Advances in microvascular techniques and refinements in microsurgical tissue transfers have enabled surgeons to combine different tissue components and reconstruction into a single-stage operation in extensive or composite defect following injury. Some problems and consideration for extensive or composite defects are form, shape, function, and dimension of the defect sites. Therefore combination of two or more flaps is required to reconstruct extensive or composite defect. This paper presents our clinical experience of four cases of combined free flaps with or without sequential microanastomosis in reconstruction of upper extremity based on peroneal flowthrough, thoracodorsal, and dorsalis pedis vascular system. Satisfactory results were obtained without flap loss and complications. The free flaps were combined in th following fashion; two cases by bridge fashion, one by chimeric microanastomosis and one by simple chimeric fashion. The median follow-up time on all patients was 21.7 months. Donor site morbidity was minimal. Extensive soft tissue or composite defects can be effectively covered by various combined flaps. Even though the risk for complication exists, the options of combination with or without sequential microanastomosis can add a functional or sensory dimension to reconstruction of complex wounds.

의료용으로 사용되고 있는 고분자는 체내에 적용했을 경우 초기의 염증반응이 종종 문제점으로 대두되며, 이는 면역세포의 활성산소의 배출과 관련되어 있다. 본 연구에서는 이러한 부작용을 해결하기 위해 항산화제를 필름표면에 코팅하여 활성산소 발생 저해에 의한 항염증 효과를 평가하고자 하였다. Macrophage에 의한 염증 효과를 평가한 결과, 항산화제가 코팅된 polylactic acid 필름에서 pro-inflammatory cytokine의 생성이 저해됨을 확인하였다. 또한 항산화제 코팅 시 코팅제로 사용되는 고분자의 종류 및 분자량에 따른 방출 거동을 관찰한 결과, 고분자의 종류 및 분자량에 따라 항산화제의 방출 제어가 가능하였다. 이는 항염증 반응에 효과적으로 대응할 수 있는 항산화제 코팅 polylactic acid 필름을 제조함으로써 다양하고 광범위한 체내용도 전개가 가능할 것으로 생각된다. Polylactic acid (PLA) films have been widely used in medical devices for surgical treatments. However, their use has been associated with complications, such as inflammation around the PLA film implant area. Several antioxidant agents have been incorporated on the surface of PLA films to suppress initial excessive inflammatory reactions through the quenching of reactive oxygen species (ROS) released from macrophages. The suppression of inflammation was evaluated by measuring the levels of various pro-inflammatory cytokines produced by the macrophages. In addition, by measuring quercetin release behavior, we determined the optimum type and molecular weight of the film coating polymer for the effective suppression of inflammation. These results suggest that the incorporation and controlled release of quercetin could be a promising method to reduce inflammation induced by PLA films.

본 논문에서는 지열을 이용한 PHC, 강관, 동관말뚝 형태인 에너지 파일의 지반 조건별 내부 충진재 열전도도 효율과 겨울철 열교환기 내부 순환수의 유속별(0.3㎧, 0.6㎧, 0.9㎧) 24시간 운용(8시간 운용, 16시간 대기)시 에너지 파일의 열전달 거동을 ABAQUS(ver 6.10)와 FLUENT(ver 6.3.26)를 사용하여 분석하였다. 분석결과 말뚝 재료와 지반조건별 열전달 효율은 동관, 강관, PHC 말뚝순으로 열전달 효율이 좋은 것으로 나타났으며, 말뚝 중 PHC 말뚝 형태의 에너지 파일을 유속별 8시간 운용에 따른 분석결과 경계조건이 일정할 때 유속 조건 중 유입속도 0.6㎧ 일 때 평균 유출온도(275.4K) 및 열교환율(103.1 W/m)로 경제적인 유속으로 판단되었으며, 8시간 운용 후 초기조건과 동일하게 16시간동안 지열이 에너지파일에 공급 시 정상상태와의 온도차는 0.1K로 열량이 충분히 충전되어 에너지 파일의 겨울철 8시간운용 - 16시간대기는 운용상 적절한 것으로 판단된다. This paper presents the thermal conduction analysis (using ABAQUS ver 6.10 and FLUENT ver 6.3.26) of geothermal energy for PHC, steel and copper energy piles by considering subsurface environment, thermal efficiency of grouting materials, and fluid velocity of circulating fluid. Results show that higher thermal efficiency for copper pile is observed followed by steel and PHC piles depending on the grouting materials and subsurface condition. The fluid velocity of 0.6㎧ presents most efficient outflow temperature (275.4K) and heat exchange rate (103.1 W/m) for the case of PHC pile during 8 hours operation. Analysis of operation schedule concludes that 16 hours of stand-by allows charging geothermal energy following 8 hours operation in winter season is most appropriate with 0.1 K of temperature difference from the steady-state condition.
우편 자동화통합망에서 비디오코딩시스템의 분배 알고리즘 성능분석

Video Coding is a handwork coding process performed by an operator at the Video Terminal. He or she manually recognizes address or postal code on the failed letters from the optical character recognition of letter sorting machine. These days, the Integrated Automatic Mail Processing (IAMP) system is thought to significantly improve the production of mail sorting operation. Unlike the current postal code recognition, Video Coding plays an important role in the IAMP system for focusing on recognizing addresses of letters. This is true because the postal code’s recognition rate of sorting machines is about 94%, but the rate of address recognition remains at only 85%. In this paper, I will discuss how to enhance the processing effect of IAMP system, and construct alternate designs of video coding by considering interrelated factors. The developed simulation models are tested on various mailing conditions to check the system performance. From the results, the optimal operation method of video coding system will be suggested.
