        Ⅳ injection is the giving of substances directly into a vein and is widely used in hospitals. This passive method does not control the flow rate consistently. As a result, it causes an unnecessary nurse's overwork and decrement of Ⅳ injection's effect. To solve these problems, the flow control infusion pump has been developed. But because of relatively heavy weight and high price, it has not been used widely, yet. In this study, automatic Ⅳ injection system with small pressing mechanism and efficient control system is designed and fabricated. The system is smaller in size and uses lower power than the existing commercial product. Presented system is also equipped with networking capability, which enables monitoring the status of several automatic IV injection system.

        EGR is a primary emission control technique for the suppression of NOx, but combustion instability is occurred occasionally due to mal-distribution of fresh air and EGR gas at each cylinder. In this study, to improve EGR distribution characteristics, numerical simulations are performed to obtain the flow, the mixing characteristics and the EGR gas distribution at each cylinder for various configurations of individual EGR supply system in a 4-cylinder engine. The results show that the branch diameter has an important role in the mixing and EGR distribution characteristics. Also, the flow of EGR gas is highly depends on the air flow to each cylinder.

        사례보고 : 칡즙 복용 후 발생한 독성간염 2예

        건강에 대한 관심 증가와 함께 대체요법의 사용이 늘어나면서 각종 식물제제를 포함한 건강보조식품으로 인한 부작용들이 나타나고 있다. 저자들은 칡즙을 복용한 후 발생한 독성간염 두 예를 경험하여 보고한다. 환자들은 구역, 구토 등으로 내원하였으며, 입원 후 혈액생화학검사, 혈청검사 및 간조직검사를 시행받았고 RUCAM 점수체계를 기준으로 독성간염으로 진단되었다. 이들은 보존적치료 후 검사실 소견 및 순간탄성도검사 등에서 빠른 호전을 보였다. 칡즙의 복용으로도 독성간염이 생길 수 있다는 점을 염두에 두고, 건강보조식품에 대한 경각심을 가져야겠다. Herbal remedies and health foods are widely used, and their side effects have been reported. We describe two cases of symptomatic toxic hepatitis that developed in middle-aged women after ingesting arrowroot juice. The clinical manifestations were nausea, vomiting, and jaundice. The diagnosis of toxic hepatitis was made using the Roussel Uclaf Causality Assessment Method score on the basis of the patient`s history and laboratory data. After supportive care, the patients showed rapid improvements of clinical symptoms, laboratory findings, and liver stiffness. Clinicians should be aware that the consumption of arrowroot juice can cause toxic hepatitis. (Korean J Hepatol 2009;15:504-509)

