足三里에 施術한 旱蓮草 藥鍼의 免疫增進과 抗癌作用에 관한 硏究

To study the effects of anti-cancer, anti-metastasis and immune response improvement of herbal-acupuncture with Eclipta prostrata diffuse herba infusion solution(ELP-HAS), we injected ELP-HAS into Chok-samni(St36) of C57BL/6 mice implanted intravenously with B16-F10 melanoma. We have reached the following conclusion through the effect on the number of CD25^+/CD4^+, CD8^+/CD3e^+, CD69^+/B220^+, NK1.1^+/CD3e^+ cells in mouse PBMCs, the effect on the pulmonary colony number, and the effect on MST(Median Survival Time) and ILS(Increase of Life Span) of C57BL/6 mice implanted intravenously with B16-F10 melanoma. The results were obtained as follows: 1. In the experiment groups treated with ELP(Eclipta prostrata) Herbal-Acupuncture, the spleen cell proliferation in Balb/c mouse was significantly increased compared with control group. 2. In the experiment groups treated with ELP(Eclipta prostrata) Herbal-Acupuncture, the percentage of CD25^+/CD4^+, CD8^+/CD3e^+, CD69^+/B220^+, NK1.1^+/CD3e^+ cells in C57Bl/6 mouse PBMCs was increased compared with control group. 3. In the experiment groups treated with ELP(Eclipta prostrata) Herbal-Acupuncture, the pulmonary colony number of C57BL/ mice implanted intravenously with B16-F10 melanoma was decreased significantly compared with control group. 4. In the experiment groups treated with ELP(Eclipta prostrata)Herbal-Acupuncture, MST(Median Survival Time) and ILS(Increase of Life Span) of C57BL/6 mice implanted intravenously with B16-F10 melanoma were increased significantly compared with control group.

Betulinic acid (BA), a pentacyclic triterpene isolated from Lycopus lucidus, has been reported to be a selective inducer of apoptosis in various human cancer and shown anti-inflammatory and immunomodulatory properties. We postulated that BA modulates the immunomodulatory properties at least two groups of protein mediators of inflammation, interlukin-1$\beta$ (IL-1$\beta$) and the tumor necrosis factor- $\alpha$ (TNF-$\alpha$) on the basis of the critical role of the monocytes and tissue macrophages in inflammatory and immune responses. TNF-$\alpha$ and IL-1$\beta$ were produced by BA in a dose dependent manner at concentration of 0.625 and 10 $\mu$g/mL. The production of NO associated with iNOS was inhibited when treated with LPS at the concentration of 2.5 to 20 $\mu$g/mL of BA whereas COX-2 expression was decreased at 2.5 to 20 $\mu$g/mL. These modulations of inflammatory mediators were examined in LPS-stimulated RAW 264.7 cells and peritoneal macrophages. The morphology of macrophage was also examined and enhanced surface CD 40 molecule was expressed when treated BA at 0.625∼5 $\mu$g/mL with or without LPS. Furthermore, BA (20 $\mu$g/mL) enhanced apoptosis by producing DNA ladder in the RAW 264.7 cells. Our results indicated that BA induced activation of macrophage and pro-inflammatory cytokines. This may provide a molecular basis for the ability of BA to mediate macrophage, suppress inflammation, and modulate the immune response.

Background: Scars can affect patients, leading to emotional complications. Various laser technologies including fractional laser, Q-Switched Nd:YAG laser, and pulsed dye laser have been tried to revise the scar. Up to now, there has been no reliable study to evaluate an effect of picosecond Nd:YAG laser for reducing scars. Objectives: This retrospective clinical study was performed to investigate the beneficial effect of picosecond Nd:YAG laser in treatment for hypertrophic scars. Methods: A retrospective review was conducted on patients treated with picosecond Nd:YAG laser for the purpose of scar improvement. Twenty-seven patients with hypertrophic scars were included in this study. The vancouver scar scale (VSS), global assessment score (GAS), and participant’s subjective satisfaction were used to determine the effect of scar improvement. All side effects were recorded to assess the safety of the modality. Results: Mean age of patients was 33.4±15.2 years old and male-to-female ratio was 2:3. Mean total treatment session was 3.4±1.3 and mean total duration of treatment was 13±7.3 weeks. Average VSS score was remarkably decreased after treatment. Average GAS and participant’s satisfaction indicated fair cosmetic improvement. No significant side effects were observed during follow-up. Conclusion: Picosecond Nd:YAG laser may be considered as an effective and safe optional modality for the treatment of scar.

This research explores the relationship between body image and clothing behavior in order to enhance selfefficacy among multicultural children based on a survey of 167 children. The statistical analysis indicated that children value(in decreasing order)family-esteem, friend-esteem, self-esteem, and school-esteem. School-esteem is the lowest, with boys displaying slightly higher levels of esteem than girls. A boys` self-esteem appears enhanced by higher satisfaction with overall appearance and weight; however, girls` self-esteem helped them address appearance and physical characteristics with positive attitudes. In addition, the clothing behavior of children significantly influenced self-esteem to differing degrees based upon gender. A boys` self-esteem can be increased by having them show interest in clothes rather than letting them wear similar clothes to their friends because their self-esteem can be heightened by higher interests and lower similarity. A girls` self-esteem can be augmented by the clean management of clothes and comfortable wearing due to their emphasis on manageability and comfort. This survey was limited to the Gyeonggi and Inchon area and leaves room for future studies.

