        The purpose of this study was to provide a curriculum for the education of medical record technician in school according to the computerization of hospital. The data used in this study were collected between 26 July 1999 and 7 September 1999 from the department of medical record of 44 tertiary hospitals in Korea using questionnaire. The results were as follows; 1) more than 70.0% of hospital were computerized but not sufficient to the needs of department of medical record. 3) most of the hospitals wanted the medical record technicians have qualified with medical informatics, specifically, information retrieval, statistical package, database, spread sheet, word processing, access and approach, power point, and windows. 4) the most important subjects in the education of medical record technicians were medical terminology, chart review, classification of disease and quality assurance. According to the above results a possible curriculum was developed. This model of curriculum can be devided as the field of medical record and medical informatics and the contents can be consisted of several areas such as biomedical science, health care delivery system, health data and structure, information technology, health care statistics analysis and research, clinical quality assessment and performance improvement and medical insurance and legal affairs in medical care.

      • 보건대학에서의 의료정보교육의 방향

        The purpose of this study was to provide the basic data for achieving effective and efficient medical information education in the College of Health Sciences through investigating the computer efficiency ability of the employees in each department in hospitals. The subjects were 232 employees who worked for ten hospitals(fiv departments) in Seoul. The data were collected by using a questionnaire from May 17 to May 24 in 2000. The results were as follows: 1. To teach the basic principles and knowledges in the fields of the computer hardware to the studentsis necessary for adaptation to an information-oriented society. 2. To teach the OA(Office Automation) such as Word Processor, Spreadsheet Presentation to the students is necessary because of the employees working in hospital must be to work their jobs related the EDPS inventively. 3. To teach the data structure, DBMS(Data Base Management System), file management system, database language related to the database to the students is necessary. 4. To teach the programming technology to the students is necessary because of the employees working in hospital needs to the mind about the computerization. 5. To practice the hospital package such as OCS(Order Communication System), hospital program in hospital is necessary.

      • 대학생의 학교보건시설이용에 관한 조사

        The purpose of this study is to provide efficient health management operating policy for college. A data for the study comes from questionary about usage condition of internal health facility in junior colleges. Total number of student who participate the response are 831 in 8 junior colleges. Among them, 53.7% of student do not know whether college is operating health facility, which means public relation is required. Response for the facility satisfaction, normal represents 46.3%, un-satisfied represents 43.3%, satisfied represents only 10.4% which means high rate of indifference and unsatisfied attitude. Systemic management for internal health related facilities, operation and public relation respected to expended facilities are also required. Current college health culture and improvement plan have to be seriously reviewed. Specially, establishment cooperation among college, sharing information and evaluation system of health related policy to college are required.

      • 의무기록정보관리 전산화 발전단계 평가

        The study was intended to evaluate the developmental phase of medical record information management system. For this study, the developmental phase of medical record information management was categorized into six stages. These were data processing Ⅰ, Ⅱ, discharge abstract system, information sharing system, and executive information system(utilization indicators, growth rate indicators and productivity indicators). Questionaries were sent to the 100 hospitals which have over 400 beds, and the answers of 67 hospitals were used for this evaluation.Data were analyzed by frequency test for the general characteristics of the cases, t-test, one-way ANOVA and correlation to examine the differences and the relationship between variables involved. The major results of this study were as follows. 1. The mean values of the medical record information management system levels. 1) The levels that marked the higher mean values were the data processing Ⅰ(92.8), the discharge abstract system(91.5), the data processing Ⅱ(61.6), and the utilization indicators(60.5) 2) The levels that marked the lower mean values were the EMR(10.3), the information sharing system(29.0), and the growth rate indicators plus productivity indicators(50.2) 2. The variables related to the mean value of medical record information management system levels. 1) The tertiary hospitals had higher mean values than those of general hospitals, Especially there were statistically significant differences in the discharge abstract system, the information sharing system and the utilization indicators. 2) The bed capacity, the begining year of computerization, the number of patients per 100 beds, and the number of employees of medical record department were not related to the mean valuse of medical record information system levels statistically. 3. The device that promotes the efficiency of the medical record information management system. 1) The hospitals that could retrieve the test results done by OCS in the medical record department were 65.7% in laboratory test and 52.7% in tissue pathdogy. In the future, all of diagnostic test results done by OCS must be retrived in the medical record department. 2) Among the 38 hospitals which used dictation system in the diagnostic radiology department, only 23 hospitals could retrieve the dictation result in the medical record department. Before introducing the PACS system, dictation result must be retrieved in the medical record department. 3) Among the 24 hospitals which keep the scanned medical record image, only 1 hospital could retrive the image in the LAN situation. From now on, scanned medical record image must be utilized for the multiusers in the hospital simultaneously. 4) Among the 40 hospitals which use E-mail system, only 2 hospitals produced the consultation report using it Using E-mail for intra-clinic communication will reduce the wasted time and produce efficiency for both clinician & patients.