        대장 게실증 임상양상에 영향을 미치는 인자에 대한 전향 연구

        목적: 평균수명의 증가에 따른 사회의 고령화와 식생활의 서구화 및 진단기법의 발전으로 우리나라 소화기관의 게실 빈도는 증가하는 추세이며 이에 따른 대장 게실의 임상양상 또한 후천적 영향에 의해 서구화될 것으로 생각된다. 이에 저자들은 대장 게실증을 진단받은 환자를 대상으로 대장 게실증에 영향을 미치는 후천적 인자를 알아보고자 하였다. 대상 및 방법: 2010년 5월부터 2012년 4월까지 인제대학교 서울백병원에서 시행한 대장내시경검사에서 게실이 관찰되었던 사람을 대상으로 하였으며, 설문을 통해 대상군의 나이, 체질량지수, 복부둘레, 운동량, 지방 섭취 정도, 흡연 및 음주의 정도를 조사하여 대장 게실의 임상양상과의 연관성을 알아 보았다. 결과: 연구 대상은 총 200명으로 나이는 54.9±11.9세(범위17-79세)였고 대장 게실증의 남녀비는 2.2:1이었다. 게실의 위치는 우측 대장이 83%였고 게실의 평균수는 4.07±3.9개였다. 게실 위치와 연관성을 보인 인자는 나이였는데 고령일 수록 좌측에 위치하였고(p=0.001), 게실의 수는 허리둘레와 유의한 연관성을 보였으며(partial correlation coefficient r`=0.143, p=0.047), 대장 게실염은 나이가 적을수록 증가하였다(p=0.002). 결론: 대장 게실의 위치는 고령일수록 좌측에 호발하였으며 게실 수는 복부비만과 연관성을 보였다. Background/Aims: The prevalence of colonic diverticulosis in Korea is increasing in conjunction with the adoption of western dietary pattern, extension of lifespan, and advances in diagnostic modalities. The clinical characteristics of colonic diverticulosis seem to be gradually becoming similar to those of Western societies. Therefore, factors associated with the clinical characteristics of colonic diverticulosis in Korea were investigated. Methods: The data of 200 patients diagnosed with colonic diverticulosis using colonoscopy between May 2010 and April 2012 at Inje University Seoul Paik Hospital (Seoul, Korea) were prospectively collected. Clinical parameters acquired through a questionnaire include age, body mass index, waist circumference, exercise, diet, smoking, drinking habits, etc. Correlation between these factors and the clinical features of diverticulosis were analyzed. Results: Mean age of the patients was 54.9±11.9 (range 17-79) years and male to female ratio was 2.2:1. Most diverticula were located on the right side of the colon (83%) and the mean number of diverticulum was 4.07±3.9. Factor associated with the location of diverticulum on the left side was age (p=0.001). There was a positive correlation between the waist circumference and the number of diverticulum (partial correlation coefficient r`=0.143, p=0.047). Diverticulitis occurred more frequently in younger patients than in older patients (p=0.002). Conclusions: Colonic diverticulosis in older patients is found more frequently on the left colon, and the number of diverticulosis is associated with central obesity.         수‧전동 휠체어 구동부 시스템 설계

        환자 및 노약자 이동에 중요한 역할을 하고 있는 수·전동휠체어의 핵심부인 구동부를 새롭게 설계했다. 구동 부는 고속모터와 다단기어 치형을 설계적용하고, 굽힘강도와 면압강도를 고려해 큰 추진력을 얻을 수 있도록 성능을 검증하였다. B-Type의 마찰판식 클러치 전자식 브레이크(다판 방식의 무여자 작동형)를 동 축상에 설치해, 큰 토크에 도 급격한 제동이 가능하도록 했다. 본 연구에 설계된 다단치차 감속기를 수·전동휠체어 구동장치에 사용하므로서, 소형화, 경량화를 이루고 구동 효율이 기존제품보다 30% 향상된 구동부를 구성할 수 있었으며, 보수 및 관리의 용이성을 함께 추구했다. Application of electric wheelchair, sort of wheelchair which is playing important role in transporting patients and old people, has been increasing. In this study, we designed the electric wheelchairs' driving system. Using the multi-step gear, the driving system can get great power, even though the small capacity of motors. First, we designed the multi-step gear, test its bending strength and contact strength, as well as verified its performance. We installed 'B-type electric brake(Multiple plate clutch, Anti-magnetization) in same axle of the driving system, so it is possible to stop under huge torque and small size. Using this driving system of the multi-step gear which we designed, it's possible to improve driving gear efficiency 30% up and create the high-competitive electric wheelchair. And, it is easy to repair and control.

