Background: There are few epidemiological studies on patch test with additional batteries in Korea. Objective : We report the results of patch tests with additional batteries which are relatively uncommonly used in contact dermatitis patients. Methods : Standard patch testing was performed with Finn chamber on Scanpor tape. We used additional batteries including cosmetic and fragrance series which are commonly used, and the others including 14 batteries(so-called other batteries). Results : Over the last 8 years, 719 patients(male 184, female 535) were tested in our contact, dermatitis clinic. 178 patients(24.8%) were tested with both Standard and other batteries, In other batteries, rubber additive series were most frequently used(9.2%), and textile colours finish(4.3%) and hairdressing series(3.6%) were used in order of frequency. Rarely performed batteries were(meth) acrylate ser ies, photographic chemicals series, wood set, oil & cooling fluid series, and various allergens. Patients tested with steroid batteries did not show positive reactions. Conclusion : We suggest that 6 batteries((meth)acrylate series, photographic chernicals series, wood set, oil & cooling fluid series, various allergens, steroid battery) have law testing frequencies and positive rates. N/A Histopathologic Findigs of Normal Scalp and Alopecia Areata in Transverse Sections Jin Ho Chung,M.D., Hee Chul Eun, M.D.* Department of Dermatology, Capital Armed Forces General Hospital Seoul, Korea Department of Dermatology*, College of Medicine, Seoul National University, Seoul, Korea To observe the normal transverse histopathological findings of the follicles in the human scalp, a 4-or 6-mm punch biopsy specimen was taken from the normal scalp in 5 normal adults and 10 alopecia areata patients. We cbserved the normal transverse findings of the follicles in the transverse sections of normal scalps. The histopathological findings of the transverse sections taken from patches of 10 alopecia areata patients were also observed. The results were as follows ; 1) The follicular unit which usually consisted of 2-4 terminal follicles and I vellus follicle, sebaceous lobules and arrector pili muscles was observer in transverse section. The normal density of follicular units was about 0.62/㎣. 2) There were differences in the transverse sectional findings of the hair follicle according to levels, such as mode of keatinization in outer root sheath. 3) In the transverse section of the scalp biopsy specimen, it was possible to differentiate the various phases of the hair growth cycle. 4) The transverse sections of the patches in alopecia areata showed that 23%-100% of follicles were in telogen stage. 5) The observation of transverse sections of scalp biopsy specimen seems to be a useful method in the histopathologic study of the hair follicle disorders particularly alopecia areata.
인체 각질형성세포 배양모델을 이용한 수종 용제의 독성도 측정에 관한 연구

Background: As in vivo models such as animal and human tests have many potential problems the keratinocyte culture model has previously been used as an in vitro model for testing skin irritancy for common skin irritants. Objective : To determine the skin irritant potency of several solvents, we employed cultured human keratinocytes as an in vitro model. Methods : To evaluate the cytotoxicity of solvents, the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide(MTT) test, lactic dehydrogenase(LDH) release test, and tritiated thymidine incorporation test were used. Results : Dose dependent decrease of cell viability and DNA synthesis, and dose dependent increase in leakage of LDH liberation were observed in normal cultured human keratinocytes after exposure to several solvents. The cytotoxicity potency of several solvents measured by each method were slightly different. Conclusion : These observations suggest that the cultured human keratinocyte model would be useful in evaluating the cytotoxicity of solvents. (Kor J Dermatol 1996;34(1): 58~63)

The incidence of mercury sensitivity in millitary personnel without contact dermatitis was investigated using the patch test with 1% ammoniated mercury and 0.1% thimerosal. A questionaire study relating to mercury exposure was also performed in 124 subjects. We also investigated whether the several vaccines curently in use in Korea contain mercury products. The results were as follows . 1. Ammoniated mercury sensitivity was in 10.8% and thimerosal sensitivity was 6.1% in all tested subjects, a similar percentage observed in patch test clinic of the authors. 2. Vaccination history was statistically related significantly, to the senstivity to mercury allergens (P<0.05). 3. All subjects sensitized to thimerosal were also allergic to ammoniated mercury, suggesting crosssensitivity. 4. Hepavax-B, D. P.T., Japanese encephalitis vaccine, and fluvax, currently in use in Korea, contain thimerosal as a preservative. (Kor J Dermatol 1992; 30(3): 347-353)

