Melanotic oncocytic metaplasia (MOM) of the nasopharynx is an extremely rare benign lesion which is characterized by the presence of usually a small, pigmented lesion near the Eustachian tube opening, but never reported the form of cyst without pigmentation. A 76-year-old man who had a symptom with globus pharyngeus for 3 months was presented to otolaryngology clinic. The lesion was found as a rare form of cystic mass without pigmentation through endoscopic examination. Microscopically, there were well-formed glandular structured oncocytic cells with abundant eosinophilic granular cytoplasm. Scattered brown pigments were also noted, which were positive for S-100 and negative for HMB-45 in immunohistochemistry. He was diagnosed with MOM of the nasopharynx. MOM of the nasopharynx should be always taken into consideration about its various clinical forms as a benign mimicker of malignant melanoma.

The olfactory neuroblastoma (ONB) is a rare malignant tumor that arises in the nasal cavity and paranasal sinuses. The most common treatment is the craniofacial resection, but recently endoscopic resection has been used as an initial treatment to reduce complications. In addition, depending on the stage of the disease, radiation therapy or chemotherapy can be performed before or after surgery. ONB can also recur, showing two patterns of recurrence: local recurrence and distant metastasis to cervical lymph nodes and lungs. Local recurrence is usually treated with craniotomy, but this is an invasive treatment method that can cause various complications. Therefore, non-invasive treatment methods such as stereotactic radiation therapy are now being considered. The authors report a recurred ONB case successfully treated with Gamma knife radiosurgery without craniofacial resection, as the patient showed delayed bilateral cervical lymph node metastasis and suspected recurrent intracranial invasion after treatment.
Retiform Hemangioendothelioma Originating in the Paranasal Sinus: A Case Report

Retiform hemangioendothelioma (RHE) is an extremely rare low-grade variant of angiosarcoma that mostly involves the dermis or subcutaneous tissue, but which has never been reported in paranasal sinus. A 53-year-old woman with the problems of a palpable lump on the left lower eyelid and anosmia visited to our hospital. An expansile mass was observed in the left frontoethmoid sinus in radiologic images, which was suspected to be attributable to bony erosion of the orbital wall and intracranial expansion. Endoscopic sinus surgery was performed to remove the mass. Histopathologically, tissues were infiltrated by arborizing vessels arranged in a retiform pattern. The immunohistochemistry also showed a positive result for CD31 and a partial positive for Ki-67. Because the recurrence was suspected 4 months after surgery, chemoradiotherapy following second operation was performed. Ultimately, we report a rare case of RHE in the nasal cavity.

We report a case of metastasis of hepatocellular carcinoma (HCC) into an isolated sphenoid sinus cavity presenting with intractable nasal bleeding. A 62-year-old man who diagnosed with HCC, visited our department with recurrent nasal bleeding for a month. Endoscopic examination and imaging demonstrated a homogeneous enhancing solid mass in the right sphenoid sinus. Although several anterior nasal packing was performed, epistaxis of oozing pattern persisted. Eventually, nasal bleeding was successfully controlled after palliative radiation therapy.

Background and Objectives The cause of idiopathic sudden sensorineural hearing loss(ISSNHL) is still unclear, but recently, chronic inflammation and thrombosis have received attention. The neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) aresome of the markers that show the state of inflammation and ischemia, which are measuredroutinely in the complete blood cell count (CBC) test. The aim of this study were to investigatethe relevance of NLR and PLR with ISSNHL. Subjects and Method Enrolled in our retrospective analysis were 103 patients diagnosedwith ISSNHL. Blood samples were taken from the patients and hearing assessments were performed. NLR and PLR were calculated using the CBC results. Then the patients were dividedinto 4 groups using Sigel’s criteria according to their response to the treatment, which wereagain classified two groups, the “recovered” and “unrecovered” group. Results NLR, PLR, and neutrophil values of the unrecovered group were significantly higherthan the recovered group (p=0.002, p=0.009, and p=0.038, respectively). On the other hand,lymphocyte values were significantly higher in the recovered group (p=0.007). After adjustmentin a multivariate logistic regression analysis, NLR was associated with the recovery ofISSNHL (Odds ratio=1.290, p=0.042). In addition, NLR and PLR values were also significantlydifferent between the groups classified by the Sigel’s criteria (p=0.009 and p=0.029, respectively). Conclusion PLR values may be useful in predicting hearing recovery after treatment in patientswith ISSNH. It is also expected to be a potential marker for predicting the prognosis and determiningfurther treatment options.

