        Characterization of microfiltration and ultrafiltration membranes for application in kombucha filtration

        Membranes are barriers that act as filters, retaining particles and molecules larger than their pore sizeand allowing smaller particles and molecules to permeate. This study aimed to evaluate membranes ofmicrofiltration and ultrafiltration for later application in the filtration of kombuchas. To achieve the proposedobjective, the membranes were compacted and characterized in terms of hydraulic permeabilityand kombucha permeate flux. The hydraulic permeability of the microfiltration membrane was 308.15Lh1m2bar1 and that of the ultrafiltration membrane was 12.05 Lh1m2bar1. For both membranes,the permeate kombucha flux reduced with time for each pressure evaluated individually, dueto fouling.

        Mineral Composition of Different Strains of Edible Medicinal Mushroom Agaricus subrufescens Peck

        Agaricus subrufescens Peck is a well-known Basidiomycota fungus (Royal Sun Agaricus), with rising demand in consumption and production worldwide. This particular mushroom with high medical value has been used successfully in cancer therapy and in the treatment of some bacterial and viral diseases. Four strains of A. subrufescens (Si2.2, 853, 1105, and 2603) were cultivated, and 22 mineral elements of basidiomes (fruit bodies) were analyzed (caps and stipes separately). The data obtained about the mineral compositions were compared to the “reference” Agaricus bisporus (white button mushroom) and to the average of wild growing Agaricus species. The mineral composition of A. subrufescens strains can be characterized by the following: (1) high levels of valuable macroelements, i.e., potassium (28–30,000mg/kg of dry matter), phosphorus (7–11,000mg/kg of dry matter), and calcium and magnesium (for both elements, 1,000–1,500mg/kg of dry matter); (2) significantly higher level of copper (compared to A. bisporus, 70–150mg/kg of dry matter) and zinc (140–250mg/kg of dry matter); (3) low quantity of sodium (140–180mg/kg of dry matter); (4) attention should paid to the detectable amount of cadmium (2–17mg/kg of dry matter) in strain Si2.2; (5) low or undetectable concentrations of some other poisonous microelements like As, Cr, and V; and (6) the distribution of elements in caps and stipes is characteristic—the majority of beneficial elements have higher contents in caps than in stipes, but some other elements, such as Ca, Fe, and Na, appear in an inverse proportion. In conclusion, it can be said that the mineral composition of A. subrufescens is definitely positive, with the exception of the above-mentioned Cd level.

        Treating Treatment-resistant Depression with Esketamine Nasal Spray When All Therapeutic Options Have Been Exhausted: Clinical Experience from a Spanish Cohort of Expanded Use

        Objective: Treatment Resistant Depression (TRD) is commonly defined as the lack of response to two or more antidepressants with different mechanisms of action. Up to 30% of patients diagnosed with major depressive disorder might be considered to present TRD. The objective of this study was to assess the effectiveness and tolerability of esketamine in patients diagnosed with TRD, who were referred to our program after exhausting all available treatments. A secondary objective consisted in researching the relationship between response and previous use of electroconvulsive therapy. Methods: A prospective, observational study was carried out in patients enrolled in the expanded use of esketamine in our center. They received esketamine prior to its marketing authorisation, for therapeutic purposes. Sixteen subjects were analyzed. Effectiveness was assessed with the Montgomery-Asberg depression rating scale (MADRS). Patients were followed up to 4 months after the administration. Results: Esketamine showed a rapid, robust effect in improving depressive symptoms, with no specific correlation between outcome and any demographic or clinical traits evaluated. No differences were found between patients that previously received Electroconvulsive Therapy, and those that didn’t. 10 out of 16 patients responded (> 50% change in baseline MADRS scores), but only five achieved remission (< 12 points in the global MADRS score). We provide some recommendations, based on clinical experience, to improve tolerability and adherence, and to manage adverse effects. Conclusion: Results suggest that esketamine is a safe, effective and rapid-acting option for TRD. More studies are needed to properly assess predictors of response outcome.

        Effect of artificial aging on mechanical and physical properties of CAD-CAM PMMA resins for occlusal splints

        Purpose. This study aimed to assess and compare the color stability, flexural strength (FS), and surface roughness of occlusal splints fabricated from heat-cured acrylic resin, milled polymethyl methacrylate (PMMA)-based resin, and 3D-printed (PMMA) based-resin. Materials and methods. Samples of each type of resin were obtained, and baseline measurements of color and surface roughness were recorded. The specimens were divided into three groups (n = 10) and subjected to distinct aging protocols: thermomechanical cycling (TMC), simulated brushing (SB), and control (without aging). Final assessments of color and surface roughness and three-point bending test (ODM100; Odeme) were conducted, and data were statistically analyzed (2-way ANOVA, Tukey, P <.05). Results. Across all resin types, the most significant increase in surface roughness (Ra) was observed after TMC (P < .05), with the 3D-printed resin exhibiting the lowest Ra (P < .05). After brushing, milled resin displayed the highest Ra (P < .05) and greater color alteration (∆E00) compared to 3D-printed resin. The most substantial ∆E00 was recorded after brushing for all resins, except for heat-cured resin subjected to TMC. Regardless of aging, milled resin exhibited the highest FS (P < .05), except when compared to 3D-printed resin subjected to TMC. Heat-cured resin exposed to TMC demonstrated the lowest FS, different (P < .05) from the control. Under control conditions, milled resin exhibited the highest FS, different (P < .05) from the brushed group. 3D-printed resin subjected to TMC displayed the highest FS (P < .05). Conclusion. Among the tested resins, 3D-printed resin demonstrated superior longevity, characterized by minimal surface roughness and color alterations. Aging had a negligible impact on its mechanical properties.

