process in tourism life cycle. It does not only provide personalized tourism information and products for tourists, but also, provide opportunities with deriving creative business items for venture business mans. The service platform would also ICT based service system to support Sharing economy in the Fourth Industrial Revolution.

Background/Aims: Vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF) are important angiogenic factors. The aim of this study was to measure the plasma level of VEGF and bFGF in patients with gastric cancer and to compare them with those of normal controls. Additionally, we examined the relationship between cancer progression and the levels of VEGF, bFGF, serum CEA and CA19-9. Methods: A total of 108 subjects including 62 normal controls and 46 patients with gastric cancer were recruited. Then, the patients were classified into 3 groups: early cancer group (stage I), advanced gastric cancer group (stage II & III) metastatic gastric cancer group (stage IV). Plasma VEGF and bFGF were measured by ELISA. Serum CEA was measured by radioimmunoassay, and serum CA19-9 was measured by microparticle enzyme immunoassay. Results: Plasma levels of VEGF and bFGF were higher in patients with gastric cancer than in normal controls (p<0.001). Furthermore, patients with metastatic gastric cancer have much higher plasma VEGF and bFGF levels than patients with early and advanced gastric cancer. Serum CA19-9 level was higher in metastatic gastric cancer compared with those of other two groups (p<0.001). The cut-off value of VEGF was 50.5 pg/mL and the cut-off value of bFGF was 19 pg/mL to discriminate the patients with gastric cancer from controls. Conclusions: Plasma VEGF and bFGF as well as serum CA19-9 could be useful markers for gastric cancer progression, especially in metastatic gastric cancer. (Korean J Gastroenterol 2003;41:351-357)

기계적강도가 좋고 제막성능이 우수하며 화학적으로 안정한 Poly(vinylidene fluoride)(PVDF)를 사용하여 습식응고방법에 의해 한외여과막을 제조하였다. 막의 구조는 유체역학적인 저항을 줄이기 위해 비대칭구조로 제조하였으며 막두께를 200㎛ 이하로 조절하였다. 막의 성능 및 구조에 미치는 용매의 영향을 검토하기 위하여 dimethyl formamide(DMF), dimethyl sulfoxide(DMSO) 및 dimethyl acetamide(DMAc)를 제막용매로 사용하였으며 제막용액에 휘발성 난용매인 acetone을 첨가하여 그에 따른 효과도 검토하였다. 사용된 용매의 조성은 (a) 단일용매계 : DMF, DMSO 및 DMAc, (b) 2성분용매계 ; DMSO/DMF, DMF/DMAc 및 DMAc/DMSO, (c) 2성분용매/난용매계 : DMF/Aceton, DMAc/Acetone, DMSO/Acetone 등이다. 한외여과막의 성능을 평가하기 위하여 polyethylene glycol(PEG, 평균분자량 1,500∼20,000)수용액을 사용하여 배제율과 투수성을 측정하였으며 시차 주사전자현미경으로 막의 단면을 관찰하였다. 실험결과로부터 PVDF한외여과막은 2 기압하에서 10∼200(ℓ/㎡h)의 투수량을 나타냈음을 알았다. DMF를 용매로 사용한 막의 배제율이 매우 우수한 반면 투수성이 떨어지며 DMSO를 사용하였을 때는 배제율도 비교적 크고 물투과성도 좋은 것을 알 수 있었다. 2성분용매계에서는 DMSO/DMF(1/1)의 경우 PEG 7,500의 배제율이 9.5%이며 PEG 20,000은 93.5%의 배제율을 나타내어 매우 우수한 분획성능을 가짐을 알 수 있었으며 투구량도 89.4(ℓ/㎡·h)로 매우 우수한 한외여과성능을 나타내었다. Poly(vinylidene fluoride)(PVDF) was prepared for the ultrafiltration membranes because of its good film forming ability, high mechanical strength, and chemical resistance to solvents. Membranes were prepared by solvent casting method on a glass plate. To investigate the effect of casting solvents on the membrane performances dimethyl formamide(DMF), dimethyl sulfoxide(DMSO), and dimethyl acetamide(DMAc) were used as casting solvents. The solvent composition of casting solution was selected from one of followings; (A) homosolvent system of DMF, DMSO or DMAc, (B) binary solvent system of DMSO/DMF, DMF/DMAc or DMAc/DMSO, and (C) binary solvent system of solvent and volatile nonsolvent such as acetone. Ultrafiltration experiments were carried out with 0.2% aqueous solution of polyethylene glycol (PEG, average molecular weight of 1,500-20,000). Water flux and rejection were measured, and the cross-sectional structures of membranes were observed by a scanning electron microscope. From the result of water permeation, it was revealed that most of all the PVDF membranes had the water flux 10∼200 (ℓ/㎡·hr) at 2 atm.. Membranes prepared by DMF solvent system had the high rejection values inspite of poor water fluxes in comparison with other membranes. DMAc and DMF solvent systems showed the sharp slope of rejection curves with the increase of molecular weight of solute, and especially, membranes prepared by DMSO had good performances for ultrafiltration.
