There has been a sizable growth in the use of rich media over the World Wide Web(WWW) as more individuals and institutions realize rich-media's potential in a broad range of applications, including electronic commerce, education and training, news, etc. Internet and WWW technologies provide the information of the web. Libraries are systems that carry out interactions among information and knowledge seekers. Library become more and more important for the provision of data. Total information system for books management exist in many forms. We present total information system for books management which is applied to several different sources, e.g., administrator security login, statistics about the present state of lending, printing management, and convenient GUI(Graphic User Interface) environment.

With the boom of business-to-customer applications and the need to run web applications over a variety of user devices, the intenet community is rapidly realizing the power of web as a platfrom for electronic commerce. Efficient software reusability and server overhead is an important issue. Most of shopping malls for electronic commerce is implemented with Asp or php which is web-programming. But Asp or php is dropping reusability of program and have dependent to platform. We have designed a system shopping mall and a algorithm for efficient software reusability and reducting sever overhead. This paper proposes an implementation of shopping mall based on the JSP for electronic commerce. We describe some of these results in this paper.

Aptamers, capture molecules(oligonucleotides) for endothelial progenitor cells(EPCs), are able to recognize circulating EPCs and to bind them. To evaluate effects of the aptamer-coated stent, six pigs were utilized and three different stents-cobalt-chromium bare-metal stent(CC), paclitaxel-eluting Taxus stent(Boston Scientific, Natick, Massachusetts)(TA), and aminoparylene and aptamer-coated stent (AA)-were implanted in the three epicardial coronary arteries in a random fashion at 8 to 10 atm. Stent length and diameter was 18 mm and 3 mm. After one month, the animals underwent follow-up coronary angiography with acetylcholine(10 mL, 3 ug/mL) stimulation test and were euthanized for histologic analysis. There was a decrease in MLD in all stents: CC(12.3 ± 5.5%), TA(10.4 ± 3.7%), and AA(11.2 ± 6.3%). There was, however, no statistical difference between the groups. Neointimal thickness was lower in TA. During the acetylcholine stimulation test, severe spasm (total occlusion) was noted in 1 pig at the proximal and distal edges of the Taxus stent. Re-endothelialization score was 3.40 ± 0.52, 1.33 ± 0.52, and 3.60 ± 0.55 in CC, TA, and AA, respectively. Thrombus appearance rate was 0/6, 4/6, and 1/6 in CC, TA, and AA, respectively. The AA showed a tendency toward lower incidence of stent thrombosis and endothelial dysfunction than TA. However, the inhibition of neointimal hyperplasia was not different between CC and AA.

<P>This study compared clinical outcomes of drug-eluting stents (DES) versus bare-metal stents (BMS) in large coronary arteries in patients with acute myocardial infarction (MI). A total of 985 patients who underwent single-vessel percutaneous coronary intervention (PCI) in large coronary arteries (≥ 3.5 mm) in lesions < 25 mm were divided into DES group (n = 841) and BMS group (n = 144). Clinical outcomes during 12 months were compared. In-hospital outcome was similar between the groups. At six months, death/MI rate was not different. However, DES group had significantly lower rates of target-lesion revascularization (TLR) (1.7% vs 5.6%, <I>P</I> = 0.021), target-vessel revascularization (TVR) (2.2% vs 5.6%, <I>P</I> = 0.032), and total major adverse cardiac events (MACE) (3.4% vs 11.9%, <I>P</I> = 0.025). At 12 months, the rates of TLR and TVR remained lower in the DES group (2.5% vs 5.9%, <I>P</I> = 0.032 and 5.9% vs 3.1%, <I>P</I> = 0.041), but the rates of death/MI and total MACE were not statistically different. The use of DES in large vessels in the setting of acute MI is associated with lower need for repeat revascularization compared to BMS without compromising the overall safety over the course of one-year follow-up.</P>

<P><B>Background and Objectives</B></P><P>We sought to investigate the relationship between levels of high-sensitivity C-reactive protein (hs-CRP) and N-terminal pro-B-type natriuretic peptide (NT-proBNP) and the infarct size and left ventricular (LV) volume after acute myocardial infarction (MI).</P><P><B>Subjects and Methods</B></P><P>Eighty-six patients with acute ST-elevation MI underwent delayed enhancement multidetector computed tomography immediately after they underwent percutaneous coronary intervention to determine the infarct size. LV function and remodeling were assessed by echocardiography. Hs-CRP and NT-proBNP were measured at admission, 24 hours and two months later.</P><P><B>Results</B></P><P>Both hs-CRP and NT-proBNP at 24 hours showed a positive correlation with infarct size and a negative correlation with LV ejection fraction at the baseline and two months later. NT-proBNP at two months correlated with infarct size, LV ejection fraction, and LV end diastolic and systolic volume indices at two months. In patients with high NT-proBNP levels at 24 hours and two months, infarct size was larger and LV ejection fraction was lower. NT-proBNP was higher in patients who developed LV remodeling at two months: 929 pg/mL vs. 134 pg/mL, p=0.002. In contrast, hs-CRP at two months showed no relationship to infarct size, LV function, or LV volumes at two months.</P><P><B>Conclusion</B></P><P>Elevated hs-CRP level 24 hours after the onset of acute MI is associated with infarct size and LV dysfunction, whereas elevated levels of NT-proBNP 24 hours and two months after the onset of acute MI are both correlated with infarct size, LV dysfunction, and LV remodeling.</P>

