Sympathetic Block for Treating Primary Erythromelalgia

Primary erythromelalgia is a rare condition that’s characterized by erythema, an increased skin temperature and burning pain in the extremities. The pain is often very severe, and treating erythromelalgia is frustrating and difficult. We report here on the case of a 12-year old girl with primary erythromelalgia in both lower extremities. The pain was refractory to medical treatment, but a bilateral sympathetic block with lidocaine and triamcinolone resulted in relief from the pain. Our experience with this disease demonstrates that sympathetic blocks are effective in improving the symptoms and they may be attempted on erythromelalgia patients who do not respond to other treatments, including medication and epidural blocks.

The abnormal accumulation of protein aggregates is a dominant pathological feature common in neurodegenerative diseases. Autophagy contributes to the processing of aggregated proteins resistant to proteasomal degradation. Autophagic degradation is multi-step process. and especially aggresome formation is a specific and active cellular process for appropriate autophagy-mediated protein homeostasis mechanism. Here. we showed that preconditioning of cells with a non-toxic low dose ofMG132 induced autophagy, using an in vitro experimental model that closely represents the characteristics of the autophagy pathway under proteasome inhibition. Clear and large aggresome-like protein accumulation was observed in the perinuclear region of differentiated SH-SY5Y cells with preconditioning stimulus. This results in up-regulation of autophagosome formation and turnover and degradation of intracellular ubiquitinated and p62-bound protein aggregates. Pretreatment with low dose of MG132 attenuated proteinopathy-related cytotoxicity. Together. our experimental model could provide a proper in vitro system for studying the autophagy-related pathophysiology of neurodegeneration. especially therapeutic targeting of intracellular aggresome-like aggregates formation. ⓒ 2014 Elsevier Ireland Ltd. Background and Objectives Ergonovine stress echocardiography (ErgECHO) has been proposed as a noninvasive tool for the diagnosis of coronary vasospasm. However, concern over the safety of ErgECHO remains. This study was undertaken to investigate the safety and prognostic value of ErgECHO in a large population. Methods We studied 3,094 consecutive patients from a single-center registry who underwent ErgECHO from November 2002 to June 2009. Medical records, echocardiographic data, and laboratory findings obtained from follow-up periods were analyzed. Results The overall positive rate of ErgECHO was 8.6%. No procedure-related mortality or myocardial infarction (MI) occurred. Nineteen patients (0.6%) had transient symptomatic complications during ErgECHO including one who was successfully resuscitated. Cumulative major adverse cardiac events (MACEs) occurred in 14.0% and 5.1% of the patients with positive and negative ErgECHO results, respectively (p<0.001) at a median follow-up of 10.5 years. Cox regression survival analyses revealed that male sex, age, presence of diabetes, total cholesterol level of >220 mg/dL, and positive ErgECHO result itself were independent factors associated with MACEs. Conclusions ErgECHO can be performed safely by experienced physicians and its positive result may be an independent risk factor for long-term adverse outcomes. It may also be an alternative tool to invasive ergonovine-provoked coronary angiography for the diagnosis of vasospastic angina.
Serial Ultrasonographic Evaluation of Postpartum Uterine Involution in Miniature Schnauzer Dogs

This study was undertaken to determine the normal appearance of the postpartum uterine involution. Postpartum changes in uterine shape, architecture, echogenicity and diameter were monitored with ultrasonography in 8 Miniature Schnauzer dogs. Vaginal discharge was observed visually during the 3 weeks of postpartum. A large amount of viscous dark green discharge changed progressively to a small amount of transparent discharge. In the uterine shape, the transverse images were crescent or polygonal at the beginning, but became circular after 16 days. At postpartum day 24, the longitudinal images of placental and interplacental sites were similar to each other The echogenicity of uterine structure was clearly distinguishable among the hyperechoic serous membrane, hypochoic myometrium, hyperechoic endometrium, and anechoic uterine contents during the first week postpartum, but afterward the interluminal boundaries and echogenicity became obscure gradually. In addition, the anechoic substance of the endometrial cavity was not observed after postpartum day 28. In the uterine diameter, the diameter of placental sites decreased markedly from 24 mm at the first day postpartum to 13 mm at day 7, and the diameter of interplacental sites decreased from 15 mm at postpartum day 1 to 10 mm at day 7. At postpartum day 65, the uterus (6.4 mm) changed little and had a uniform homogenous echo, and thus it was confirmed that uterine involution was completed at that time in Miniature Schnauzer dogs.
재활용품 활용 놀이에서 나타난 만 4세반 유아들의 놀이양상과 환경보호 실천

