Brain type of creatine kinase (CKB) regulates energy homeostasis by reversibly transferring phosphate groups between phosphocreatine and ATP at sites of high energy demand. Several types of cancer cells exhibit upregulated CKB expression, but the function of CKB in cancer cells remains unclear. In this study, we investigated the function of CKB in breast cancer by overexpressing CKB in MDA-MB-231 cells. The overexpression of CKB did not affect cell growth rate, cell cycle distribution, ATP level or key mediators of aerobic glycolysis and lactate dehydrogenase isoform levels. Meanwhile, CKB overexpression did increase resistance to doxorubicin. TGF-β-induced Smad phosphorylation and Smad-dependent transcriptional activity were significantly up-regulated by CKB expression without changes in inhibitory Smad protein levels. Moreover, treatment with TGF-β considerably enhanced cell viability during doxorubicin treatment and decreased doxorubicin-induced apoptosis in CKB-expressing MDA-MB-231 cells compared to control cells. These results suggest that CKB attenuates doxorubicin-induced apoptosis and potentiates resistance to doxorubicin by enhancing TGF-β signaling in MDA-MB- 231 cells.
Cloud-Barista 오픈소스 기반 성능 인지 멀티 클라우드 인프라 프로비저닝

Cloud infrastructures have been expanding all over the world, and types of services have been diversified. Cloud users are adopting multi-cloud, in which two or more clouds are utilized to overcome restrictions that might arise from using a single cloud. However, using multi-cloud increases the complexity of provisioning and managing cloud resources. In this paper, to alleviate problems, we researched a way to efficiently provision multi-cloud infrastructures. In particular, we analyzed the performance of each cloud service through performance benchmarking and proposed a performancebased optimal provisioning technique. The main contribution points of this paper are as follows: 1) it introduces a dynamic provisioning structure of multi-cloud infrastructures implemented through CB-Tumblebug of the Cloud-Barista open source project, 2) it presents a mechanism for evaluating heterogeneous cloud infrastructures' performances, and 3) it analyzes results of performance experiments for major cloud infrastructures. Lastly, we demonstrated the process of configuring and provisioning multi-cloud infrastructures based on their performances through CB-MapUI, a web client for CB-Tumblebug. The effectiveness of the appropriate multi-cloud infrastructure configuration can be examined through the experimental results and demonstrations. 클라우드 인프라는 전 세계적으로 확대되고 있으며 서비스의 형태도 다양화되고 있다. 사용자는 단일 클라우드 사용에서 발생할 수 있는 제약을 극복하고자 두 개 이상의 클라우드를 연계 활용하는 멀티 클라우드를 도입하고 있다. 그러나 멀티 클라우드와 함께 인프라의 배치와 관리 복잡성도 더욱 높아졌다. 본 논문은 이러한 문제 경감을 위해, 효율적인 멀티 클라우드 인프라 프로비저닝 방안을 모색한다. 특히 성능 평가를 통해 이종 클라우드의 자원 성능을 분석하고, 최적 프로비저닝할 수 있는 구조를 제안한다. 본 논문에서는 1) Cloud-Barista를 통해 구현된 멀티 클라우드 인프라의 동적 프로비저닝 구조를 소개하고, 2) 클라우드 성능 평가를 위한 메커니즘을 제시하며 주요 클라우드의 성능을 실험을 통해 분석한다. 3) 그리고 멀티 클라우드 인프라를 구성하고 동적으로 프로비저닝하는 과정을 실증한다. 제시된 실험 및 실증을 통해 최적 멀티 클라우드 인프라 구성의 실효성을 살펴볼 수 있다.
Surgical Results of Third or More Cardiac Valve Operation

Background: We evaluated operative outcomes after third or more cardiac operations for valvular heart disease,and analyzed whether pericardial coverage with artificial membrane is helpful for subsequent reoperation. Methods:From 2000 to 2012, 149 patients (male:female=70:79; mean age at operation, 57.0±11.3 years) underwent theirthird to fifth operations for valvular heart disease. Early results were compared between patients who underwenttheir third operation (n=114) and those who underwent fourth or fifth operation (n=35). Outcomes were also comparedbetween 71 patients who had their pericardium open during the previous operation and 27 patients who hadartificial membrane coverage. Results: Intraoperative adverse events occurred in 22 patients (14.8%). Right atrium(n=6) and innominate vein (n=5) were most frequently injured. In-hospital mortality rate was 9.4%. Total cardiopulmonarybypass time (225±77 minutes vs. 287±134 minutes, p=0.012) and the time required to prepare aorticcross clamp (209±57 minutes vs. 259±68 minutes, p<0.001) increased as reoperations were repeated. However,intraoperative event rate (13.2% vs. 20.0%), in-hospital mortality (9.6% vs. 8.6%) and postoperative complicationswere not statistically different according to the number of previous operations. Pericardial closure using artificialmembrane at previous operation was not beneficial in reducing intraoperative events (25.9% vs. 18.3%) and shorteningoperation time preparing aortic cross clamp (248±64 minutes vs. 225±59 minutes) as compared tono-closure. Conclusion: Clinical outcomes of the third or more operations for valvular heart disease were acceptablein terms of intraoperative adverse events and in-hospital mortality rates. There were no differences in the incidenceof intraoperative adverse events, early mortality and postoperative complications between third cardiac operationand fourth or more.

