        복강경 결직장 수술 전 장준비: 경구 기계적 장세척군과 단순관장군의 전향적 비교연구

        Purpose: Although most randomized trials demonstrated no advantage of mechanical bowel preparation for colorectal resection, an oral solution is still widely used. The aims of this study were to evaluate whether a single phosphate enema is as effective as oral polyethylene glycol (PEG) solution in preventing anastomotic complications after laparoscopic colorectal surgery and to examine the clinical courses of anastomotic complications. Methods: Between September 2006 and December 2007, 309 patients underwent laparoscopic colorectal resection with primary anastomosis. The bowel preparation used was PEG solution during initial period (PEG group), but since February 2007, a single phosphate enema (enema group) was utilized. Postoperative data were prospectively recorded. In patients with anastomotic complications, the clinical course was compared between the two groups. Results: There were 150 patients in the PEG group and 159 patients in the enema group. Demographics did not differ between the two groups. Anastomotic leakage occurred in 3.3 percent of the patients in the PEG group and 5.7 percent of the patients in the enema group (P=0.326). The rates of anastomotic bleeding were 2.0 and 2.5 percent, respectively (P=0.761). The hospital stays for patients with anastomotic complication were not different between the two groups (P=0.137), but patients in the PEG group (80%) needed reoperation more frequently than those in the enema group (11.1%) (P=0.023). Conclusion: These results suggest that laparoscopic colorectal surgery may be safely performed with a single phosphate enema instead of oral polyethylene glycol.       • 3차원 매칭을 통한 웹기반 금형판별 시스템

        The purpose of this study is to develop a Web-based mould identification system. A web-based mould identification system matches specific feature information obtained from vision and OMM system with CAD database. The system is exchange message between a server and a client by making of ActiveX control, and the result of mould identification is shown web-browser at remote site. Proposed mould identification process performs a 2D matching process which compares feature extracted from 2D vision image with CAD database before the information of the volume, specific surface of 3D object using reverse engineering is used to differentiate each mould with composite surface. The results are simulated and analyzed to verify the effectiveness of the proposed methods.

        Relationship between the Severity of Diversion Colitis and the Composition of Colonic Bacteria: A Prospective Study

        Background/Aims: Diversion colitis is the inflammation ofthe excluded segment of the colon in patients undergoingostomy. It has been suggested that a change in colonic floramay lead to colitis; however, direct evidence for this diseaseprogression is lacking. The aim of this study was to evaluatethe relationship between the severity of diversion colitis andthe composition of colonic bacteria. Methsods: We used culturemethods and polymerase chain reaction to analyze thecolonic microflora of patients who underwent rectal cancerresection with or without diversion ileostomy. In the diversiongroup, we also evaluated the severity of colonoscopic andpathologic colitis before reversal. Results: This study enrolled48 patients: 26 in the diversion group and 22 in the controlgroup. Significant differences were observed between thetwo groups in the levels of Staphylococcus (p=0.038), Enterococcus(p<0.001), Klebsiella (p<0.001), Pseudomonas(p=0.015), Lactobacillus (p=0.038), presence of anaerobes(p=0.019), and Bifidobacterium (p<0.001). A significantcorrelation between the severity of colitis and bacterial compositionwas only observed for Bifidobacterium (p=0.005,correlation coefficient=-0.531). Conclusions: The colonicmicroflora differed significantly between the diversion andcontrol groups. Bifidobacterium was negatively correlatedwith the severity of diversion colitis.

      • 박리-재부착 이후의 재발달 난류경계층 I

        본 연구에서는 재부착 이후의 재발달 난류경계층에서의 난류구조와 비평형 상태에서 평형상태로의 변화 과정을 실험적으로 고찰하고자 한다. An experimental study has been performed to investigate the process from nonequilibrium state to equilibrium state in redeveloping turbulent boundary layer beyond separation-reattachment using pitot tube and hot-wire anemometer. The model sued in the experiment has the form of a backward facing step which is assembled by a two-dimensional 4:1 half elipse and a plate. Measurements are carried out up to a distance of about 50 step height downstream of the step, where the reattachment observed at about x/h=6.5. The profiles of the shape factor H the Clauser parameter G and the coefficient of friction $C^{f}$ exhibited the characteristics similar to those of the equilibrium turbulent boundary layer from x/h=25, and the profiles of the trubulent quantities did from x/h=35. However, the wake region of the boundary layer does not seem to recover the equilibrium turbulent boundary layer even at x/h=50. By considering the distributions of the intermittency factor it has been noted that the turbulence structure changes gradually from a mixing layer to a turbulent boundary layer along downstream direction after reattachment. This becomes clearer as we analyse the one-dimensional energy spectra and the dissipation energy spectra which are measured and caculated at various downstream positions after the backward facing step.p.

