A Rare Case of Tracheomalacia Associated with Vascular Ring in an Infant

Tracheomalacia can be a life threatening upper air way obstructive disease in an infant and vascular rings can be also a major rare cause of tracheoesophageal obstruction. These two rare entities can be combined in one patient because the vascular ring can cause secondary tracheomalacia during development of fetus. The diagnosis of this combination and adequate surgical correction is occasionally difficult. This is a report of an infant who had not diagnosed tracheomalacia associated with vascular ring until 5 months of age because of the prolonged tracheal intubation. The rigid bronchoscopic examination performed under impression of tracheomalacia revealed a concentric tracheal collapse, an unusual bronchoscopic findings of tracheomalacia, which raised a suspicion of the tracheal compression by vascular rings. The 3-D reconstructive DT aortography clearly demonstrated the double aortic arch. The patient was treated surgically by simple division of the left aortic arch and aortopexy with good result. The vascular ring such as double aortic arch should be considered during the diagnosis of tracheomalacia in infants. If the tracheomalacia is associated with vascular ring, simultaneous surgical correction should be performed.

Nerogenic tumor of various histologic types may arise in the posterior mediastinum. Mediastinal schwannoma is a frequent paraspinal neurogenic tumor, but malignant mediastinal schwannoma is rare tumor which is derived from Schwann cells. Although there are some reports dealing with approach for screening patients with symptoms suggesting malignancy and the imaging criteria for distinguishing malignant from benign schwannoma but the results are not clearly defined. We present a case of hugh mediastinal schwannoma which was taken for malignancy in imaging studies because of its invasiveness.
Tracheal and esophageal injury by fish bone

Tracheal injury associated with esophageal injury due to fish bone is very rare. Also, treatment of mediastinitis due to esophageal perforation when it is diagnosed late remains controversial. We report the case that we have successfully experienced treatment of mediastinitis due to tracheal and esophageal injury by fish bone.

Adenoid cystic carcinoma (ACC) of larynx is a rare disease, accounting for less than 1% of laryngeal malignancies, These tumors arise almost exclusively in the subglottic and supraglottic regions, while the proportion of glottic ACCs is small. We describe a case of a young man with a subglottis adenoid cystic carcinoma, The patient underwent laryngeal microscopic surgery and radiotherapy, Five years after radiotherapy, local recurrence and distant metastasis Clung and kidney) was detected and he underwent palliative chemotherapy and local treatment. He is still alive for seven years after the initial diagnosis. The treatment strategies for laryngeal ACC are still controversial due to the rarity of the condition. We suggest that multimodality approach may be helpful in deciding a treatment option, and thorough and consistent follow-up for recurrence is mandatory for these patients.
A Case of Tracheoesophageal Fistula Treated by Total Laryngectomy

A 47-year-old man was referred for TEF. He underwent tracheostomy three months ago to maintain prolonged ventilator care. Computed tomography (CT) scan and fiberoptic examination showed bilateral vocal cord palsy with median fixation and about 2 inch sized long segmental tracheoesophageal fistula (TEF) tract along the necrotic cricoid and tracheal cartilages. Narrow field total laryngectomy was performed to remove devitalized cartilages and mucosa, and repair TEF. He discharged without complication except mild stenotic change of tracheal fenestration 19 days later.
Prognostic Factor of Laryngeal Papillomatosis: 20 Years Experience

Background: Laryngeal papillomatosis, which is cased by human papillomavirus, is the most common benign neoplasm of the larynx. However, the prognosis of this disease remains really unpredictable. The aim of this paper is to determine whether any clinical features at the time of diagnosis could predict its course. Material and Method: Eighty-six patients treated at our institution during the last 20 years were analyzed retrospectively. All patients had microsurgery under general anesthesia. All patients' follow-up period was more than 1 year. We divided the patients into 1) Juvenile versus adult group based on their age 20, 2) single surgical method with laser versus dual surgical method with laser and microdebrider group, and 3) single subsite versus multiple subsites group. And we compared the therapeutic outcome. Results: The recurrence rate was 100% (15 patients) in the JP group and 56% (40 patients) in the AP group. Juvenile versus adult group was the only independent prognostic factor by univariate, and multivariate analysis. Microdebrider resection technique and multiple subsites were not associated with treatment result. Time period from the first surgery to recurrence detection was different statistically only for the age group. The number of surgery in the JP group ranged from 1 to 31 (mean 8.8). In the AP group the number of surgery ranged from 1 to 25 (mean 3.7). It was statistically different. Conclusion: Prognosis for the laryngeal papillomatosis remains unpredictable. Only patients' age was the only independent prognostic factor.
Treatment of Peritonsillar Abscess: Needle Aspiration versus Incision and Drainage

Background and Objectives: The Objective of this study was to compare the effectiveness of the needle aspiration method and the I&D method in the treatment of peritonsillar abscess. Materials and Methods: A prospective clinical study was performed on 83 patients. All the patients were hospitalized after random treatment with either I&D or needle aspiration alone, received the same intravenous antibiotic therapy. Among the 83 patients, 73 patients who could be observed for longer than 6 months and had not undergone a tonsillectomy during the follow-up period, were analyzed for treatment outcomes. Results: There was no statistically significant difference in the initial failure rate(p=0.572), the hospitalized days(p=0.956), the recurrence rate(p=0.531) for the needle aspiration(35 patients) and I&D groups(38 patients). But, The mean duration of fever were statistically different in the needle aspiration(1.51 hours) and I&D groups(3.05 hours) (p=0.031). Conclusion: Two methods are thought to be similar in effectiveness, except that duration of fever was longer in the I&D group than in the needle aspiration group. However, taking advantages of the needle aspiration method into consideration, the needle aspiration of peritonsillar abscess may be more appropriate than I&D as an initial method for peritonsillar abscess
