Background/Aims: Healthcare-associated pneumonia (HCAP) occurs outside hospitals, but its characteristics are similar to those of hospital-acquired pneumonia (HAP). We evaluated the clinical and microbial characteristics of HCAP in Korea. Methods: Of 130 subjects with suspected pneumonia, 49 were classified as HCAP and 81 as HAP. We retrospectively examined the clinical presentations, outcomes, pathogens, and drug resistance rates of Pseudomonas aeruginosae in both groups. Results: The clinical presentations, including the symptoms and laboratory findings, at the time of hospitalization were comparable in both groups. The hospital mortalities of HCAP (28.6%) and HAP (34.6%) did not differ significantly; the length of the hospital stay was similar for all of the survivors (14 vs. 17 days, respectively). Of the identified pathogens, methicillin-resistant Staphylococcus aureus was significantly less common in HCAP than in HAP (two vs. 18 cases, respectively, p<0.01), whereas Klebsiella pneumoniae was more common in HCAP (five vs. zero cases, respectively, p<0.01). The frequency of other Gram-negative rods was similar in both groups. The rate of resistance to antibiotics in P. aeruginosae in both groups was substantial, with the highest resistant rate to ciprofloxacin (50% and 61.5% in HCAP and HAP, respectively). Conclusions: Although the clinical features and outcomes of HCAP were comparable to those of HAP in the study population, the frequency of methicillin-resistant S. aureus was significantly lower in HCAP compared to HAP. 농림위성의 목표 수직기하 정확도 결정을 위한 남한 지역 수치표고모델 상대 오차 분석

식량자원의 확보와 환경생태계에 매우 중요한 요소인 산림과 농경지는 정기적인 모니터링이 요구된다. 농림 위성 영상 자료는 우리나라의 기존 산림 및 농경지 모니터링 방법의 보완재로 효과적으로 활용될 것으로 예상되고 있다. 농림위성의 발사 이전에 사전연구로써 목표 수직기하정확도 산정을 위해 수치표고모델 분석을 수행하였다. 특히 농림위성의 주요 관심 지역인 우리나라 산악지역과 농경지의 특성을 고려하여, 경사도와 식생에 따른 분석을 수행하였다. 공주, 제주 그리고 삼척 지역에 대하여 Shuttle radar topography mission digital elevation model과 Copernicus digital elevation model를 가을과 겨울에 촬영한 드론 LiDAR 수치표면모델 그리고 국토지리정보원 5 m 수치지형모델을 기준으로 평균 상대오차를 비교했다. 그 결과 Shuttle radar topography mission digital elevation model은 8.35, 8.19, 그리고 7.49 m의 상대오차를 나타냈으며, Copernicus digital elevation model는 각각 5.65, 6.73, 그리고 7.39 m의 상대 고도 오차를 나타냈다. 남한 전체에 대하여 Shuttle radar topography mission digital elevation model과 Copernicus digital elevation model를 국토지리정보원 5 m 수치지형모델을 기준으로 지형 경사에 따른 상대 고도 오차를 나타냈다. 그 결과 경사도 0°~5° 사이에서 Shuttle radar topography mission digital elevation model과 Copernicus digital elevation model의 상대 고도 오차는 각각 약 3.62와 2.52 m로 Shuttle radar topography mission digital elevation model의 오차가 더 큰 것으로 산출되었다. 하지만 경사도가 증가함에 따라 이러한 양상은 반전되어 경사도 35° 이상에서는 각각 10.16, 그리고 11.62 m 로 Copernicus digital elevation model의 상대오차가 더 크게 나타났다. Forest and agricultural land are very important factors in the environmental ecosystem and securing food resources. Forest and agricultural land should be monitored regularly. CAS500-4 data are expected to be effectively used as a supplement of monitoring forest and agricultural land. Prior to the launch of the CAS500-4, the relative canopy height error analysis of the digital elevation model on South Korea was performed to determine the vertical target accuracy. Especially, by considering area of interest of the CAS500-4 (mountainous or agricultural area), it is conducted that vertical error analysis according to the slope and canopy. For Gongju, Jeju, and Samcheok, the average root mean squared differences were calculated compared to the drone LiDAR digital surface models, which were filmed in autumn and winter and the 5 m digital elevation model from the National Geographic Information Institute. As a result, the Shuttle radar topography mission digital elevation model showed a root mean squared differences of about 8.35, 8.19, and 7.49 m, respectively, while the Copernicus digital elevation model showed a root mean squared differences of about 5.65, 6.73, and 7.39 m, respectively. In addition, the root mean squared difference of shuttle radar topography mission digital elevation model and the Copernicus digital elevation model according to the slope angle were estimated on South Korea compared to the 5 m digital elevation model from the National Geographic Information Institute. At the slope angle of between 0° to 5°, root mean squared differences of the Shuttle radar topography mission digital elevation model and the Copernicus digital elevation model showed 3.62 and 2.52 m, respectively. On the other hands root mean squared differences of the Shuttle radar topography mission digital elevation model and the Copernicus digital elevation model respectively showed about 10.16 and 11.62 m at the slope angle of 35° or higher.
TerraSAR-X 영상으로부터 Modified U-NET을 이용한 홍수 매핑

