IgG4-Related Sclerosing Disease, an Emerging Entity: A Review of a Multi-System Disease

Immunoglobulin G4-related systemic disease (IgG4-RSD) is a recently defined emerging entity characterized by a diffuse or mass forming inflammatory reaction rich in IgG4-positive plasma cells associated with fibrosclerosis and obliterative phlebitis. IgG4-RSD usually affects middle aged and elderly patients, with a male predominance. It is associated with an elevated serum titer of IgG4, which acts as a marker for this recently characterized entity. The prototype is IgG4-related sclerosing pancreatitis or autoimmune pancreatitis (AIP). Other common sites of involvement are the hepatobiliary tract, salivary gland, orbit, and lymph node, however practically any organ can be involved, including upper aerodigestive tract, lung, aorta, mediastinum,retroperitoneum, soft tissue, skin, central nervous system, breast, kidney, and prostate. Fever or constitutional symptoms usually do not comprise part of the clinical picture. Laboratory findings detected include raised serum globulin, IgG and IgG4. An association with autoantibody detection (such as antinuclear antibodies and rheumatoid factor) is seen in some cases. Steroid therapy comprises the mainstay of treatment. Disease progression with involvement of multiple organ-sites may be encountered in a subset of cases and may follow a relapsing-remitting course. The principal histopathologic findings in several extranodal sites include lymphoplasmacytic infiltration, lymphoid follicle formation, sclerosis and obliterative phlebitis, along with atrophy and destruction of tissues. Immunohistochemical staining shows increased IgG4+ cells in the involved tissues (>50 per high-power field, with IgG4/IgG ratio >40%). IgG4-RSD may potentially be rarely associated with the development of lymphoma and carcinoma. However, the nature and pathogenesis of IgG4-RSD are yet to be fully elucidated and provide immense scope for further studies.

Cardiac tamponade due to purulent pericarditis with a characteristic greenish fluid is rare in this antibiotic era. It is highly fatal despite early diagnosis and advanced treatment. Gram-positive cocci are the leading cause of purulent pericarditis, which usually results from a direct or hematogenous spread of organisms to the pericardium from the primary foci of infection. We describe an index case of rapidly developing pericardial tamponade caused by oropharyngeal polymicrobial infection in the absence of a primary source of infection in a 62-year-old man, who was successfully managed with emergency large-volume pericardiocentesis followed by pericardiectomy.

The study aims to develop a multifunctionalfinished linen fabric by using an environment-benign, greenand sustainable route. The cellulose-based linen fabric has been functionally modified by aminopolysaccharide (chitosan, CS), zinc oxide nanoparticles (ZnO NPs), andflower extract of medicinal plant,Bombax ceiba. The NPs were stabilized by the extract on chitosan polymeric layer, which was bound onthe fabric through citric acid. Thus, developedfinished fabric, at three different pH values, was examinedfor bending length, tensile strength, crease recovery, UV-protection, antibacterial, and antioxidantactivity. The embedded ZnO NPs have been confirmed through FT-IR, Energy dispersive X-Rayspectroscopy (EDX), scanning electron microscopy (SEM), and transmission electron microscopy (TEM). The results showed effective crease recovery angle (CRA) and hiked UV protection factor (UPR) withsplendid percent antibacterial activity against Escherichia coli and Staphylococcus aureus (>90%) and highfree radical scavenging ability (80–90%) for the modified fabrics. The durability of the multifunctionalproperties has also been determined by several launderings.

DC drives even though considered obsolete in comparison to the AC induction motor drives, still retain several advantages in the control mechanism and operating characteristics. Despite the claims and counter-claims of suppliers over recent years there is no single drive technology that is ideal for all applications. The paper presents a full proof feedback loop speed control system and a soft start feature for a Shunt excited DC drive. Decisively such a control system can be adopted in tractive applications such as hybrid electric and Hydrogen fuel cell vehicles.
Embryo Culture of Taxus wallichiana (Zucc.)

Zygotic embryos were excised from immature and mature seeds of the Himalayan yew, Taxus wallichiana. The embryos germinated precociously when kept in darkness for 5 weeks and developed into full seedlings within 10-12 weeks. The highest rate of embryo germination ($81\%$) was obtained in modified Lloyd & McCown' s woody plant medium containing macro and micronutrients at half strength supplemented with $1\%$ activated charcoal, which supported both the best embryonic growth ($43\%$) and seedling development ($32\%$). However, the supplementation of basal media with kinetin, thidiazuron, 6-benzyl aminopurine or $GA_3$ had no effect on the germination of the embryos. The embryos derived from immature seeds germinated but the frequency of embryonic growth was better in mature seeds. Stratification of seeds effected precocious germination of embryos. Seeds kept at $4^{\circ}C$ for 1 week germinated earlier and at a higher frequency irrespective of the stage of seed maturity, while the germination rate declined with prolonged cold treatment for 1 month at that same temperature. Analysis of taxanes in germinating seedlings revealed that root tissues contained high levels of taxol, 10-deacetyl-baccatin ill and baccatin ill as compared to shoots. Thus embryo culture technique appears to overcome the lengthy dormancy requirement of T. wallichiana seeds.

EEG signals are not the direct access to the level of thought and thought related actions but it represents shadow of that and hence to some extent thought related patterns can be inferred and used for controlling action The Useful information contained in EEG signal is in the form of EEG Rhythm. Rhythmic analysis of EEG signal is very important for extraction of good features for classification of movement imagery data. This paper proposed the feature which is sum of power spectral density of left hemisphere channel (C3) as well as right hemisphere channel (C4) for movement imagery trials.

To curtail the Electrode placement time and accelerate the process it is required to record two channels placed on right and left hemisphere of brain based on active region depending on mental task. Raw EEG signals are contaminated with various kinds of noise such as ocular artifacts (EOG) and high frequency mussel noise etc. This paper is an application of wavelet transform based on Filter bank approach whereby it is possible to extract the noise free sub-band signal from noisy EEG signal. The performance of the Extracted Sub-band Components has been evaluated by taking the Power Spectral density for left hemisphere as well as right hemisphere channel sub-band components.