본 연구의 목적은 재활용품 활용 놀이에서 나타난 만 4세반 유아들의 놀이양상과 환경보호 실천이 어떠한가를 탐색하기 위함이다. 이러한 연구목적을 이루기 위해 인천시 N구에 위치한 Y어린이집의 만 4세반 유아 22명을 대상으로 2020년 10월 5일부터 11월 27일까지 총 8주간의 실행연구를 실시하였다. 교사면담자료, 유아면담자료, 녹화 전사자료, 연구자 관찰일지 등 수집된 자료들은 귀납적 분석에 활용되었으며 연구결과는 다음과 같다. 첫째, 재활용품 활용 놀이에서 나타난 유아들의 놀이양상은 ‘재활용품 활용 놀이 그 자체를 즐기기’, ‘재활용품 종류에 따른 놀이 유형의 차이 나타내기’, ‘선행경험과 상상하기를 통해 재활용품에 접근하기’로 나타났다. 둘째, 유아들의 환경보호 실천은 ‘재활용품의 가치를 세며 은행 놀이에 적극 참여하기’, ‘재활용 가능 물품을 활용한 놀잇감과 교구 만들기를 즐기고 적극 활용하기’, ‘내가 할 수 있는 환경보호 실천목록 공유하기’로 나타났다. 본 연구에서 나타난 연구결과는 유아교육기관에서의 놀이재료로서 재활용품의 활용 가능성과 이를 통한 유아 놀이 활성화 방안 및 유아 환경교육을 위한 실천적 정보를 제공할 수 있을 것으로 기대된다. The purpose of this study is to explore the play aspect and the environmental protection practices of the 4-year-old preschoolers shown in the recyclable material-used play. To achieve this research objective, the action research was conducted from October 5, 2020 to November 27, 2020 in total of 8 weeks. 22 preschoolers in the class of 4-year-old at the Y Child Care Center located in N-gu(Korean administrative unit district), Incheon city participated in this research.. The collected materials such as the teachers interview, the preschoolers interview, the recorded-transcription, and the researcher observation diary were used for the inductive analysis. The findings of the current research are as follows. First, the preschoolers’ play aspects in the recyclable material-used play were shown as ‘Enjoying the recyclable material-used play activity itself,’ ‘Showing differences in the play patterns depending on the recycled product types,’ and ‘Approaching the recycled products through the prior experiences and imagination.’ Second, preschoolers’ environmental protection practices include ‘Positively participating in the bank play while counting the value of recycling,’ ‘Enjoying making the playthings and the teaching tools out of recyclables and actively using them,’ ‘Sharing one’s available list of environmental protection practices.’ The findings of this study are expected to provide the applicability of recycled goods as a play material in the early childhood education institution. Also, the findings provide practical means to aid preschoolers’ play and early childhood environment education.
Sequence discrimination of the Zα domain of human ADAR1 during B–Z transition of DNA duplexes

<P><B>Abstract</B></P><P>The Zα domain of human ADAR1 (Zα<SUB>ADAR1</SUB>) preferentially binds Z-DNA rather than B-DNA with high binding affinity. Zα<SUB>ADAR1</SUB> binds to the Z-conformation of both non-CG-repeat DNA duplexes and a d(CGCGCG)<SUB>2</SUB> duplex similarly. We performed NMR experiments on complexes between the Zα<SUB>ADAR1</SUB> and non-CG-repeat DNA duplexes, d(CACGTG)<SUB>2</SUB> or d(CGTACG)<SUB>2</SUB>, with a variety of protein-DNA molar ratios. Comparison of these results with those from the analysis of d(CGCGCG)<SUB>2</SUB> in the previous study suggests that Zα<SUB>ADAR1</SUB> exhibits the sequence preference of d(CGCGCG)<SUB>2</SUB>≫d(CACGTG)<SUB>2</SUB>>d(CGTACG)<SUB>2</SUB> through multiple sequence discrimination steps during the B–Z transition.</P>
