산과 수술에서 흡수성 합성 봉합사 (Safil(R)/Safil(R) Quick/MonosynR)의 유용성

목적 : 산과적 수술 영역에서 Safil(R)/Safil(R) Quick/Monosyn(R) 합성봉사들의 유용성을 평가하고자 하였다. 방법 : 2004년 4월부터 9월까지 삼성서울병원 산부인과에서 질식 자연분만을 한 100명의 산모와 제왕절개 수술을 시행받은 198명의 임산부를 대상으로 하였다. 질식 분만 시 회음절개의 봉합 치료군 50명에게는 Safil(R) Quick, 대조군 50명에게는 chromic catgut을 사용하였고, 제왕절개 시 대상 환자를 두 군으로 나누어 첫 번째 군에는 100명을 포함시키고 두 번째 군에는 98명을 포함시켰다. 첫 번째 군인 자궁 절개 봉합 치료군 50명에게는 Monosyn(R), 대조군 50명에게는 chromic catgut을 사용하였고, 두 번째 군인 근막 봉합 치료군 49명에게는 Safi(R)을 사용하였고, 대조군 50명에게는 Vicryl(R)을 사용하였다. 이들을 대상으로 수술 전후, 퇴원 시, 퇴원후 상처 봉합 상태 및 합병증들을 조사하였다. 결과 : Chromic catgut을 사용한 군과 비교하였을 때, Safil(R) Quick 군은 질식 분만일로부터 퇴원일까지의 기간이나 분만 후 봉합 부위 치유 상태 및 감염, 혈종, 열개 등의 합병증의 빈도가 차이가 없었고, Monosyn(R) 군도 제왕절개 수술 시간이나 수술 3일 후 헤모글로빈 수치의 감소 및 합병증의 빈도가 차이가 없었다. Safil(R) 군도 Vicryl(R) 군과 비교하였을 때 제왕절개 수술 시간과 퇴원시의 통증 강도 및 합병증의 빈도가 유의한 차이를 나타내지 않았다. 결론 : Safil(R) Quick은 회음절개의 봉합에 있어 chromic catgut의 대체 봉합사로 사용될 수 있고, Monosyn(R)과 Safil(R)은 각각 제왕절개 시 자궁 절개 부위와 근막의 봉합에 chromic catgut과 Vicryl(R)의 대체 봉합사로 사용될 수 있다. Objective : To evaluate the clinical applicability and safety of absorbable synthetic suture materials (Safil(R)/Safil(R) Quick/Monosyn(R)) in obstetrical surgeries. Methods : This clinical trial includes 100 patients who delivered vaginally and 198 patients who were undergone cesarean section from April 2004 to September 2004. In cases of vaginal delivery, patients were divided with the same number into the study group in which Safil(R) Quick was used and the control group in which chromic catgut was used for episiotomy and perineal laceration repair. In case of cesarean delivery, patients were divided into two groups. The first group which included 100 patients was subdivided with the same number into the study group in which Monosyn(R) Was used and the control group in which chromic catgut was used for uterine repair. The second group which included 98 patients was subdivided with the same number into the study group in which Safil(R) was used and the control group in which Vicryl was used for rectus fascia repair. Wound healing status and complications are assessed during postoperative stage, at hospital discharge, and at postpartum out-patients follow-up. Results : Compared with chromic catgut groups Safil(R) Quick group showed no difference in hospital stay from vaginal delivery, wound healing status and surgical complications and Monosyn(R) group showed no difference in operating time, decrease in hemoglobin at postoperative day 3 and surgical complications. Safil(R) group showed no difference in operating time, postoperative pain and surgical complications compared with Vicryl group. Conclusion : Safil(R)/Safil(R) Quick/Monosyn(R) were equivalent with regard to most aspects of their clinical suitability and may be useful alternative suture materials in obstetrical surgeries.