      • 정보기술을 활용한 진료행위프로토콜 개발

        The main purpose of this paper is to explore the general methodology of developing protocol for medical service activities by using information technology and to actually develop a protocol for the case of cesarean section patients. Data for the study were obtained from a large university hospital located in Seoul, Korea. The data items include discharge summaries, prescriptions, and medical fee records of discharged cesarean section patients for the period of January through March, 1999. We analyzed the data by using newly developed method of 'SAS Enterprise Miner' as well as ordinary statistical programs of SAS. We performed cluster analysis on the basis of length of stay and the amount of medical fees in order to determine the most appropriate group, and the results for the group were used in the development of the protocol. The protocol(major result of this study) is as follows: 1) basic prescriptions -lst day of hospitalization : BR, V/S CHECK, etc. -2nd day of hospitalization : NPO, BR, etc. -3rd day of hospitalization : BR with WA, NPO -4th day of hospitalization : BR with WA, V/S CHECK, SOW -5th and 6th days of hospitalization : BR with WA, V/S CHECK 2) examinations -1st day of hospitalization : NST, FAST, TYPE & SCREENING -2nd day of hospitalization ABGA 3) medication/injection -1st day of hospitalization 5% dextrose -2nd day of hospitalization : metronidazole, methylergometrin maleate, etc. -3rd day of hospitalization : dextrose/ sodium chloride, aminosteril, etc. -5th day of hospitalization : iron protein succinylate, methylergometrin maleate

      • 종합병원 직원의 정보화교육을 위한 정보기술 이용행동의 전제요인의 변이에 관한 연구

        Using the core constructs of the theory of reasoned action(TRA) and the technology acceptance model(TAM), this study tried to identify, based on demographic variables and IT adoption status, differences of hospital workers' behavioral pre-dispositions in using IT. Information derived from this study can contribute to desinging an effective IT training program that can fully take into account and persons with low education had Significantly lower scores of behavioral pre-dispositions than other sub-groups. Thus this study insists that focused training efforts for those groups result in a more effective solution to promote use of IT than an unfocused general IT training session. Moreover, the development of special IT training programs for the identified groups be an essential part for promoting their adoptions and use of IT.

      • 소비자 중심의 노인 e-Health 모형

        The social change of 21th century are moving two directions. The one is the aged society, and the other is information society which has been closely related in the development of human society. These two directions of social change have been affected by science evolution, specially both micro-electronic revolution and bio-technology. Korean society has been rapidly changing to the aged and information society. The telecommunication technology may apply to a lot of fieds in health care field, particularly e-health fields. So the purpose of this Study is to applicate health information system in the field of aged health and develop the model of e-Health for elderly. Available information systems for e-health are Clinical Information System(CIS), Treatment Planning System, Electronic patient record system, Community Health Information Network(CHIN), Telemedicine, and Information and Data Exchange Access System(IDEAS). E-health system may progress the health care delivery and could supply 'On-demand'' service for individual needs. It is needed to develop sites for older population. It should be one of the strategies to enhance internet accessibility of the aged. It will be significant information for the aged as a cyber support system. Senior-net should be built up for responding to increasing number of older internet user. Control manager has to screen and manage senior-net sites when it be developed. About setting-up welfare information-telecommunication systems and apparatuses for high information society, it is needed that stuck a more effective development system from a design processing for elderly. For the one step further evolution of health care we need to effort develop the application science and technology in telecommunication for the aged.

      • 건강정보 인터넷 사이트 평가

        Through the internet the public in South Korea has access to a growing supply of information on health and disease. In South Korea an estimated 13.93 million people used the internet in 2000. The number has increased rapidly compared to 1.63 million in 1997. Health information is often said to be one of the most retrieved types of information on the internet. However, a concern has emerged for the quality of health information documents contained on the world Wide Web. Lack of evaluation and oversight, and ease of publication, have led to inaccurate and misleading health-related publications on the Internet. For those seeking easy ways to identify high-quality and reliable information rating systems to evaluate the quality of health information on the internet should be provided and developed. Given this background, the purpose of this study was to evaluate health information web sites on the internet. In this study, we aimed to survey websites providing health information. 440 websites were selected by using four search engines, Yahookorea (http://www.yahoo.co.kr), LycosKorea (http://www.lycos.co.kr), Empas (http://www.empas.co.kr), and Naver (http://www.naver.com), to conduct searches in November 2000. General quality criteria were used for the evaluation. These included ownership, currency, authorship, source, feed-back mechanism, links, and functionality. More than 50% of web sites did not provide the date of publication or update of information, author and author credentials, references to source, etc. Websites of universities and universities' hospitals were more likely to provide name and type of provider (p<.01), authors name (p<.001), and references to source than other service providers (p<.01). There is a need for better evidence-based health information as well as a need to develop simple criteria that ordinary people can understand and use. In addition, gateway services that operate a selective process and provide links to other organization that provide high-quality health information should be offered and developed.

      • Analysis of Daily Medical Charge and Hospital Revenue : Case of Treatment for Herniated Lumbar Disc

        The purpose of this study is to analyze the effect of hospital revenue increase by shortening length of stay after examining of medical charge by details of treatment. In doing so, herniated lumbar disc is chosen and the aspect of medical charge for it by length of stay is first examined. 169 patients who were discharged from an University Hospital of 1000 beds scale from January 1,1997 to December 31,1997 after treatment for herniated lumbar disc were made the subject of this study. The results of medical charges by details of treatment and length of stay were investigated according to operated cases or unoperated cases. Medical charge for herniated lumbar disc in case of operation was low on the first day of admission. However, it was high on the second and the third day of admission, while it rapidly decreased on the forth day of admission. It gradually decreased after the forth day and displayed finally constant level. The effect of annual revenue per bed by shortening of the average length of stay by one day, the annual revenue increased 1,678,000 won in case of operated patients, while it increased 793,000 won in case of unoperated patients.