        Current Status and Trends in Inflammatory Bowel Disease Surgery in Korea: Analysis of Data in a Nationwide Registry

        Purpose: Inflammatory bowel disease (IBD) in Korea has been increasing in recent years, but accurate statistics about operations for IBD are lacking. The purpose of this study was to investigate the trends and current status of IBD surgeries in Korea. Methods: Using a national database from the Korea Health Insurance Review and Assessment Service, we analyzed data from patients who underwent surgery for Crohn disease and ulcerative colitis from January 2009 to October 2016. Results: The mean number of patients who underwent surgery for Crohn disease was 791.8 per year. Colorectal surgery, small bowel surgery, and anal surgery were performed fairly often (31.2%, 29.4%, 39.4%, respectively), and laparoscopic surgery continued to increase, recently exceeding 30%. About 50% of Crohn patients used biologics before and after surgery, and those patients also underwent a relatively high rate of anal surgeries (44.2%). The mean number of patients who underwent surgery for ulcerative colitis was 247.6 per year. Colorectal surgery accounted for more than half of all operations, and laparoscopic surgery has been increasing rapidly, having been performed in about 60% of patients in recent years. The incidence of colorectal cancer in patients with ulcerative colitis was very high and increased rapidly during the study period, reaching about 80%. Conclusion: The number of patients undergoing laparoscopic surgery for IBD in Korea has increased significantly. Biologics are actively used by patients with Crohn disease, with a high proportion of anal surgeries required. Many of the surgical indications for ulcerative colitis have shifted into colorectal cancer.

      • 박리-재부착 이후의 재발달 난류경계층 II -난류 모델들에 관한 고찰-

        본 연구의 목적은 첫째, 재발달 경계층에서의 난류에서지 및 난류전단응력에 대한 전달방정식들의 각 항의 측정자료들을 보다 정확히 제시하고 항들간의 균형을 비교 평가함으로써 비평형 유동으로부터 평형유동으로 회복되는 과정을 검토하고, 둘째, 대표적인 난류 모델들로써 표존 k-.epsilon.모델 및 레이놀즈 응력 모델을 사용한 수치계산을 수행함으로써 이와같은 모델들이 비평형 유동을 서술함에 있어 발생될 수 있는 문제점들을 고찰하는데 있다. A consideration on the trubulence models for describing the redeveloping turbulent boundary layer beyond separation-reattachment in the flow over a backward-facing step is given through experimental and numerical studies. By considering the blance among the measured values of respective terms in the transport equations for the turbulent kinetic energy and the turbulent shear stress, the recovering process of the redeveloping boundary layer from non-equilibrium to equilibrium has been investigated, which takes place slowly over a substantial distance in the downstream direction. In the numerical study, the standard K-.epsilon. model and the Reynolds stress model have been applied to two kinds of flow regions, one for the entire downstream region after the backward-facing step and another for the downstream region after reattachment. Then the results are compared to a meaningful extent, with the experimental values of the turbulent kinetic energy k, the turbulent energy production term P, the dissipation term K-.epsilon. model, a necessity for a new modelling has been brought forward, which can be also applied to the case of the nonequlibrium turbulent flow.

      • 고정도 직선 이송을 위한 캠-슬라이더와 조정-캠을 이용한 테이프 피더 개발

        In recent years, a study of tape feeder which can feed the chips to chip-mounter very quickly is important issue. For that reason, the precision tape feeder of linear feeding type is developed using linear cam-slider mechanism. In this paper, for improvement of existing tape feeder, various analysis of tape feeder operation such as motion, position, velocity and acceleration, are executed using COSMOS Motion of SolidWorks COSMOS Application. Besides, experiment for obtaining real motion image is executed using high-speed camera. Control-cam and cam-slider which have improved in stability and precision is developed by comparing analytic and experiment result. For superior tape feeder, those are applied to the new tape feeder.
