한방병원 갑상선 클리닉 내원 환자들의 임상적 특성 분석

Objective : Thyroid diseases are one of the most common endocrine disorders. However, the first chosen-treatments including anti-thyroidal and hormone-supplement therapy in western medicines, sometimes have failed to improve abnormal thyroid hormone secretions and clinical symptoms. Therefore, there has been a growing expectation for Oriental Medicine to take the role of alternative therapy. However, there has been little analysis of clinical characteristics. In this study, clinical characteristics of thyroidal disease patients who visited Kyung-Hee oriental hospital from January to July 2004 were analyzed. Methods : A total 86 patients diagnosed as hyperthyroidism, hypothyroidism or thyroid nodule were selected. Clinical history, including sex, age, symptoms, disease duration and methods of previous therapy and thyroidal test, such as thyroid function test (TFT), thyroid scan and other thyroid examinations. Results : In distribution of onset age and sex, the patients who have visited oriental hospital with thyroidal diseases showed similar characteristics with typical patients. In clinical symptom, the most frequent clinical sign was fatigue in both hyperthyroidism and hypothyroidism, which differs from the classical characteristics of those diseases. Most patients complained the unresolved symptoms, some of which are attributable to abnormal thyroid hormone secretions, in spite of western therapy with anti-thyroidal and hormone supplement agents. Conclusions : In oriental therapy, it will be necessary to consider the unique clinical features of thyroidal diseases different from classical patterns, and this study suggests that oriental therapy is a good alternative method for patients whose first treatment with western agents failed. Furthermore, more public information to these patients about oriental medicine would be helpful to increase cure rates, and give various choices to patients with thyroid diseases.
육미지황탕(六味地黃湯),택사탕(澤瀉湯), 실비음(實脾飮)이 메산지움세포 증식, Fibronectin 합성 및 MHC-class II 발현에 미치는 영향(影響)

Objective : To analyze the effects of Yukmijihwang-tang, Taeksa-tang, Silbi-um on mesangial cell proliferation, fibronectin synthesis and MHC-class II expression. Methods : Laboratory studies were performed with the method of surface enzyme immunoassays or flow cytometry after addition of peripheral blood mononuclear cells(PBMC) supernatants treated with medications using the cultured human mesangial cells. Results : 1. Silbi-um produces more suppressive effect than control group and hydrocortisone group on the mesangial cell proliferation. In Yukmijihwang-tang, Taeksa-tang and Silbi-um, mesangial cell proliferation significantly decreased than in hydrocortisone group 2. In the 'without fetal bovine serum' study, Yukmijihwang-tang take more suppressive effect than Control group on the fibronectin synthesis. In the 'with fetal bovine serum' study, Yukmijihwang-tang, Taeksa-tang, Silbi-um all have suppressive effect, but it hasn' t any statistical significance. 3. Yukmijihwang-tang, Taeksa-tang, Silbi-um all have a suppressive effect on the MHC-class II expression. Conclusions : Herb medicine generally show a suppressive effect on the suppression of the mesangial cell proliferation, fibronectin synthesis and MHC-class II expression.
생약복합제제의 약효 연구(제27보) : 패령탕 및 가미패령탕의 이뇨, 해열, 소염 및 진통작용에 대하여

이뇨제의 대표적 처방으로 이용되고 있는 오령산에 패독산을 합한 패령탕과 상기 처방에 백질려 및 백검을 가미한 가미패령탕에 대하여 임상적 효능을 규명하기 위한 기초적 연구의 일환으로 실험동물을 사용하여 이뇨, 해열, 소염 및 진통효과를 검토한 바 유의한 결과를 얻었기에 보고하는 바이다. 1. 정상 흰쥐에 대하여 검액 고농도 투여군에서 뇨량 및 뇨 중전해질의 증가효과가 인정되었다. 2. HgCl₂ 투여에 의하여 유발된 급성 신부전 흰쥐에 대하여 뇨량 및 뇨중전해질의 유의성 있는 증가효과가 인정되었다. 3. 급성신부전 흰쥐의 혈장에 있어서 urea nitrogen은 F-I, II 200㎎/100g용량 투여군에서 대조군에 비하여 각각 1.46배, 1.3배의 유의한 증가작용이 있었다. 4. 마우스에 대하여 검액 F-I,II 20㎎/10g용량 투여군에서 뇨량의 증가작용이 있었다. 5. 패령탕 및 가미패령탕의 유의성있는 해열효과가 인정되었다. 6. 패령탕 및 가미패령탕의 유의성있는 항염증효과가 인정되었다. 7. 패령탕 및 가미패령탕의 유의성있는 진통효과가 인정되었다.
시호의 사구체 메산지움 세포 증식억제 효능 및 작용기전 연구