The rise in temperature induced by global warming caused in El Nino and La Nina, and abnormally changed the temperature of seawater. Rainfall concentrates in some locations due to abnormal variations in seawater temperature, causing frequent abnormal floods. It is important to rapidly detect flooded regions to recover and prevent human and property damage caused by floods. This is possible with synthetic aperture radar. This study aims to generate a model that directly derives flood-damaged areas by using modified U-NET and TerraSAR-X images based on Multi Kernel to reduce the effect of speckle noise through various characteristic map extraction and using two images before and after flooding as input data. To that purpose, two synthetic aperture radar (SAR) images were preprocessed to generate the model's input data, which was then applied to the modified U-NET structure to train the flood detection deep learning model. Through this method, the flood area could be detected at a high level with an average F1 score value of 0.966. This result is expected to contribute to the rapid recovery of flood-stricken areas and the derivation of flood-prevention measures.

Two Hahns Macaws (Ara nobilis nobilis), three and nine-month-old, with a three-weeks history of weight loss and anorexia were diagnosed presumptively with proventricular dilatation disease (PDD) by radiographic examination. The birds were treated with antimicrobials, analgesics, and fluid administration.However, these birds died three weeks after the first signs of PDD. At necropsy, the birds had severely dilated proventriculus, severe pectoral muscles atrophy, and blood vessels congestion on cortex of cerebrum. The partial matrix gene of avian bornavirus (ABV) was detected by RT-PCR from tissues of the brain, feather calami, and proventiculus of each PDD-suspected birds. This report describes the first detection of ABV in PDD-suspected Hahns Macaw in Korea.

Total 18 propolis samples were collected from different regions in Korea and were analyzed for the contents of total flavonoids, active compounds, and anti-bacterial activity. In the analysis of the active compound contents, the total flavonoid and active compound contents varied according to the production regions, but all samples showed high contents of caffeic acid, cinnamic acid, chrysin, flavone, carbohydrate, protein, fat, and minerals. In the analysis of the anti-bacterial activities, all propolis samples showed highest anti-bacterial activity against Bacillus subtilis (KCTC 1022), and also showed relatively high activity against Salmonella typhimurium (KCTC 2515), Candida albicans (KCTC 7965), Staphylococcus aureus (CCARM 201), and Pseudomonas aeruginosa (CCARM 225).

Nontuberculous mycobacterial infection is rarely accompanied by pleural involvement. We report a very rare case of Mycobacterium intracellulare pulmonary disease accompanied with pleural effusion. A 76-year-old man was admitted to our hospital because of dyspnea. A chest radiograph and CT showed bilateral bronchiectasis and bronchopneumonia accompanied with right pleural effusion. The fluid was lymphocyte-dominant exudative effusion, and microbiological examinations of the effusion, including staining and culturing, proved negative. However, one month after admission, subsequent cultures of bronchial washing fluid revealed the presence of M. intracellulare. The patient`s effusion was gradually resolved with antibiotic treatment, including clarithromycin. (Korean J Med 75:475-478, 2008)
