Isolated Celiac Artery Dissection and Splenic Infarction in a Patient with Protein S Deficiency: A Case Report

Protein S (PS) deficiency is a rare blood disorder associated with thrombosis. Only a small number of cases of isolated celiac artery dissection can be found in the literature. We now report a case of isolated celiac artery dissection and splenic infarction in a 44-year old male with PS deficiency. Abdominal computed tomography revealed celiac artery dissection and splenic infarction. The patient's PS activity was 64% (nl : 70∼140%) upon admission and 52% four weeks later. He was started on a regimen of NPO, antibiotics, and analgesics. He resumed oral intake of food and drugs on hospital day 3 and was discharged to his home on hospital day 8. We report a case of isolated celiac artery dissection with splenic infarction in a patient with PS deficiency that improved with conservative treatment. The patient's management did not include anti-platelet/thrombotic agents or endovascular/operational procedures.

The cloud computing industry has focused on establishing a cloud-based business environment for enterprises with efforts to convert using their own on-premise computing infrastructures to using cloud services. With these efforts, using cloud services has become natural, especially for the IT industry. The cloud computing industry is moving toward proliferation of the cloud computing environment into various evolving industries. Along with industrial trends, new technical trends such as edge computing and multi-cloud are emerging. These trends are expected to create new business models and develop related service ecosystems, providing new opportunities for service providers and new experiences for users. A mong those emerging technologies, multi-cloud technology is expected to realize unlimited global cloud computing resources by unifying cloud resources from multiple public cloud service providers. In this paper, we introduce the concept and related trends of multi-cloud technology. Subsequently, we analyze the main functionalities and several use cases of multi-cloud technology. Finally, we summarize the effects and usefulness of multi-cloud technology in the domestic cloud industry.

행복국가론: ‘평천하’ 개념을 중심으로

President Moon Jae-in vowed in his presidential inauguration speech that he would make South Korea a country worthy of being called a nation. The purpose of this study is to clarify what kind of country the nation hopes makes it ‘a country worthy of being called a nation’. While sages suggest various concepts, the common characteristic of these concepts is the 'happy nation'. Both "Great Learning" and "Abundant Meanings of the Great Learning" introduce the state of 'happy nation' as the concept of '平天下, pacify the country'. The happiness of the people is said to come from a peaceful country without war and disaster, a warm and loving embracing relationships between people, and a country where people are faithful to their nature and have peace of mind. The relationships among people and people are unified and at peace when people are faithful to their nature, and the power of the state is strengthened and true peace can be enjoyed when such a condition is established. Individual happiness and peace of the state, society, and individuals are not separate from one another, but are closely connected with each other. Such a country is called a peaceful country. 본 연구의 목적은 국민이 바라는 나라, ‘나라다운 나라’가 어떤 나라인지를 밝히고자 하는데 있다. ‘나라다운 나라’의 모습에 대해서 선각자들이 다양한 개념을 제시하고 있지만 공통적인 특징은 ‘행복국가’라고 할 수 있다. 『대학』 및 『대학연의』는 ‘행복국가’의 모습을 ‘평천하’ 개념으로 소개하고 있다. 국민의 행복은 전쟁과 재난이 없는 평화국가, 사람과 사람간의 관계가 따뜻하고 사랑스러운 포용국가, 그리고 국민 한 사람 한 사람이 본성에 충실하여 마음이 평안한 나라에서 이루어진다고 한다. 그리고 또 한 사람 한 사람이 본성에 충실할 때가 사람과 사람간의 관계가 화평하고 통합이 잘 되며, 또 그러한 상태가 될 때 국가의 힘도 강해지고 진정한 평화를 누릴 수 있다고 한다. 국가, 사회, 개인의 행복과 평화가 서로 분리되어 있는 것이 아니라 밀접히 연계되어 있는 바로 그런 세상을 평천하(平天下)라고 하는 것이다.