Objective : Mesangial cell proliferation and excessive accumulation of extracellular matrix (ECM) proteins is the common pathologic feature of glomerulosclerosis, and platelet-derived growth factor (PDGF) BB-chain, transforming growth factor betal $(TGF-{\beta}1)$, cyclin dependent kinases (CDK) and CDK inhibitors mediated in these pathophysiological processes. Bupleurum falcatum which is one of the most widely used components in traditional oriental medicines, has multiple pharmacological effects, such as antipyretic, analgesic, immune modulating, anti-inflammatory, anti-allergic, anti-thrombotic, anti-atherosclerotic, and antitussive effects. Methods : In this study, we evaluated the influence of Bupleurum falcatum on mesangial cell proliferation, DNA synthesis and expression of PDGF-BB chain, $TGF-{\beta}1$, CDKI, CDK2, CDK4, p21 and p27 in fetal bovine serum (FBS)-activated human mesangial cell. Results : Bupleurum falcatum reduced the mesangial cell proliferation and DNA synthesis more than control and captopril. And in the ELISA analysis of $TGF-{\beta}1$, and RT-PCR of PDGF-BB chain, CDK1, CDK2, CDK4, p21, and p27, Bupleurum falcatum inhibited the expression of $TGF-{\beta}1$ protein and PDGF-BB, CDK1, CDK2 gene and promoted that of p21 gene in a dose-dependent manner in comparing with control and captopril. Conclusions: These results suggest that Bupleurum falcatum may inhibit the mesangial cell proliferation and DNA synthesis by regulation of PDGF-BB and $TGF-{\beta}1$ expressions, and by modulation of CDK1, CDK2 and p21 expression.
계지복령환(桂枝茯笭丸), 소복축어탕(少腹逐瘀湯), 도홍사물탕(桃紅四物湯)이 메산지움세포 증식과 Fibronectin 합성에 미치는 영향(影響)

In order to investigate the effects of Kaejibokryungwhan, Sobokchugeotang and Dohongsamultang on mesangial cell proliferation and fibronectin synthesis, laboratory study was performed. The results are summarized as follows; 1. Mesangial cell proliferation was significantly decreased in the Kaejibokryungwhan group and the Dohongsamultang group compared with the Control group. In the Sobokchugeotang group, the mesangia1 cell proliferation activity was lesser than Control group, but it was statistically non-significant. In Kaejibokryungwhan group, the Sobokchugeotang group and Dohongsamultang group, mesangial cell proliferation was significantly decreased compared with the Hydrocortisone group 2. In the group, which contained fetal bovine serum, fibronectin synthesis was decreased in the Kaejibokryungwhan group, the Sobokchugeotang group and Dohongsamultang group compared with Control group, but the difference was statistically non-significant. In Kaejibokryungwhan group, Sobokchugeotang group and Dohongsamultang group, fibronectin was less decreased compared with that of Hydrocortisone group. 3. In the group, which contained fetal bovine serum, fibronectin synthesis was significantly decreased in Kaejibokryungwhan group and Sobokchugeotang group than those of Control group. In Dohongsamultang group, the fibronectin synthesis was decreased than Control group, but the difference was statistically non-significant. In Sobokchugeotang group, the fibronectin synthesis was decreased than Hydrocortisone group, but it was statistically non-significant. According to the above results, the mesangial cell proliferation and fibronectin systhesis could be reduced by Kaejibokryungwhan group significantly.
제 2형 당뇨병 및 당뇨 합병증의 발생과 Peroxisome Proliferator-Activated Receptor-$\gamma2$ C161T 유전자 다형성과의 관계

Objective : Peroxisome proliferator-activated receptor (PPAR)-gamma, a transcription factor in adipocyte differentiation, has important effects on insulin sensitivity, atherosclerosis, endothelial cell function and inflammation. Through these effects, PPAR-gamma2 might be involved with type 2 diabetes and vascular disease, including diabetic complications. Recently, it has been reported that the C161T polymorphism in the exon 6 of PPAR-gamma is associated with type 2 diabetes interacting with uncoupling protein 2 (UCP2) gene, and is associated with acute myocardial infarction. We studied the association of this polymorphism with type 2 diabetes and its complications, such as retinopathy, ischemic stroke, nephropathy and neuropathy in Korean non-diabetic and type 2 diabetic populations. Methods : Three hundred and thirty eight type 2 diabetic patients (retinopathy: 64, ischemic stroke: 67, nephropathy: 39 and neuropathy: 76) and 152 healthy matched control subjects were evaluated. The PPAR-gamma C161T polymorphism was analyzed by PCR-RFLP. Results : PPAR-gamma C161T genotype and allele frequency did not show significant differences between type 2 diabetic patients and healthy controls (T allele: 17.0 vs. 14.5, OR= 1.21, P=0.3188). In the analysis for diabetic complications, T allele in diabetic nephropathy was significantly higher than controls (P=0.0358). T allele in the ischemic stroke patients was also higher than healthy controls, although it had no significance (P=0.1375). Conclusions : These results suggest that the C161T polymorphism of the PPAR-gamma gene might be associated with diabetic nephropathy in type 2 diabetes.
