댄스-기반 운동치료 프로그램을 적용한 사례연구: 소뇌성 운동실조증 환자의 균형 및 보행능력에 미치는 효과

본 연구는 중추신경계의 일부분으로서 신체의 자세 및 자발적 움직임의 조절과 평형을 유지하는 기능을 담당하는 소뇌에 이상이 있는 환자를 대상으로 댄스-기반 운동치료 프로그램이 환자의 균형과 보행능력의 재학습에 어떠한 영향을 주는지 조사하였다. 연구의 참가한 소뇌질환 환자는 유전적 원인으로 인해소뇌의 신경세포가 사멸하여 그 크기가 감소하는 소뇌위축증(cerebellar atrophy)으로 진단을 받고 그로인한 운동장애인 운동실조증(ataxia)을 보인 1명의 환자를 대상으로 하였다. 연구 대상자는 파트너 댄스인 탱고 스텝에 기반을 둔 운동치료 프로그램에 8주에 걸쳐 매주 3회씩 한번에 90분씩 진행하는 수업에 참가하였다(총 24회). 연구 결과, 댄스에 기반을 둔 운동 치료 프로그램에 참여한 소뇌성 운동실조증 환자의 평지 보행 능력 측정에서의 보폭 길이가 증가하였으며 보간 너비가 감소한 것으로 나타났다. 또한 보행 주기 중 한 다리 지지기 비율과 균형 능력의 향상과 같은 긍정적 상호작용도 나타났다. 보다 기능적인 보행 능력을 평가하는 장애물 넘기 보행에서는 장애물을 넘기 직전의 움직임 동결 시간이 감소하였으며, 장애물을 넘기 전 선행하는 발의 이륙거리와 장애물 통과 속도는 초기에 비해 사후 검사에서 증가하는 양상을 나타냈다. 이러한 결과는 댄스-기반 운동치료 프로그램 훈련이 소뇌성 운동실조증 환자에게 적용되었을 때 정상적인 보행주기 패턴의 재정립과 균형 능력의 회복에 긍정적인 영향을 줄 수있음을 보여준다. It has been demonstrated that the cerebellum plays a key role in controlling voluntary movements including body balance and posture. The present study examined the effects of dance-based exercise therapy program on relearning of balance and locomotion ability in an individuals with cerebellar disease. A male patient (39 years old) with severe cerebellar atrophy who shows ataxic movement disorders participated in 1½-hour of adapted tango lessens 3 times per week, completing 24 lessons within 8 weeks. The results indicated that the participant exhibited improvements of normal walking and functional gait ability. More specifically, step width was decreased while step length, single stance time, gait speed and balance time were increased after the intervention. In addition, the participant improved on freezing time, pre-obstacle distance and crossing velocity during the obstacle crossing task. This case study suggests that a dance-based exercise therapy program may positively influence on the recovery of gait and balance ability for those who suffer movement disorders due to cerebellar disease.
장애물 보행 훈련 프로그램이 소뇌성 운동실조증 환자의 기능적 보행 능력에 미치는 효과

The purpose of this investigation was to examine the effects of the obstacle ambulation training program on functional gait parameters in an individual with cerebellar ataxia. A female participant (44 years old) with ataxia. A female disorders due to the cerebellar atrophy was recruited. The obstacle crossing training, which based on task-specific regimen, was applied for 1½-hour, three times weekly sessions for ten weeks. The results showed that, compared to baseline, obstacle ambulation time, freezing time, and toe-clearance were decreased while pre- and post- obstacle distance were increased after intervention. Thus, the patient exhibited substantial improvements of obstacle ambulation and functional gait ability. The results from this case study suggest that a task-relevant obstacle ambulation training program may have a beneficial effect on functional gait rehabilitation for individuals with cerebellar disease.
한국무용 기반 움직임 훈련이 소뇌성 운동실조증 환자들의 균형 및 보행 기능 회복에 미치는 효과