Background: We evaluated the mid-term outcomes and angiographic patency of redo coronary artery bypass grafting (CABG). Methods: Of 2,851 patients who underwent isolated CABG at Seoul National University Hospital from 2000 to 2017, 88 underwent redo CABG. Patients’mean age at redo CABG was 66.0±8.0 years. The mean interval between the first-time and redo CABG was 113.0±62.4 months. The mean follow-up duration was 86 months. Early and mid-term clinical out- comes were evaluated. Angiographic patency rates were evaluated early (1–2 days), 1 year, and 5 years after surgery. Comparative analyses between on-pump and off-pump CABG were also performed. Results: The culprits for reoperation were previous grafts (65.6%), native coronary vessels (17.8%), and both (16.7%). Off-pump CABG was performed in 75 cases (85.2%), and the mean number of distal anastomoses was 1.8±0.8. The saphenous vein (39.7%) was used most frequently, followed by the right internal thoracic artery (28.4%), right gastroepiploic artery (21.3%), left internal thoracic artery (7.8%), and radial artery (2.8%). Operative mor- tality was 1.1%. The overall survival, cumulative incidence of cardiac death, and cumulative incidence of major adverse cardiac events were 71.3%,12.0%, and 23.3% at 5 years after surgery, respectively. The overall angiographic patency rates were 95.7%, 90.1%, and 92.2% on early, 1-year, and 5-year angiograms, respectively. The angiographic patency rates of sa- phenous vein grafts were 93.1%, 85.6%, and 91.3% on early, 1-year, and 5-year angiograms, respectively. No significant differences in clinical outcomes or angiographic patency rates were observed between the on-pump (n=13) versus off-pump (n=75) groups. Multivari- able analysis revealed that age (hazard ratio [HR], 1.07; p=0.005) and chronic kidney disease (HR, 3.85; p=0.001) were risk factors for all-cause mortality. Conclusion: Redo CABG could mostly be performed using the off-pump technique and did not show increased operative mortality and morbidities.

지능 정보 사회의 도래와 함께 클라우드 컴퓨팅은 정보통신기술 전반을 융합하는 주축으로 자리 잡았다. 다양한 클라우드 컴퓨팅 인프라가 새롭게 구축되고 있으며, 새로운 클라우드 서비스들도 끊임없이 쏟아져 나오고 있다. 그러나, 클라우드 환경과 서비스가 다양해질수록, 운용에 대한 복잡성 또한 증가한다. 클라우드 서비스 브로커리지(CSB: Cloud service brokerage) 플랫폼은 사용자에게 다양한 클라우드 서비스에 대한 검색, 배치, 사용 및 관리 환경을 제공하여 통합된 멀티 클라우드의 접점을 제공하는 시스템으로, 복잡한 이종 환경의 서비스에 대하여 단순하고 일관된 사용 환경을 제공함으로써 서비스 생태계를 활성화하는 기술이다. 본고에서는 CSB 플랫폼의 개념과 기술 구조를 설명하고, 기술 동향과 더불어 국내외 표준화 동향에 대해 살펴본다.
Application of Vacuum-Assisted Closure Device in Management of Postpneumonectomy Empyema

A 57-year-old man was diagnosed with lung cancer and underwent pneumonectomy and mediastinal lymph node dissection. He was discharged without acute complications, but on a regular outpatient follow-up, he was readmitted with postpneumonectomy empyema. He was successfully treated with a vacuum-assisted closure device and for 1 year period of outpatient follow-up, there was no recurrence of empyema or lung cancer.
흄관 매설 공사의 작업 분석을 통한 자동차 장비 개발 방안에 관한 연구

The hume concrete pipe laying works are conducted in most of the construction sites. In case of the advanced countries, several systems for automating the hume pipe laying works have been developed to improve productivity, safety, and quality, and gain potential savings in costs. But there have been little efforts to accumulate technical know-how and develop a new method or equipment related to the hume pipe laying works in domestic construction industry. The purpose of this study is to analyze the existing home pipe laying work processes in detail and identify the main tasks to be considered for automation of the hume pipe laying works.
