A case of successful pregnancy and full term normal vaginal delivery with unaffected fetus following renal transplation was presented with the brief review of pertinent literature.

Uterine leiomyoma is the most common female reproductive tract tumor during reproductive years. Estrogen has been implicated the most important factor in leimyoma growth. Recently studies have also implicated insulin-like growth factor(IGF)s in leiomyoma growth and as one of possible mediators of action of estrogen in the tissues. IGF binding proteins (IGFBPs) may modulate IGF actions and have IGF-independent actions at their target tissues. The purpose of this study are to compare the expression of IGFBPs mRNA in leiomyoma and normal mymetrium and to evaluate the effects of endogenous estrogen on the these IGFBPs mRNA expression. We determined the presence and relative abundances of mRNA encoding IGFBP-1 - IGFBP-6 in thetissues of leiomyoma, myometrium adjacent leiomyoma and normal myometrium from women during late proliferative phase of menstrual cycle, and tissues of leiomyoma from women treated with a gonadotropin releasing hormone agonist(GnRHa) by Northen blot. All IGFBPs mRNA expression but IGFBP-1 were detectable in leiomyoma, adjacent myometrium and normal uterus myometrium. The order of relative abundance of IGFBPs mRNA was IGFBP-4 > IGFBP-5 > IGFBP-2 > IGFBP-3 > IGFBP-6. There was an signiticant increase in the relative abundance of IGFBP-4 mRNA in leiomyoma compared to adjacent myometrium uterus myometrium but the relative abundances of other IGFBPs mRNA were not different. The relative abundance of IGFBP-3 and IGFBP-5 mRNA in leiomyoma preexposed in vivo to GnRHa were ficantly higher than that in leiomyoma unexposed to GnRHa. However, there was no significant differences n the relative abundance of IGFBP-2, IGFBP-4 and IGFBP-6 mRNA in between GnRHa- preexposed and -unexposed leiomyoma. These data support that IGFBP-3, IGFBP-4, and IGFBP-5 may be involved in leiomyoma growth and that IGFBP-3 and IGFBP-5 mRNA expression are dependent on the in vivo estrogen status and that GnRHa may act via estrogen - IGFBP system.
