전이성 골종양에서 $^{18}F$ FDG PET/CT를 이용한 원발성 악성 질환의 진단

목적: 전이성 골종양에서 $^{18}F$ FDG (fluorodeoxyglucose) PET (positron emission tomography)/CT를 이용한 원발성 악성 종양의 진단적 유용성에 대해 보고하고자 한다. 대상 및 방법: 2003년 12월부터 2007년 12월까지 $^{18}F$ FDG PET/CT를 시행받았던 5,452명을 대상으로 후향적 연구를 시행하였다. 이들중 악성 종양의 과거력이 없으며 근골격계 동통을 호소하며 단순 방사선 검사에서 경계가 불분명한 골 파괴를 보이는 환자들에 대해 $^{18}F$ FDG PET/CT를 시행하였던 환자를 대상으로 하였으며 이들중 검사 결과에서 명백히 양성(definitively positive)으로 판정된 예만을 대상에 포함시켰다. 해당기간 중 18F FDG PET/CT를 시행한 총 5,452례중 180례(3.3%)가 기준에 해당되었으며 남자 96례, 여자 84례였다. 연령 분포는 22~90세였으며 평균 연령은 59.1세였다. 이들에 대해 원발성 악성 종양의 진단율, 나이 성별에 따른 원발성 악성 종양의 발생 및 전이성 골종양의 발생 위치에 대해 분석하였다. 결과: 원발 병소를 진단한 경우는 152례(84.4%)였으며 진단된 원발성 악성 종양은 폐암이 51례(28.3%), 유방암이 36례(18.9%), 소화기계암이 30례(16.7%)의 빈도를 보였으며 원발성 악성 종양에 의한 가장 흔한 해부학적 골전이 위치는 척추였다. 결론: $^{18}F$ FDG PET/CT는 원발성 악성 종양의 과거력이 없는 전이성 골종양 환자에서 원발 병소를 검출할 수 있는 효과적인 방법으로 생각된다. Purpose: To evaluate usefulness in diagnosing primary malignant lesion of metastatic bone tumor using $^{18}F$ FDG PET/CT. Material & Methods: Retrospective analysis was executed on 5,452 patients who were taken with $^{18}F$ FDG PET/CT between December 2003 and December 2007. 180 patients who had not any history of malignancy and complained musculoskeletal pain and had ill-defined osteolytic lesion in plain X-ray, were included. 96 male and 84 female were enrolled and mean age was 59.1 year old (22~90). We analyzed diagnostic accuracy, age and sex distribution of primary malignant lesion, location of metastatic lesion. Results: We could confirmed primary malignant lesion in 152 cases (84.4%). Most common malignant primary lesion was lung (28.3%), breast (18.9%) and gastrointestinal system (16.7%) and spine was the most common metastatic location of primary malignant lesion. Conclusion: $^{18}F$ FDG PET/CT is a effective molecular imaging detecting primary malignant lesion in patients having metastatic bone lesion without history of malignancy.

3D design technologies are going to disseminate and apply. So we conducted a survey about classification system for the 3D design information. The 3D design information may also be called “BIM (Building Information Modeling)” includes properties in the 3D drawings and is the technologies using this. The object-oriented classification is necessary for efficient information exchanges in this condition. By this, the object-oriented classification is based on the ISO 12006-3. This study analyzes the status of classification and the current development of the object oriented classification. In the end, we try to find a way to develop the domestic object-oriented classification.

작업분류체계 기반 공사비 작성을 위한 지원도구 개발

공사비정보와 일정정보는 사업을 관리하는데 있어 매우 중요한 정보로 이들이 연계활용되어야 공정공사비 관리를 원활하게 진행할 수 있다. 그러나 국내의 실정은 공사비가 내역체계 기반으로 관리됨에 따라 설계사에서 작성되어 시설, 부위, 공간 등의 정보를 갖고 있던 수량정보가 내역체계 기반으로 전환되면서 정보들을 상실하고 시공사에서는 다시 물량분개라는 작업을 통해 다시 시설, 부위, 공간 등의 정보를 부여하는 작업이 발생하고 있다. 이에 도로분야 사업을 대상으로 작업분류체계(WBS)를 그 연계방안으로 제시하고 작업분류체계 기반 공사비 작성을 통해 설계사에서 작성된 정보를 시공사에서까지 활용할 수 있도록 하고자 한다. 본 사업에서는 이러한 작업분류체계 공사비 작성을 위한 지원도구 개발을 위해 업무 프로세스 검토, 지원도구 활용 공사비 작성, 시험적용을 통한 지원도구 검증을 수행 할 계획이다. 이를 통해 내역정보와 공정정보의 연계가 실현되고, 효율적인 공사비 관리의 기반을 제공할 수 있다.

The introduction of Workflow is essential to the flexible application of system in the construction information and the automation of widely dispersed information. It prevent the delay and manage project efficiently.<br/> The purpose of this study is not the system building that carry out the simple work and hold the information in common, but that manage the construction work with priority given to a process. So, this study focus on the suitable definition of process in the design of the document management sustem of construction business based on workflow.<br/> The result of this study have a meaning of the basic research of system integration based on workflow. Process will be defined effectively through the presented process modeling methods. And also, the existing process can reuse when the process modeling methods is used.