System scaffolding and shoring are temporary structures in which vertical members, horizontal members, bracing members and trusses are assembled and installed. In order to ensure quality and safety, the quality test shall be carried out in accordance with the Guidelines for Quality Management of Construction Works (MOLIT Notice No. 2020-750). The quality test method (national standard) for Components for tied post system scaffolding and shoring is based on the Korean standards (KS F 8021) and the Safety certification standards (MOEL Notice No. 2021-22). However, the two standards differ in some aspects such as performance standards and etc, so cause confusion when applying them on-site. In addition, the standard for truss are applied only to trusses for shoring and cannot be applied to trusses for scaffolding. Therefore, this study aims to unify the two national standards and establish realistic standards.

Pipe support is a representative structure that supports slab formwork, and it is a representative temporary equipment that has been systematically managed since the 1990s when the domestic temporary equipment performance test system was introduced. Nevertheless, it is also a reality that many of the products in circulation are used products that are reused and do not meet the performance of the initial manufacturing stage. However, if only new products are insisted, it could lead to delays in the process due to an increase in construction costs and difficulties in timely delivery. On the contrary, it is not acceptable for the safety of the construction site to use products of low quality without verification procedures or standards. Therefore, this study attempts to grasp the management system such as safety certification for temporary equipment and the actual condition of quality control to maintain performance, and propose improvement plans.

건설현장에서 사용 중인 가설기자재는 신제품보다 대부분 재사용품이 사용되고 있기 때문에 여타 건설자재나 부재처럼 신제품에 대해서 만 품질관리기준을 수립하기에는 현실적 적용단계에서 많은 문제가 발생할 수 있다. 특히 반복 재사용으로 인한 성능저하가 발생할 수 있는데, 이에 가설기자재별 적정한 성능기준의 마련을 통해 일정한 품질을 확보할 필요가 있다. 따라서 본 연구에서는 가설기자재별 성능기준을 마련하기 위한 목적으로 문헌 조사 및 성능시험을 실시하였으며, 그 일환으로 파이프서포트의 성능기준에 대한 연구를 통해 적정한 재사용품의 성능기준을 제시하고자 하였다.

The Temporary construction refers to the construction or dismantling of temporary structures installed for the construction of the main structure at the construction site, and the members constituting this temporary facility are called temporary equipment. Temporary equipment used in construction sites is mostly used products that are reused rather than new products, and it is necessary to consider performance degradation due to repeated use. This study is a preliminary study to develop the performance evaluation criteria applied to temporary equipment that is distributed and reused for secondhand. This study is to analyze the performance status of temporary equipment of used goods owned by rental companies by condition grade through performance tests.

In the construction sector, there were 99 fatalities from falls associated with steel tube and system scaffolds for three years from 2016 to 2018 and out of which 27.3 percent (27 workers) were found to have occurred during installing and dismantling procedure. To erect scaffolding which is installed for the work at height, the work platforms are generally installed first and the safety guardrails are installed and vice versa to dismantle. As a result, workers are exposed to the risk of falling because they erect and dismantle scaffolding without guardrails. To minimize the risk of falling, it is necessary to study a new construction method that can erect or dismantle scaffolding while guardrails are installed (called advanced guardrail system). This study analyzed advanced guardrail system in terms of safety regulation, workability and economic efficiency by investigating the domestic situation on the use of scaffolding, reviewing domestic and foreign standards for guardrails and conducting economic feasibility study. The results of this study is expected to greatly contribute to preventing falls in scaffolding installation and dismantlement procedure. As a follow-up study, empirical research is required including physical test of scaffolding with advanced guardrail system and effectiveness analysis after trial applications.
가설구조물 구조검토의 실시간 현장 반영을 통한 안전성 확보

Based on the survey analysis of safety accidents at construction sites that occur every year, most of them are caused by temporary structures. While analyzing in detail, it is confirmed that the main factors for such accidents are the quality problem of temporary equipment, unstable construction, and arbitrary change. As temporary structure is installed only for the construction of main structure, the reality is that no attention is paid to the construction of temporary structures.