It has been demonstrated that the cerebellum plays a key role in controlling body balance and posture. Although physiotherapeutic approach presents the most optimal therapy to improve ataxia dysfunctions, the consensus on the effective way to improve CA-specific motor symptoms is not fully addressed. The present study examined the effects of a KD-based movement training on recovery of locomobility in individuals with CA. Eight patients were recruited to participate in the study. They participated in a 12-week KD-based movement training emphasizing the relearning of proper gait patterns through intensive practice of various movement components, which targets CA-specific impairments. The main kinematic data indicated that CA patients exhibited improvements in center of mass control, basic gait parameters and balance after the KD training. These results provided a preliminary evidence that a KD-based movement training may be helpful for reducing CA-specific locomotion symptoms, especially during normal and slow walking.
운동실조증 환자의 신체적 여가 경험이 우울, 일상생활, 낙상효능감 및 삶의 질에 미치는 영향

In this study, we developed physical and recreational programs and applied them to a patient with ataxia who has difficulty in the experience of leisure, and measured its effects on depression, activities of daily living, falls efficacy, and quality of life of the patient as a single case. The participant suffered a genetic disorder caused by cerebellar atrophy in which neuronal cells progressively decay over time. He participated our program two hours a day, three times a week, for 10 weeks total. The results indicated that there were positive changes in depression, falls efficacy, and quality of life of the patient. These results suggest that experience in physical leisure may be appropriate and effective therapeutic methods in patients with ataxia.
체중지지 트레드밀 훈련이 퇴행성 소뇌 질환자들의 하지 근력 및 보행 능력에 미치는 영향

Cerebellar damage causes declines in muscular strength and coordination in the lower limbs. This study examined the effects of body weight supported treadmill(BWSTT) training on muscular strength of lower limbs and walking ability in patients with degenerative cerebellar atrophy. Three patients diagnosed with cerebellar ataxia were participated in 100-week BWSTT rehabilitation training programs for total 60 minutes, 3 times per week. The results showed that their severity of impairments and walking capacity was improved after the training based on the changes in ICARS and TUG clinical tests. However, this improvement was not persisted in a long-term period because they showed a reduction in the scores after 1 month. Similar results were found in the muscular strength. All patients exhibited increased muscular strength of the lower limbs after the training, but were not persisted over a month. These findings suggest that the benefits of rehablilitation programs adopted in the present study is evident for the recovery of locomotion functions and muscular strength of the lower limbs in cerebellar patients, but they need to continue to engage in motor activity to maintain their motor function gains acquired by the training.

The purpose of this study was to investigate the combined effects of body weight supported treadmill training (BWSTT) and overground walking training based on the task specificity theory on quality of gait, walking ability, falls efficacy, and health-related quality of life in patients with degenerative cerebellar atrophy. Three patients diagnosed with cerebellar atrophy due to genetic disorders were participated in 10-week rehabilitation training programs consisted of overground locomotor and body weight supported treadmill training for total 60 minutes, 3 times per week. The results showed that the patients improved BBT, 10-MWT, and 6MWT after 10 weeks of intervention. Also they felt confident about falls indicating a positive influence on falling efficacy, and exhibited improvements in quality of life after the intervention. These findings suggest that the benefits of rehabilitation programs adopted in the present study is evident not only for the recovery of locomotion functions but also for the enhanced falling efficacy and quality of life in patients with degenerate cerebellar atrophy.

The Cerebellar damage leads to falls due to the impairments in balance control, and secondary damage is a terrifying factor that may cause injuries. Concerning the fear of falls in patients with cerebellar disease (CD), this study examined the kinematic characteristics on body control abilities such as the movements of the upper and lower limbs and the motion of stepping to recover the balance when a back fall is occurred with increased ground inclination. Eight patients diagnosed with CD and 8 age-matched normal controls (NC) participated in the study. We evaluated the ability to recover the balance independently through stepping movements by causing the situation of a backward fall with the lean-and-release method and four different ground inclination conditions. The results demonstrated that there were no differences between two groups in overall stepping characteristics such as total steps, initiation time and step distance. However, the distance between the foot and body COM was shorter and velocity of COM movement at landing of first step was higher in CD than NC as the ground inclination increased. These findings indicate that patients with cerebellar disease used an inefficient movement strategy that does not help to recover their trunk motion from falling when fear of falling is elevated.
