        신정맥 혈전증을 동반한 일측성 신 림프관확장증

        Renal lymphangiectasia is a rare cystic kidney disorder. This disorder has been termed renal lymphangioma, renal lymphangiomatosis, pararenal lymphatic cyst. We report on a 32-year-old man admitted with complaints of abdominal discomfort, in whom unilateral renal lymphangiectasia with renal vein thrombosis was diagnosed. Abdominal computed tomography revealed left perirenal fluid collection with multiloculation. Left renal vein was completely obliterated due to thrombosis and inferior vena cava (IVC) was partially obliterated. Perirenal fluid collection and renal vein thrombosis were treated initially with percutaneous drainage and anticoagulation therapy. The biochemical features of the drained fluid were compatible with lymphatic fluid. Since there was no decrease in the amount of daily drainage, left nephrectomy and IVC thrombectomy was performed on the 34th day of hospitalization. Left kidney was enlarged (15×10×18 ㎝, 750 g) and composed of numerous smooth, thin-walled cysts. Pathologic diagnosis was renal lymphangiectasia.

        POEMS 증후군에 동반된 사구체신염 1 예

        A POEMS syndrome (polyneur opathy, organomegaly, endocr inopathy, monoclonal M protein, and skin lesion) is a rare multisystem disease of unknown cause with varying clinical manifestations. POEMS-associated renal disease is also a rare condition. We experienced a 42-year-old male who had been suffered from generalized edema, low extremity weakness and pain. He also had a significant hematuria, proteinuria, azotemia, hepatospenomegaly, intraperitoneal lymph node enlargement , hypothyroidism, IgA λtype monoclonal gammopathy, specific skin change and ascites. In renal biopsy, membranoproliferative features with enlarged glomeruli, mesangial proliferation, capillary wall thickening and paucity of immune deposit was noted. We suppose that renal insufficiency could come from POEMS syndrome. His symptoms such as low extremity pain, azotemia and generalized edema were improved by hemodialysis and use of prednisolone. At present, he is under the treatment of prednisolone and melphalan. (Korean J Nephrol 2002;21(1) :178-184)

      • 지자체 GIS사업을 위한 비용효과분석 연구

        투자의 효율성제시가 불명확한 1단계 국가GIS사업(1995-2000)에 대한 대책으로, 최근 GIS예산 집행의 타당성에 대한 분석이 요구되고 있다. 특히 국가GIS사업의 많은 비중을 차지하는 지자제GIS사업에서 효율적인 투자는 성공적인 사업추진을 위해 매우 중요하다. 그러나 GIS투자의 타당성을 판단하기 위해서는 투자비용의 산정과 함께 그로 인한 편익 내지 효과를 측정할 수 있어야 한다. 또 측정할 수 있다면 얼마나 측정할 수 있고 얼마나 설득력있는 편익을 측정하여 정책결정이나 정치적, 재정적 지지를 얻는 데에 활용할 수 있는가가 핵심적인 주요사안으로 받아들여지고 있다. 따라서 본 연구는 지자체에서 합리적인 의사결정의 자료로 활용될 수 있도록 기존의 다양한 GIS비용효과 분석에 대한 이론적 고찰을 실시하고, 지자제GIS의 특성을 고려한 GIS비용효과분석방안을 고찰하였다. 이를 위해 지자체 GIS 발전단계에 따르는 효과 및 지자체 응용시스템별의 활용효과에 대한 국내외사례를 종합, 국내 지자체 GIS비용효과분석에 적용될 수 있는 비용과 효과의 정량적 정성적 평가항목을 구성하였다. 아울러 편익비용비, 순현재가치, 내부수익률의 평가기법에 따라 다양한 대안선정을 위한 민감도분석과 불확실성분석에 대해서도 살펴봄으로써, 지자제 GIS사업의 투자타당성확보 및 합리적인 의사결정 및 대안선택방향을 모색하였다. Because efficiency of the 1st-phase NGIS investment(1995-2000) has not been clearly measured, the action taken in the 2nd-phase NGIS project requires NGIS budgets to be evaluated in terms of effectiveness. Especially, the effective investments in local governments are critical for the NGIS projects, because they execute the much larger amount of budgets in total than other GIS projects. As indicated, for the successful NGIS implementation, it is important to obtain continuous political and financial supports from decision makers. As a persuasion measure for the budget appropriation, CBA(Cost-Benefit Analysis) and CEA(Cost-Efficiency Analysis) can play an important role for the decision makers. The major concern of this paper is how to measure the costs and benefits of the GIS implementation by considering important characteristics of the GIS projects in local governments, and existing theories are reviewed for this concern. The GISs in local governments can have different stages in terms of its evolution and the effectiveness of the applications can be represented variously. To identify categories for measuring costs and benefits of the various GISs, case studies and success stories are reviewed from both the foreign and domestic research. The categories of costs and benefits are determined from the tangible and intangible aspects. The categories for the quantitative and qualitative measure are proposed to evaluate the GISs in local governments. After measuring costs and benefits, three key evaluation methods in cost-benefit analysis are suggested as follows: 1) the benefit: cost ratio (B/C), 2) Internal Rate of Return (IRR), and 3) the net present values (NPV) of the costs and benefits. The sensitivity and uncertainty analysis are also helpful to make a decision for the GIS budget appropriation in local governments. In conclusion, although cost-benefit analysis is not an easy undertaking, it is certain that it can play an important role in the future for the GIS funding decisions in local governments.