Background and Objectives: There is a paucity of data regarding the benefit of clopidogrel monotherapy after dual antiplatelet therapy (DAPT) in patients treated with drug-eluting stents (DES). This study compared outcome between clopidogrel versus aspirin as monotherapy after DES for acute myocardial infarction (MI). Methods: From Korea Acute Myocardial Infarction Registry-National Institute of Health database, 1,819 patients treated with DES who were switched to monotherapy with clopidogrel (n=534) or aspirin (n=1,285) after uneventful 12-month DAPT were analyzed. The primary endpoint was net adverse clinical events (NACE), defined as a composite of death from any cause, MI, repeat percutaneous coronary intervention (PCI), stent thrombosis, ischemic stroke, or major bleeding during the period from 12 to 24 months. Results: After adjustment using inverse probability of treatment weighting, patients who received clopidogrel, compared with those treated with aspirin, had a similar incidence of NACE (0.7% and 0.7%; hazard ratio, 1.06; 95% confidence interval, 0.31–3.60; p=0.923). The 2 groups had similar rates of death from any cause (0.1% in each group, p=0.789), MI (0.3% and 0.1%, respectively; p=0.226), repeat PCI (0.1% and 0.3%, respectively; p=0.548), stent thrombosis (0.1% and 0%, respectively; p=0.121), major bleeding (0.2% in each group, p=0.974), and major adverse cardiovascular and cerebrovascular events (0.5% in each group, p=0.924). Conclusions: Monotherapy with clopidogrel, compared to aspirin, after DAPT showed similar clinical outcomes in patients with acute MI treated with DES.

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Titanium dioxide (TiO<SUB>2</SUB>) films have superior biocompatibility and may be effective as drug-binding matrices for drug-eluting stents (DESs). We sought to evaluate efficacy of a polymer-free DES coated with everolimus using nitrogen-doped TiO<SUB>2</SUB> film deposition in a porcine coronary restenosis model.</P> <P><B>Methods</B></P> <P>Forty coronary arteries in 20 pigs were randomly allocated to group 1 (bare-metal stents (BMSs), 3.0×18mm, n=10), group 2 (BMSs with nitrogen-doped TiO<SUB>2</SUB> film deposition, 3.0×18mm, n=10), group 3 [commercial everolimus-eluting stent, 3.0×18mm, n=10], and group 4 (polymer-free everolimus-eluting stent using nitrogen-doped TiO<SUB>2</SUB> film deposition, 3.0×18mm, n=10). Stents were randomly implanted in the left anterior descending coronary artery and left circumflex artery with stent:artery ratio of 1.3. Four weeks later, pigs underwent follow-up coronary angiography and were sacrificed for histopathologic analysis.</P> <P><B>Results</B></P> <P>Percent area stenosis was greater in group 1 compared to groups 3 and 4 (46.4±13.8% vs. 30.2±11.7% vs. 29.2±8.9%, respectively, p=0.005). Fibrin score was lower in groups 1 and 2, compared to groups 3 and 4: 0.87±0.67 vs. 0.76±0.61 vs. 2.27±0.24 vs. 1.75±0.31, respectively, p<0.001). Injury score and inflammation score were not different. Comparison between DES showed a higher fibrin score in group 3 than group 4 (2.27±0.24 vs. 1.75±0.31, p=0.023).</P> <P><B>Conclusions</B></P> <P>In a porcine model of coronary restenosis, a novel polymer-free DES using nitrogen-doped TiO<SUB>2</SUB> film deposition shows higher biocompatibility and compares favorably with a commercial DES.</P> <P><B>Highlights</B></P> <P> <UL> <LI> TiO<SUB>2</SUB> films may be effective as drug-binding matrices for drug-eluting stents (DESs). </LI> <LI> Polymer-free DES using nitrogen-doped TiO<SUB>2</SUB> film shows higher biocompatibility. </LI> <LI> Further research is needed for longer-term effect of TiO<SUB>2</SUB> polymer-free DESs. </LI> </UL> </P>
Cardioprotective effect of substance P in a porcine model of acute myocardial infarction

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Substance P (SP) may attenuate ischemia-reperfusion injury by reducing inflammation. We assessed cardioprotective effect of SP in a porcine model of acute myocardial infarction (AMI).</P> <P><B>Methods</B></P> <P>AMI was induced by occlusion of the left anterior descending artery on 28 swine, randomized to SP 5 nmol/kg (group 1, n = 14) and normal saline (group 2, n = 14) given intravenously 5 min before reperfusion. Blood samples were collected at baseline, 3 days and 4 weeks. Echocardiography and myocardial perfusion single photon emission computed tomography (SPECT) were performed at 1 week and 4 weeks. Histomorphometric infarct size assessment was done at 4 weeks.</P> <P><B>Results</B></P> <P>Left ventricular (LV) ejection fraction (EF) (LVEF) after AMI induction was higher in group 1 than group 2 (37.9 ± 4.6% vs. 29.4 ± 3.2%, <I>p</I> = 0.001) but not different at 4 weeks. No significant difference was observed in perfusion defect extent and total perfusion defect on SPECT at 1 week and 4 weeks. Pathologic infarct size (% LV) was significantly smaller in group 1 than group 2 (2.4 ± 2.3% vs. 5.7 ± 2.5%, <I>p</I> = 0.020). The ratio of neutrophil to lymphocyte on day 3 and serum creatinine concentration at 4 weeks after AMI were lower in group 1.</P> <P><B>Conclusions</B></P> <P>In a porcine model of AMI, SP improved LVEF early post-MI and reduced infarct size.