Background: The classical in vitro method for researching the fibroblast metabolism is a mono layer culture system. But there are a lot of facts that artificial influence of monolayer culture situations do not resemble the in vivo conditions. Recently three-dimensional culture method of fibroblast in collagen gel was developed and in this system fibroblasts seem to behave like in vivo situations. Objective : So we examined the effects of ascorbic acid. TGF-b, and IFN-r on fibroblast in collagen gel culture system and then compared these effects with those in monolayer culture system. We also studied the regulatory mechanism of collagen gene by ascorbic acid, TFG-b, and IFN-r by Northern blot analysis in monolayer culture system. Methods : Using fibroblasts of the neonatal foreskin, we measured the collagen synthesis after treatment of ascorbic acid, TGF-b, and IFN-r in three-dimensional culture and monolayer culture system. We aslo checked the change of mRNA of collagen after treatment in monolayer culture. Results : The relative levels of collagenase-digestible protein synthesis in the three-dimensional culture seemed to be lower than those in the monolayer system. However, difference of fold in crease was not statistically significant. The expression of collagen mRNA was increased after treatment of TGF-b and decreased after IFN-r treatment. Conclusion : From these results, fibroblasts in the three-dimensional culture system might resemble the in vivo conditions The molecular level of regulation of collagen synthesis by TGF-B. and IFN-r was thought to be transcriptional. The three-dimensional culture system could be a good model of culture system of fibroblast in studying of aging process or the role of cytokine in wound healing. (Kor J Dermatol 1996;34(4) : 608~615)

Background: Long-term oral itraconazole and terbinafine are widely used in the treatment of onychomycosis. Accelerated nail growth in patients with itraconazole has been described in several reports. However, there has been no report regarding the effect of antifungal agents on cultured nail matrix cells(NMCs). Objective :We applied several antifungal agents on cultured human NMCs and epidermal kera- tinocytes(EKs) to compare the cytotoxicity of several antifungal agents and also to verify possible stimulating effects of itraconazole and 6-hydroxyitraconazole on nail growth. Methods : To evaluate the effect of antifungal agents, the 3-(4,5-dimethylthiazo1-2-yl) 2,5-diphenyl- tetrazolium bromide(MTT) test, tritiated thymidine incorporation test, and lactic dehydrogenase(LDH) leakage test were used. Results: Dose dependent decreases in cell viability and DNA synthesis, and dose dependent increases in LDH liberation were observed in cultured human NMCs and EKs after exposure to several antifungal agents. The dose-response reaction patterns for NMCs and EKs to antifungal agents were similar. The cytotoxicity potency of several antifungal agents measured by each method were slightly different. Itraconazole and 6-hydroxyitraconazole did not show stimulating effects on cell proliferation in in vitro monolayer cell culture systems. Conclusion : These observations suggest that itraconazole appeared less cytotoxic but showed no stimulating effects on nail matrix cell proliferation in vitro. Cultured human EKs as well as NMCs may be useful in evaluating the effects of agents which are involved in nails. (Korean J Dermatol 1998;36(3): 415-421)

Background : Neurofibiomat,osis type 1(NF-1) is a multisystemic disorder of genetic ori gin, affecting one in every 3000 to 4000 people. It is clinically important in the aspect of dermatology, pediatrics, orthopedic surgery, neurology, neurosurgery and ophthalmology. Objective : The purpore of this study was to elucidate the clinical characteristics of NF-1 in Korean people. Methods : We carried out a retrospective study on 112 patients which were compatible to the diagnostic criteria of Riccardi and Neurofibromatosis Conference Statement. The results were compared with other western studies. Results : The age of onset, sex ratio, family history of neurofibromatosis, and clinica features of cafe-au-lait spot, neurofibroma, and axillary freckinings did not differed from western countries. However, some characterist,ics of NF 1(e.g. Lisch nodule) were not as sessed in the most of the cases and incomplete evaluations of the systemic diseases wen found. Conclusion : In this study t.he clinial features of NF-1 did not differ from western coun tries in many aspects. A more intensive evaluation of patient,s status is needed to manag; NF-1 patients appropritely. (Kor J Dermatol 1995;33(6): 1091-1098)

N/A Background : In white people, a distinction between intrinsic aging and photoaging has been well documented in histologic studies. However, no histological studies to date have attempted to announce morphological changes with photoaged skin of Koreans. Objective : Our purpose was to compare the histologic change with advancing age between exposed skin and nonexposed skin of Koreans. Methods : Twenty-one male volunteers between 20 and 80 years of age were recruited. The histochemical and immunhistochemical studies were performed with the skin obtained from face and buttock. Results : 1. With advancing age, there was a decreasing tendency in the epidermal thickness. However, there was no difference between exposed skin and non-exposed skin. 2. Density of dopa-positive melanocytes was much higher in the face than in the buttock at all ages. Decline of melanocyte density with advancing age is not prominent in the facial skin. 3. In the facial skin, accumulation of thickened degraded elastic fibers and a decrease in collagen fibers are evident with advancing age. In the facial skin, increase of acid mucopolysaccharides is also found with advancing age. Conclusion : Koreans, the histologic changes in the skin with photoaging are distinct from those in the skin with intrinsic aging. (Korean J Dermatol 2003;41(6) : 754~760)