        Kimura 병을 동반한 IgA 신증 1 예

        Kimura's disease is a granulomatous disease which develops in the skin, subcutaneous tissues and lymph nodes and which is characterized histologically by the presence of lymphnid follicles, vascular proliferation and infiltration with eosinophils. This disease is mostly confined to the Far East. The frequency of renal involvement in association with this disease is apparently high. But there wasn't any case report of IgA nephropathy associated with Kimura's disease. We report a case of IgA nephropathy associated with Kimura's disease with a review of the literature.

        컴퓨터를 이용한 유아 교육용 학습 자료의 개발에 많은 관심과 연구로 인해 다양한 학습 자료들이 발표되어 있다. 본 논문에서는 패밀리 게임기로 잘 알려진 8 비트 게임기에 유아 교육용 프로그램을 구현하였다. 구현된 프로그램은 단계별 유아 교과 내용을 유아들의 학습 수준을 최대한 고려하여 다단계 학습 구조로 구성하였고, 유야 중심의 상호 작용적인 형태를 취함으로써 학습 효과를 한 단계 끌어 올릴 수 있도록 하였다.

        발효녹용의 균주 선별 및 발효녹용의 생리활성

        본 연구에서는 녹용의 효율을 높이고 아울러 새로운 약리효과를 기대하기 위해 녹용을 분해하는 미생물을 탐색하였으며, 선정된 미생물 이용하여 발효시킨 녹용에 대하여 생리활성을 측정하였다. Bacillus sp., Lactobacillus sp. 및 버섯균사체중 녹용발효에 적합 균주로 B. subtilis KH-15, SCB-3과 버섯 균사체인 Cordyceps militaris, Phellinus linteus, Inonotus obliquus 26136, Inonotus obliquus 26147을 이용하여 발효녹용 제조 시 발효물의 고형분 함량은 B. subtilis KH-15와 SCB-3가 가장 높았으며, total sugar, uronic acid, S-GAG 함량은 C. militaris에 의한 발효물이 각각 1619.3 ㎍/mL, 302.0 ㎍/mL, 119.9 ㎍/mL로 가장 높은 함량을 보였으며, sialic acid 함량도 21.6 ㎍/mL로 B. subtilis KH-15의 24.8 ㎍/mL보다 다소 낮은 함량을 보였다. 면역 증강을 예측할 수 있는 항보체 활성은 B. subtilis 균주로 발효한 경우는 B. subtilis KH-15 균주의 ITCH50값이 42.3%로 SCB-3 29.9%보다 높은 값을 보였으며, 버섯균사체를 이용한 발효 녹용 중에는 C. militaris가 50.1%로 가장 높은 항보체 활성을 보였다. 항산화력을 알 수 있는 ABTS 라디칼 소거능은 B. subtilis KH-15 발효물이 4.97 ㎎/mL로 가장 높은 소거능을 보였으며, DPPH 라디칼 소거능은 Inonotus obliquus 26147가 16.98 ㎎/mL의 IC50값으로 가장 높은 라디칼 소거능을 보였다. 이상의 결과에 의하면 녹용 추출물에 비하여 녹용 발효물은 uronic acid, S-GAG, sialic acid 등 생리활성물질을 많이 함유하였으며, 항보체 활성과 라디칼 소거능이 증진되는 것을 확인하였다. 따라서 녹용자체가 지니는 조혈작용 등 생리활성 외에 항보체 활성처럼 새로운 활성이 발현될 수 있을 뿐만 아니라 라디칼 소거능처럼 기존의 활성을 증진시키는 효과를 가지고 있음을 확인하였다. This study was conducted to isolate strain for the preparation of fermented antler (Cervus cornu parvum) and evaluate its physiological activities. The growth degrees of twenty-one samples from Bacillus sp., Lactobacillius sp. and mushroom strain on antler extract agar were evaluated in this study, and Bacillus subtilis KH-15, SCB-3, Cordyceps militaris, Phellinus linteus, Inonotus obliquus 26136, and Inonotus obliquus 26147 were selected. The fermented antler extract by C. militaris had relatively higher contents of total sugar (1619.3 ㎍/mL), uronic acid (302.0 ㎍/mL), sulfated-glycosaminoglycan (S-GAGs) (119.9 ㎍/mL) and sialic acid (21.6 ㎍/mL) than any other extracts. The anti-complementary activities of all fermented antler extracts were higher than non-fermented antler extract, and among these samples, fermented antler extract by C. militaris showed the highest anti-complementary activity (inhibition of 50% total complement hemolysis, ITCH50; 50.1% at 1,000 ㎍/mL). The ability of fermented antler extract by B. subtilis KH-15 to scavenge 2,2-azino-bis-(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) radical (IC50; 4.97 ㎎/mL) was significantly the highest (p<0.05), whereas the extract from I. obliquus exerted significantly (p<0.05) high 1,1-diphenyl-2-picrylhydrazyl (DPPH) radical scavenging activity (IC50; 16.98 ㎎/mL) among all samples. The results of this study suggest that physiological effects including immuno-modulating and antioxidant activities of the antler may be increased through fermentation process.