단편소설 수업의 세 가지 모형

소설은 재밌다.소설은 독자가 자기 삶의 인물과 상황을 성찰하며 상상하게 하기 때문이다. 그 성찰과 상상 속을 거닐다 보면 독자들에게 소통의 힘이 자란다. 성찰과 상상이란 스스로 묻고 답하는 시간, 곧 자신과의 '소통'이기 때문이다. 이 소설의 재미를 고등학교 학생들과 나누는 수업 기획이 내게 필요했다. 그래서 나는 교사가 소설의 기본내용과 배경지식을 정리하여 보여주고, 학생들이 직접 작품을 비판하고 상상하면서 자신의 이야기로 소화하는 과정 나는 이러한 기준에 의해 선정한 단편소설을 활용하여 세 가지 모형의 수업-<선택>, <질문>, <상상>을 기획하였다. 이것은 중학교 학생들과 함께했던 옛이야기 수업을 바탕으로 한 것이다. 옛이야기란 우리에게 익숙하고 평범하고 당연한 '이야기'이다. 단편 소설은 낯설고 복잡하며 정교한 '이야기'이다. 둘은 다르지만, 결국 '이야기'이다. 학생들이 이야기를 체험하는 것을 목표로 하는 수업이라면 나는 옛이야기 수업을 진행한 방식이 단편소설수업에도 적용될 수 있다고 생각했다. 그래서 옛이야기 수업 방식을 수업시간에 진행하고자 했다. 이를 위해 먼저 중요한 것은 소설을 고르는 것이다.그것은 학생들이 함께 읽을 만큼 적절한 분량이면서도 학생들의 삶에 가까이 있으며 학생들이 호기심을 느낄 만한 요소를 가지고 있고,교사의 안내가 있다면 더 풍요롭게 읽을 수 있는 작품을 고르는 일이다.이것이 내가 여러 소설들 가운데 단편소설을 고른 이유이다. 을 단편소설 수업에도 적용하였고 학생들과 교사 모두에게 재미있으며 의미 있는 결과를 얻었다.그 경험을 바탕으로 세 가지 수업 모형-<선택>, <질문>,<상상>에 따라 사례를 정리하였다. 나는 이 모형과 사례들이 교사들의 단편소설 수업 기획에 영감을 주기를 기대한다.

The shipbuilding industry has complex manufacturing processes and huge size facilities. It also has very long lead time as compared with the other manufacturing industries. Therefore, it is important to establish a production plan systematically considering the current state of the shipyard to operate the fixed facilities efficiently. Recently, Korean small and medium-sized shipbuilding companies have been undergoing a crisis because of the internal and external factors. In this study, a study on the improvement of planning system for the small and medium-sized shipbuilding companies was carried out using the information strategy planning (ISP) in order to establish an efficient planning and competitiveness. Using ISP method, problems of the planning system of 3 domestic small and medium-sized shipbuilding companies were derived. Based on the derived problems, priorities for system improvements were determined. In future research, planning system will be improved based on common requirements derived from each shipyard.

갑상선 결절의 초음파 유도 중재 시술

The conventional treatment of symptomatic benign thyroid nodules is surgical excision under general anesthesia. Ultrasound-guided ablation of thyroid nodules has emerged as an alternative to surgery for symptomatic benign thyroid nodules. The procedure can be performed on an outpatient basis and has a low complication rate. Cystic thyroid nodules can be effectively managed with percutaneous ethanol injections and solid thyroid nodules can be treated using thermal ablation, such as radiofrequency ablation (RFA), laser ablation (LA), high intensity focus ultrasound (HIFU), and microwave ablation. Ethanol injections use a chemical reaction and thermal ablation uses heat injury to cause the cells to break down and the nodule to shrink. In recent years, RFA is available for achieving local control of recurrences of papillary thyroid carcinoma in patients who are poor candidates for general anesthesia. RFA and LA are the most commonly used techniques. HIFU and microwave ablation are newer techniques and should be considered experimental due to insufficient evidence regarding their utility.