Background: Congenital melanocytic nevus may be defined as a lesion present at birth and containing nevus cells. Maturation in the melanocytic nevus means a conversion of pigment-producing large, clear type A cells to nerve-like type C cells through lymphocytoid type B cells. Objectives: This study was performed to investigate the clinical and pathologic characteristics of congenital melanocytic nevus and associated factors with nevus cell types in Korean population. Methods: This study was performed with patients histologically confirmed as congenital melanocytic nevus between 2007 and 2015. The proportion and region of interest (ROI) of type A, B and C cells were calculated using ‘image J’ program. Statistical differences were assessed using the Kruskal-Wallis test and Mann-Whitney test. Results: Mean size of nevus was 2.7±2.0 cm, and the most common location of the lesion was head and neck (n=20, 32.3%). Depth of the nevus cell and the nevus cell infiltration to the eccrine gland positively correlate with the proportion of the type C cell to the type A+B cell (p<0.05), and angioadnexocentric nevus was reported at 4 cases (4.84%). Conclusion: As the depth of the nevus cell and the nevus cell infiltration to the eccrine gland increase, the proportion of the type C cell to the type A+B cell markedly increases. Clinicopathologic features of angioadnexocentric nevus identified at this study are identical to previous studies.

Background: A low-fluence 1064-nm Q-switched neodymium-doped yttrium aluminum garnet (QSND) laser has been used for treatment of pigmented disorders. However, the photobiological mechanism of the low-fluence 1064-nm QSND laser is not yet fully understood. Objectives: To evaluate inhibitory effects of the low-fluence 1064-nm QSND laser on the melanogenesis and assess whether increasing the number of passes of low-fluence the 1064-nm QSND laser has positive correlation with improvements of the pigmentation. Methods: The skins of six Hair Moprpholoy-2 melanin-possessing hairless mice were treated with ultraviolet B radiation followed by 0, 2, 4, 6 passes the low-fluence 1064-nm QSND laser. Pigmentation changes of skins were evaluated with spectrophotometry and pigmentation-related protein expressions. Additionally, biopsy specimens were obtained to figure out the difference of melanin particles. Results: The low-fluence 1064-nm QSND laser improved the lightness of the skin in comparison to the control skin. Biopsy specimens showed decreased amounts of melanin particles. In the western blot of the specimen, expressions of MITF and tyrosinase were reduced. Conclusion: The low-fluence 1064-nm QSND laser has effects on the melanogenesis which change expressions of the pigmentation-related proteins. Moreover, the more passes of the laser toning can bring the more progress on the skin pigmentation.

Recently, it is revealed that systemic diseases are associated with rosacea. Objectives: To investigate the relationship between gut microbiome, associated diseases and rosacea. Methods: A cross-sectional analysis was performed among healthy female subjects. Study data were obtained from 282 subjects who participated in a health check-up program. Rosacea was assessed by a questionnaire. Other medical illnesses were evaluated by thorough medical examination and history taking. Stool microbiota was analysed by QIIME (Quantitative Insights Into Microbial Ecology) software. Results: The study included 12 patients with rosacea and 270 subjects without rosacea (control group). After adjusting for age, body mass index (BMI) was significantly higher in rosacea patients as compared with the control group (odds ratio 1.20, 95% confidence interval 1.02-1.42). Seven metagenomic genera were correlated with rosacea: Peptococcaceae family unknown genus, Methanobrevibacter genus, Acidaminococcus genus, Megasphaera genus, Lactococcus genus, Eggerthella genus, Lactobacillales order unknown family unknown genus (p<0.05). Conclusion: This study shows that rosacea is associated with BMI and specific gut microbiota.

Background: The use of antiviral agent has changed the prognosis of patients with hepatitis B virus (HBV)-related end stage liver disease. In these patients, therefore, a more efficient prognostic model for determining early mortality is necessary to properly select those who require liver transplantation. In this study, we aimed to develop a new prognostic model. Methods: We retrospectively analyzed a total of consecutive 194 patients with decompensated HBV-related liver cirrhosis (≥CTP score 7, ascites, or jaundice) who had initially started antiviral treatment in Korea university Anam hospital. Univariate and multivariate cox-regression modeling was used to develop a model for predicting 6-month mortality Results: The study population was predominantly male (128/194) and median age was 51 years. Antiviral agents were administered for a median of 41 months (lamivudine in 157 patients, entecavir in 37). At baseline 147 and 12 patients had ascites and encephalopathy, respectively, with a mean CTP score of 9. Twenty-one (10.8%) patients died within the first 6 months of treatment. Univariate analysis revealed that baseline variables such as age, the presence of ascites or encephalopathy, serum bilirubin, prothrombin time, albumin, Na, BUN, alkaline phosphatase, and HBV DNA levels were associated with the deaths within 6-months (all P<0.05). Among them serum bilirubin, prothrombin time, HBV DNA levels and age were found to be independent risk factors in multivariate analysis. Using these four risk factors, we developed new scoring system to predict 6-months mortality. This new prediction model showed AUROC of 0.941, higher than those of CTP score and MELD score which were 0.893 and 0.862, respectively. Conclusions: This newly developed prediction model for early mortality will be useful in selecting the candidates of urgent liver transplantation in patients with decompensated HBV-related liver cirrhosis.