        레지오넬라균 출현위해도에 대한 현행 온천수 수질기준의 적합성 분석

        Observed trends in climate change, globalization and an aging population have an effect on public health conditions in Korea, prompting a reevaluation of current environmental regulations. In this study, we evaluated the performance of the total coliform (TC) standard, which is the only microbiological standard in the current regulation regime for hot spring water, by estimating correlation with the presence/absence of Legionella, a non-fecal opportunistic pathogen with heat-tolerance. Microbiological data in 7 studies that surveyed Legionella in hot spring waters were subjected to meta-analyses with the odds ratio (OR) as the effect size. The presence/absence of Legionella was significantly correlated to TC levels [OR = 3.1(1.5-6.4, 95% CI), p = 0.002]. Due to there being no direct explanation as to the reason for the occurrence of TC, mesophilic fecal bacteria, being coupled with Legionella presence, the mechanism of the correlation between the two kinds of bacteria was further investigated. Legionella presence was more prevalent with a high heterotrophic plate count [HPC; 4.0(2.2-7.2); p < 0.001] and water temperature [4.3(1.4-13.6), p = 0.011] when the temperature range was <40oC. However, it was reverse-correlated with water temperature when the temperature was >50oC [0.2(0.1-0.4), p < 0.001]. Therefore, bacterial standing crops in hot spring waters appear to be determined by water temperature in general, and this forces TC and Legionella levels be correlated. In accordance with this relationship, HPC rather than TC reflect the levels of non-fecal contamination better. Therefore, employing HPC as the sole microbiological standard, or adding HPC into the current standard of hot spring water assessment, is suggested as a proactive measure to prevent health issues arising from contamination.

        경기도지역 어린이집과 유치원 급식의 소금함량 및 유아 식사준비자의 나트륨 섭취관련 식태도 조사

        Dietary habit of excess sodium consumption is formed mainly by excessive salt intake from the younger age and this may lead to hypertension, stroke, and stomach cancer. This study was performed to estimate the salt content in kindergarten meals and provide basic data on meal providers` dietary attitude to sodium intake for nutrition education. We collected data on161 food items from 16 institutions in Gyeonggi-do and salt content was calculated from salinity and weight of individual food items. The average salt content from lunch meals was 2.2 g, which was about daily adequate intake of sodium for children aged 3 to 5 years old. Greatest contributor to the salt content in a meal was soup and stew (47.8%). The most salty dishes were sauces and kimchi followed by stir-fried food, deep-fried food, braised food, and grilled food. The salt content was higher in soup and stew despite of low salinity, due to the large quantity per serving. The salt contents of soups and kimchi were 40.6% and 14.3%, respectively of the total salt content in dish groups. Staff members and caregivers at home who prepared food for the child showed preference for one-dish rice meal, dried fish and salted mackerel, and broth when eating soup, stew, and noodles. Caregivers showed higher sodium index score and had higher preference for processed food such as Ramen, canned food, and ham compared with staff members (p < 0.05). These results suggested that monitoring salt content of kindergarten meals and nutrition education for those prepare meals for children are needed to lower sodium intake in childhood.         차량용 HDD 의 수동형 진동저감 시스템 설계

        The shock performance of hard disk drives has been a serious issue for Car PC. Since the vibration and disturbances from a car gives an adverse effect on a HDD of Car PC, it is necessary to protect a HDD from them. In this study, passive vibration attenuation system for a vehicle HDD was developed. Acceleration from the ground through the tire and suspension system was measured to figure out the frequency translated to Car PC. Critical frequency to Car PC was determined by exciting it with a shaker and measuring a data transmitting speed from HDD. A newly designed vibration attenuation system was fabricated to protect HDD from the acceleration. It was shown that the developed system had an excellent vibration attenuation ability.

        극대세장비 마이크로 홀을 이용한 양성자 빔 집적 응용

        EDM is the manufacturing process that uses the thermal energy to machine electrically conductive part. Despite a lot of research has been conducted for decades, the best aspect ratio of the micro hole using micro-EDM has not been over 30, yet. In the present study, new fabrication scheme was introduced to increase the aspect ratio of micro hole dramatically. Micro holes with less than 10 aspect ratio were aligned and welded together to manufacture a micro hole with extreme aspect ratio. Alignment of the micro hole with over 380 aspect ratio was conducted by the home-made apparatus installed with microscope and laser beam. The micro hole with extreme aspect ratio was used to shape pencil beam from proton beam generated from MC-50 cyclotron. The pencil beam was utilized to machine test specimen whose result was compared with GEANT4 computer simulation. It was shown that the experimental and simulation result were closer as the aspect ratio of the micro hole was bigger.
