The purpose of this research was to constructed clinical database to support medical research, hospital management, electronic medical record. Data was collected from records of hospital discharges, order, hospital charges of 4,689 patients who were hospitialized with 10 disease categorys at a university medical center from January 1, 1997 to December 31, 1997. In order to database modeling, the new concepts of Information Modeling are needed. In this study, Entity-Relationship Diagram are suggested. The kinds of Entity are Patient, Order, Treatment, Disease.

In the health-care market, the shift from a fee-for-service to a DRG environment has dramatically altered the landscape. To survive in this situation , hospital have to change. Information technolgy is one of change means. Knowledge Management on Hospital is necessary to Information technolgy. This study was aimed to investigate medical utilization statistics of type and it’s reliability. Second objective is to suggest methods to improγe reliability of patient survey statistics. We collected patient survey data, health insurance data, discharge summary data in 2005 and reliability of medical utilization statistics analyzed using rate of diagnosis. We found correspondence rate of diagnosis among health insurance statistics, patient survey statistics, discharge injury survey statistics was not high. The rate of Mental and behavioral disorders was 5.0% in health insurance statistics, was 2.4% in patient survey statistics, was 1.3% in discharge injury survey statistics. The rate of diseases of the respiratory system was 10.3% in health insurance statistics, was 8.5% in patient survey statistics, was 12.1% in discharge injury survey statistics. Based on these results, to improvement reliability of medical utilization statistics, this study suggest public health statistics system building, performance of patient survey four times yearly, securing expert.

The purpose of this study is to identify the factors of regional differences in self-perceived health. To do this, we collected the 2012 Community Health Survey data and created variables related to self-rated health. Data analysis using multiple regression and decision tree model was performed. The region with many male population, high socioeconomic level and good health behavior has a strong tendency towards high self-perceived health status. On the other hand, the region where there are many chronic disease patients has poor self-ratings of health. Regions were classified into 14 groups by the decision tree analysis. It is recommended that the results of this study are used as base data for self-perceived health improvement projects. 본 연구는 지역별 주관적 건강인식의 차이 및 변이의 요인을 규명하는 것이다. 이를 위하여 2012년 지역사회건강조사 자료를 수집하여 지역별 주관적 건강인식률과 관련 변수를 생성하여 이를 이용하여 지역별 주관적 인식률의 차이 요인을 규명하였다. 분석방법은 다중회귀분석과 의사결정나무분석을 이용하였다. 모델의 우수성에 대해서 평가한 결과 의사결정나무 모형이 우수한 것으로 평가 되었다. 의사결정나무에 따르면 평균 가구월수입이 높을수록, 필수 의료서비스 미치료율이 낮을수록, 건강행태군집 수준이 높을수록, 1인 가구 비율이 낮을수록, 아파트 거주 비율이 낮을수록 주관적 건강인식률이 높았다. 의사결정분석 결과 지역의 유형은 12개로 분류되었으며 이를 기반으로 지역별 특성에 맞는 주관적 건강인식률을 향상시킬 수 있는 방안을 도출 할 수 있는 기초자료를 제공하였다.
데이터마이닝 기법을 이용한 건강보험공단의 수술 통계량 근사치 추정 -허니아 수술을 중심으로-

Objectives: The aim of this study is to develop a methodology for estimating a nationwide statistic for hernia operations with using the claim database of the Korea Health Insurance Cooperation (KHIC). Methods: According to the insurance claim procedures, the claim database was divided into the electronic data interchange database (EDI_DB) and the sheet database (Paper_DB). Although the EDI_DB has operation and management codes showing the facts and kinds of operations, the Paper_DB doesn't. Using the hernia matched management code in the EDI_DB, the cases of hernia surgery were extracted. For drawing the potential cases from the Paper_DB, which doesn't have the code, the predictive model was developed using the data mining technique called SEMMA. The claim sheets of the cases that showed a predictive probability of an operation over the threshold, as was decided by the ROC curve, were identified in order to get the positive predictive value as an index of usefulness for the predictive model. Results: Of the claim databases in 2004, 14,386 cases had hernia related management codes with using the EDI system. For fitting the models with applying the data mining technique, logistic regression was chosen rather than the neural network method or the decision tree method. From the Paper_DB, 1,019 cases were extracted as potential cases. Direct review of the sheets of the extracted cases showed that the positive predictive value was 95.3%. Conclusions: The results suggested that applying the data mining technique to the claim database in the KHIC for estimating the nationwide surgical statistics would be useful from the aspect of execution and cost-effectiveness.