        Molecular Level Relationships of Purple Nonsulfur Bacteria and their Relatives

        광합성 세균과 비광합성 세균에 속하는 종들 사이의 유연관계를 파악하기 위해 DNA 혼성화 방법을 실시하였다. 혼성화도는 종내 균주들 사이와 Rhodobacter capsulatus와 Rhodopseudomonas blastica 사이를(72-88%) 제외하고는 전체적으로 낮게 나타났다(2-35%). 광합성 세균 Rhodopseudommonas Palustris와 비광합성 세균 Pseudomonas aeruginosa ATCC 27853, Bradyrhizobium japonicu, 사이의 D%는 광합성 세균 사이의 D%보다 약간 높게 나타났다(26-33%). Rhodopseudomonas blastica와 Rhodobacter capsulatus 사이의 D%는 72%로 유전적 유연관계가 매우 높은 것으로 나타났다. DNA-DNA hybridization by kinetic method was carried out between species of purple nonsulfur photosynthetic bacteria and nonphotosynthetic bacteria. The degrees of homology percent were shown to be low (2-35 D%) with the exception of high homology % (72-88 D%) for strains within a species and between Rhodobacter capsulatus and Rhodopseudomonas blastica. The D% between the purple nonsulfur photosynthetic bacteria, Rhodopseudomonas palustris, and nonphotosynthetic bacteria, Pseudomonas aeruginosa ATCC 27853 or Bradyrhizobium japonicum were a little higher (26-33 D%) than the D% between any other photosynthetic bacteria. The homology % between Rhodopseudomonas blastica and Rhodobacter capsulatus was 72 D%, which showed genetic relationship.

        만성 요통환자의 근저항운동과 Gyrotonic Expansion System(R)이 요부 신근력, 유연성 및 통증변화에 미치는 영향

        This study was aimed at determining the effects of gyrotonic expansion system(R) (GXS) training and MedX training on lumbar extension strength, lumbar flexibility and relief of back pain with patients of chronic low back pain. Twenty-four subject in two group (GXS group: n=12, & MedX group: n=12) were trained twice per week for eight weeks with protocol of MedX exercise and GXS(gyrotonic expansion system(R)) exercise program. Both groups were tested on MedX machine for lumbar extension strength and were measured Schober test for lumbar flexibility and VAS test for Pain change at baseline and after 8weeks. Lumbar extension muscle was categorized into 7 angles (72°, 60°, 48°, 36°, 24°, 12°, 0°). Paired Sample t-test was utilized to analyze time differences and Two-way repeated ANOVA was performed to compare the group differences. Results showed a significant improvement of lumber extension strength for 56.30% in MedX group and for 57.49% in GXS group after 8 weeks at all angles. Result further on lumbar flexibility indicated increase of 6.0% in MedX group and increase of 7.5% in GXS group. Change of back pain significantly was decreased for 43.71% in MedX group and 49.12% in GXS group after 8weeks of training.

        마우스 태생간 조혈세포이식시 이식거부 반응을 극복하기 위한 조혈세포 용량 및 흉선세포의 역할에 관한 연구

        N/A To observe whether an increase in cell doses can overcome graft rejection, whether sustained chimerism is present, and whether fetal thymic cell transplantation has any effect on the survival and the cell dose needed, fetal liver transplantation was done from 17-18-day old DDY fetus to an ICR mouse (syngeneic transplantationl) previously irradiated. The results revealed that better survival was obtained, regardless of syngeneic of allogeneic fetal liver cell transplantion, through an increase of the cell dose. A 90F long- term survial was noticed with a cell dose of 15×108/kg in syngeneic transplantation. In the case of allogeneic transplantation, however, only a 50% long - term survival rate was observed with the same dose. The concurrent thymic cell transplantation had a good effect on the long - term survival and the reduction of cell dose required, The cell dose needed for the achievement of a 90% long - term survival in syngeneic and allogeneic transplantation was 5x108/kg and 15x108/kg, respectively The sustained chimerism was evident and the graft-versus-host disease was mininal in fetal liver cell transplantation. It was evident that fetal liver cell transplantation could be one of substitution in solving the problems of bone marrow transplantation The cell dose required to prevent graft failure decreased with fetal thymic cell transplantation The mechanism of the salutary effect of concurrent thymic cell transplantation is not yet to be elucidated.
