Fabry disease is a progressive X-linked disorder of glycosphingolipid metabolism caused by a deficiency of the α-galactosidase lysosomal enzyme. The partial or complete deficiency of the lysosomal enzyme leads to an accumulation of neutral glycosphingolipids in the vascular endothelium and visceral tissues throughout the body. In the heart, glycosphingolipids deposition causes progressive left ventricular hypertrophy (LVH). We report a case of Fabry disease which was suspected based upon two-dimensional echocardiographic finding of LVH. A 44-year-old man was admitted to evaluation of aggravated exertional dyspnea of two weeks duration. He had been diagnosed with end-stage renal disease of unknown etiology at age 41 followed by renal transplantation that year. He had been treated with oral immunosuppressive agents. On hospital day two, transthoracic echocardiography revealed concentric LVH. Left ventricular systolic function was preserved but diastolic dysfunction was present. Fabry disease was confirmed by demonstration of a low plasma α-galactosidase A (α-Gal A) activity. Analysis of genomic DNA showed α-Gal A gene mutation. The patient was diagnosed with Fabry disease.

PURPOSES: Used in transportation planning and traffic engineering, almost traffic simulation tools have input variable values optimized byoverseas traffic flow attribution because they are almost developed in overseas country. Thus, model calibration appropriated for internal trafficflow attribution is needed to improve reliability of simulation method. METHODS : In this study, the traffic flow model calibration is based on expressways. For model calibration, it needs to define eachexpressway link according to attribution, thus it is classified by design speed, geometric conditions and number of lanes. And modifiedgreenshield model is used as traffic flow model. RESULTS : The result of the traffic model calibration indicates that internal congested density is lower than overseas. And the result ofanalysis according to the link attribution indicates that the more design speed and number of lanes increase, the lower the minimum speed, thehigher the congested density. CONCLUSIONS: In the traffic simulation tool developed in overseas, the traffic flow is different as design speed and number of lanes, butroad segment don't affect traffic flow. Therefore, these results need to apply reasonably to internal traffic simulation method.

A roentgenological and gross anatomical studies were made in dissected specimens and in linical materials. Total of 239 vertebral columns were analysed for the presence of osteophytes in the spine._ Clinical and roentgenological examination was made in 214 unselected patients with presen-6g orthopedic complaints, while anatomical and radiological study was also directed in 245 Cases cadaver dissection in our department. Classification was based on the four degrees of development, and evaluation was directed as to the distribution at each level, as well as their localization on each vertebral body. Following results could be obtained. 1. The osteophyte could be demonstrated from the age of twenty reaching a peak in men at age of 30-49 and in women at age of 50-59. Its appearance seemd to be accelerated in the heavy laborer by 10 years than the ordinary person. 2. The incidence was greater on the anterior aspect than on the posterior aspect of the vertebral body. 3. Regional distribution was predominantly high at tenth to twelfth thoracic, and third to fourth lumbar vertebrae, and was apparently related to the curvatures of the vertebral column and the line of gravity crossing them, as in heavy labor. 4. The osteophytes associated with aging were probably the result of physiological compress-ion forces acting on a vertebral column, in which disc degeneration and osteoporosis could claim a significant causative factor. 5. Its presence could be confirmed either by simple dissection of a cadaver or roentgenographic examination.
