A single-chip MPEG2 decoder dedicated to MP@HL is described, which consists of specific functional units, macroblock level pipeline buffers, and sequence controller. Owing to the sophisticated I/O interface facilities among functional units, macroblock level pipeline buffers are successfully incorporated with functional units. A new organization for frame memory and its interface is also devised. The designed decoder contains K transistors, and occupies 81.0㎟ with a 0.6㎛ triple-metal CMOS technology.

Student independent project activity is beneficial for enhancing many kinds of abilities, e.g., team working, collaboration,communication, presentation, creativity, leadership, and so on. More than ten project teams actively perform every year in the Centerfor Innovation and Creativity Development of The University of Tokushima. The project management workshop was planned in order tomake project activity successful. In this workshop, the team members studied the importance of holding a common objective amongthem and of making a clear plan for achieving the activity. They practiced brainstorming method and KJ method and experienced a PDCAcycle. The team activity will be fruitful on the basis of the project management method.

Many patients with mental disorders visit emergency departments (EDs). However, the majority of these patients do not receive psychiatric assessment. In the present study, we investigated the detailed proportion of patients with mental disorders visiting an urban ED in the largest northern city in Japan. A retrospective chart review study was performed at a University Hospital from January 2012 to December 2015. The reasons for psychiatric consultations made by ED staff, and the primary psychiatric diagnoses were investigated. Among all living patients, 20% of them received consultations. The most common reason for consultation was suicide attempt followed by agitation or insomnia. Of all diagnoses, organic mental disorder was the most frequent and the mean age was significantly higher than the other diagnostic groups. Our study indicated that the frequency of psychiatric consultation was high. This indicates the high demand for mental health services at the ED. A thorough psychiatric assessment can provide adequate psychiatric services to acute patients; thereby possibly preventing suicide attempters from later actually dying by suicide.

ObjectiveaaElectroconvulsive therapy (ECT) is a reasonable option for intractable depression or schizophrenia, but a mechanism of action has not been established. One credible hypothesis is related to neural plasticity. Three genes (Oct4, Sox2, c-Myc) involved in the induction of induced pluripotent stem (iPS) cells are Wnt-target genes, which constitute a key gene group involved in neural plasticity through the TCF family. Klf4 is the other gene among Yamanaka’s four transcription factors, and increases in its expression are induced by stimulation of the canonical Wnt pathway. MethodsaaWe compared the peripheral blood gene expression of the four iPS genes (Oct4, Sox2, c-Myc, and Klf4) before and after modified ECT (specifically ECT with general anesthesia) of patients with intractable depression (n=6) or schizophrenia (n=6). Using Thymatron ten times the total bilateral electrical stimulation was evoked. ResultsaaBoth assessments of the symptoms demonstrated significant improvement after mECT stimulation. Expression of all four genes was confirmed to increase after initial stimulation. The gene expression levels after treatment were significantly different from the initial gene expression in all twelve cases at the following treatment stages: at the 3rd mECT for Oct4; at the 6th and 10th mECT for Sox2; and at the 3rd, 6th and 10th mECT for c-Myc. ConclusionaaThese significant differences were not present after correction for multiple testing; however, our data have the potential to explain the molecular mechanisms of mECT from a unique perspective. Further studie should be conducted to clarify the pathophysiological involvement of iPS-inducing genes in ECT.