Aspirin exacerbated respiratory disease (AERD) is a well-known disease characterized by the triad of bronchial asthma, nasal polyps, and aspirin intolerance. AERD has been known to be the most severe form of chronic rhinosinusitis, marked by a larger polyp burden and higher rates of recurrence after endoscopic sinus surgery (ESS). Although ESS improves sinonasal symptoms and asthma severity score, it does not address the underlying abnormal metabolism of arachidonic acid. Herein, we describe two cases of AERD presented as recurrent nasal polyposis after ESS, which was successfully treated with aspirin desensitization following ESS.

Complete removal is most important in treating the malignant tumor from the auricle, but the cosmetic and reconstructive plan after removal should also be considered because auricular appearance contributes enormously to the facial aesthetics. Mohs micrographic surgery (MMS), which is commonly used in dermatology, is considered as minimal marginal surgery that offers superior cure rates in the treatment of facial skin cancer. Therefore, MMS could provide a minimally invasive way to manage the malignant tumor of the auricle. This article reports that basosquamous cell carcinoma arising in the auricle was treated with wide resection combined with MMS. Conclusively, we could accomplish the surgical purpose of both completely removing the tumor and maintaining the auricular aesthetic shape by additionally introducing MMS in the treatment.

Background and Objectives The aims of this study were to evaluate the rate and risk factors involved in bilateral central lymph node metastasis in patients with papillary thyroid cancer (PTC) found in the isthmus and compared them to tumors located in other thyroid regions, using those findings to establish a surgical strategy for treating these tumors. Subjects and Method We compared the clinical and pathological data of 48 patients with isthmic PTC and 141 patients with PTC found in other thyroid regions, all of whom underwent total thyroidectomy and bilateral central neck dissection. Results The rates of bilateral central lymph node metastasis were higher in the isthmus group than in the non-isthmus group (29.2% vs. 9.9%; p=0.001). On multivariate analysis, the isthmic location of the tumor was an independent risk factor for bilateral central lymph node metastasis (OR=3.458; p=0.005). But the positional relationship between the tracheal midline and the nodule was not clear in lymph node metastasis in the isthmus group. Conclusion Bilateral central neck dissection should be considered for isthmic PTC regardless of the relation between nodule and tracheal midline because of the high rate of bilateral central lymph node metastasis. Inverted papilloma is a disease in which complete removal of the tumor is essential due to thepossibility of malignant transformation. The frontal sinus is not always easy to access endo-scopically because of its location and structural complexity. Therefore, an external approachis sometimes required for tumor control. The conventional method has been radical surgerywith osteoplastic flap (OPF) using bi-coronal incision although postoperative complicationsneed to be considered. A 42-year-old male was diagnosed with inverted papilloma originatingin the frontal sinus, and the lesion was removed through endoscopic approach. However, it re-curred 20 months after surgery, so an external approach was considered for the complete re-moval of the tumor. To minimize complications, we performed using mini OPF via sub-eye-brow incision, with success. Mini-OPF may be an optional treatment for recurring cases ofinverted papilloma frontal sinus that is endoscopically unreachable.

Retrobulbar hematoma is one of the major complications following endoscopic sinus surgery (ESS). Symptoms and physical findings include temporary blindness, ophthalmoplegia, mydriasis, ptosis, proptosis, and eyelid ecchymosis. Recently, we experienced a rare case of the delayed retrobulbar hematoma with intact lamina papyracea after ESS. Therefore, we present this case with an emphasis on the importance of prevention, identification and management of delayed retrobulbar hematoma.
