The dilatation and effacement of the cervical canal during normal and abnormal labor is still a largely unexplained phenomenon. Therefore, in order to elucidate how cervical collagen types are involved in dilatation and effacement of the cervix in term pregnancy, distribution of collagen type Ⅰ, Ⅲ and Ⅳ in nonpregnant and pregnant cervical connective tissue was examined by the immunohistochemical staining method with the use of type-specific anticollagen antibodies. Uterine cervical tissues from 14 women of child-bearing age were examined. Cervical tissues from nonpregnant women were obtained at the time of hysterectomy for leiomyoma and uninvolved regions were used. Cervical tissues were also obtained from term pregnant uterus at the time of cesarean section and immediate postpartum hysterectomy, and from patients of early pregnacy(within 12 weeks) with leiomyoma. The results obtained are as follows: 1) Non-pregnant cervix composed of dense fibrous connective tissues was diffusely stained with antibodies to type Ⅰ and type Ⅲ collagens. Type Ⅳ collagen was located only in the basement membrane. 2) In the cervix at term pregnancy, a marked decrease in fibrous connective tissue with increased proportion of smooth muscle fibers was observed. In the cervix at term pregnancy, clear spaces due to edema were focally infiltrated with neutrophilic polymorphonuclear leukocytes . 3) In the cervix at term pregnancy, type Ⅰ and type Ⅲ collagens were distributed only around smooth muscle fiber bundles with some fibroblasts. Type Ⅳ collagen was distributed in a linear fashion delineating individual smooth muscle fibers and vascular basement membranes. 4) The infiltration of neutrophilic polymorphonuclear leukocytes seems to be closely related to the dissociation of fibrous connective tissues of the cervix at term pregnancy. 5) In the cervix at early pregnancy(within 12 weeks), little change was observed in smooth muscle fibers, although fibrous connective tissues became slightly sparse. The distribution of type Ⅰ, Ⅲ and Ⅳ collagens resembled that of the nonpregnat cervix.