건설사업 프로세스의 통합 정보화를 위한 포털시스템 개발

본 논문에서는 건설사업 프로세스의 통합 정보화를 위한 포털시스템을 제안하고, 이 시스템의 개발 및 운영에 대해 기술한다. 본 논문에서 제안된 포털시스템은 정책 결정이나 실질적인 업무를 담당하고 있는 기관사용자를 대상으로 한 기관포털시스템과 관련업체 및 일반 사용자를 대상으로 한 대민포털시스템으로 구성된다. 본 논문에서는 포털시스템의 개발 방법론으로 관리기법(Method/1)을 본 논문에 맞게 커스터마이징하여 사용하였으며, 자바 기반의 웹 어플리케이션 프레임워크를 적용하여 시스템을 개발했다. 포털시스템 개발 후에는 단계별 테스트를 실시하여 시스템의 품질을 향상시켰으며, 시스템의 안정적인 운영과 신속한 유지보수를 위해 Help-desk를 운영하고 사용자 및 운영자 교육을 실시하였다. 향후에는 외부 연계 기능 및 추가적으로 요구되는 컨텐츠를 개발할 계획이다. In this paper, we develop a construction business information portal system. Our portal system consists of two major parts including a institution portal and a public portal. The institution portal is for the people working at governmental agencies and the public one is for general users. This paper adopted Method/1 as the development methodology of the portal system and made the portal using Java-based web application framework. After the development of the portal. we tested it step by step to improve its quality and run Help-desk for the stable operation and maintenance. In the future, we will develop some additional contents required by users.

수치지도 갱신을 위한 건설CALS 표준의 확장개발에 관한 연구

The construction CALS standards are now going to spread into RCMO(Regional Construction Management Office) and public organization. The management and maintenance of the CALS standards is required for revising the standard to accept the request from other organizations and disseminating the standards and improving the standard supporting tools. The National Geographic Information Institute in the Ministry of Land, Transport and Maritime Affairs made a public announcement of "the Guideline of CAD drawing for the numeric map modification". So, prompt modification of the national geographic information using as-built drawing is essential and information integration between CAD drawing and nuneric map is necessary. The purpose of this study is to develop the construction CALS standards for the numeric map modification.

건설분야 수량산출정보의 연계 및 교환을 위한 디지털 수량산출정보 교환표준과 지방국토관리청의 도로관리관점에 따라 구성된 작업분류체계를 바탕으로 도로분야 수량산출정보를 지원도구를 통해 생성한다. 건설사는 지원도구에서 작성한 도로공사 수량산출정보를 건설사업관리시스템에 등록함으로써 공정·공사비 관리에 활용하고 있다. 본 연구는 공정·공사비의 통합관리를 통해 실질적인 사업관리가 가능하도록 10개 도로건설공사 시범사업을 선정하여 문제점과 개선방안을 도출하였다.

Sj$\"{o}$gren 증후군은 외분비샘에 임파구 침윤으로 발생한 안구와 구강 건조를 특징적인 증세로 하는 만성적이며 서서히 진행하는 자가면역질환이나 저자들은 건조성 증세 없이 전신적 증세만을 호소하였으며 방사선 검사상 다발성 근골격계 전이로 오인되었으나 신세뇨관 산증을 동반한 일차성 Sj$\"{o}$gren 증후군으로 확진되었던 증례를 경험하였기에 보고하고자 한다. Sj$\"{o}$gren's syndrome is a systemic, autoimmune disorder of the exocrine glands with associated lymphocytic infiltrates of the affected glands and characteristic of dry eye and mouth. We report a case of Sj$\"{o}$gren's syndrome in which the patient presented systemic symptoms without sicca symptoms and had radiologic findings mimicking multiple skeletal metastasis and was finally confirmed with Sj$\"{o}$gren's syndrome with renal tubualr acidosis.

도로분야 공사비정보 관리를 위한 설계변경 기능 개발

국토해양부는 지방국토관리청의 도로공사와 관련하여 발생하는 공사비 정보의 교환 및 공유를 목적으로 작업분류체계 기반의 공사비 관리 항목을 추가하여 전자설계도서 작성·납품 지침을 개정하였다. 이를 반영하고 문제점 개선 및 활성화를 위하여 5개 지방국토관리청에서 관리중인 10개 도로건설공사를 시범적용 사업으로 지정하여 발생하는 공사비 정보를 디지털 수량산출정보로 관리하고 있다. 시범사업을 진행하면서 건설공사에 발생하는 설계변경 업무를 전자화하여 관리하는 기능을 개발하고 추가 적용하였다. 이를 통해 건설사에서 생성한 설계변경 정보를 건설사와 발주처가 동일한 형태로 공사비 정보를 공유 및 관리 가능하도록 하였다.