        Tip and taper compatibility of accessory gutta-percha points with rotary and reciprocating instruments

        Objectives This study was conducted to evaluate and compare the tip and taper compatibility of accessory gutta-percha points (AGPs) with various rotary and reciprocating instruments. Materials and Methods Using a profile analyzer, tip and taper measurements were taken of 10 AGPs of each of the 14 models available from Odous de Deus and the 4 models available from Dentsply-Maillefer. Diameter measurements were taken at 1-mm intervals, from 3 mm from the tip (D3) to 16 mm. Results Based on the mean values obtained, 3-dimensional (3D) models of the AGPs were drawn in Autodesk Fusion 360 and superimposed on 3D models of each instrument selected (Mtwo, Reciproc, RaCe, K3, and ProDesign Logic) to determine the compatibility between the instrument and the AGP. Data corresponding to the tips and tapers of the various AGPs, as well as the tip and taper differences between the AGPs and the instruments, were analyzed using descriptive statistics. The tapers of the AGPs were subject to the American National Standards Institute/American Dental Association No. 57 standard. The Odous de Deus extra-long medium and extra-long extra-medium AGPs were shown to be compatible with Mtwo, K3, and ProDesign Logic instruments with taper 0.06 and tip sizes 25 and 30, while the Dentsply fine and fine medium cones were compatible with Mtwo, RaCe, and K3 instruments with conicity of 0.04 and tip sizes 35 and 40. Conclusions Both the Odous de Deus and Dentsply commercial brands included 2 AGP models with tip (D3) and taper compatibility with Mtwo, RaCe, K3, and/or Prodesign Logic instruments.

        Electron beam treatment for eliminating the antimicrobial activity of piperacillin in wastewater matrix

        Effluents of wastewater treatment plants represent critical control points for antibiotic resistancemanagement. To meet strict regulations coming into effect in the future advanced technologies need tobe implemented that can remove the factors contributing to the development of resistance in receivingnatural environments. By performing microbiological assays we show that electron beam treatment of acomplex synthetic effluent wastewater matrix is able to eliminate one of these factors, the antimicrobialactivity of the b-lactam antibiotic piperacillin present at environmentally relevant concentration. SinceOH governs the antibacterial inactivation the technology needs to be designed to the stoichiometricpresence of OH.

        Clot Composition Analysis as a Diagnostic Tool to Gain Insight into Ischemic Stroke Etiology: A Systematic Review

        Mechanical thrombectomy renders the occluding clot available for analysis. Insights into thrombus composition could help establish the stroke cause. We aimed to investigate the value of clot composition analysis as a complementary diagnostic tool in determining the etiology of large vessel occlusion (LVO) ischemic strokes (International Prospective Register of Systematic Reviews [PROSPERO] registration # CRD42020199436). Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we ran searches on Medline (using the PubMed interface) and Web of Science for studies reporting analyses of thrombi retrieved from LVO stroke patients subjected to mechanical thrombectomy (January 1, 2006 to September 21, 2020). The PubMed search was updated weekly up to February 22, 2021. Reference lists of included studies and relevant reviews were hand-searched. From 1,714 identified studies, 134 eligible studies (97 cohort studies, 31 case reports, and six case series) were included in the qualitative synthesis. Physical, histopathological, biological, and microbiological analyses provided information about the gross appearance, mechanical properties, structure, and composition of the thrombi. There were non-unanimous associations of thrombus size, structure, and composition (mainly proportions of fibrin and blood formed elements) with the Trial of Org 10172 in Acute Stroke Treatment (TOAST) etiology and underlying pathologies, and similarities between cryptogenic thrombi and those of known TOAST etiology. Individual thrombus analysis contributed to the diagnosis, mainly in atypical cases. Although cohort studies report an abundance of quantitative rates of main thrombus components, a definite clot signature for accurate diagnosis of stroke etiology is still lacking. Nevertheless, the qualitative examination of the embolus remains an invaluable tool for diagnosing individual cases, particularly regarding atypical stroke causes.

        The impact of first-trimester intrauterine hematoma on adverse perinatal outcomes

        Purpose: The aim of this study was to evaluate whether the presence of an intrauterinehematoma (IUH) on an early pregnancy ultrasound scan showing a live fetus was related toadverse perinatal outcomes. Methods: We performed a retrospective cohort study to evaluate pregnant women whounderwent an ultrasound examination in early pregnancy, between 6 weeks 0 days and 10weeks 6 days. We compared the perinatal outcomes between women with and without firsttrimester IUH using the Mann-Whitney and Fisher exact tests. Furthermore, we performed astepwise regression analysis to identify possible predictors of miscarriage among maternalcharacteristics, ultrasound parameters, and IUH. Results: During the study period, data from 783 pregnancies were included, and the incidence ofIUH was 4.5% (35 of 783). We observed a higher proportion of miscarriage following the scan(28.6% vs. 10%, P=0.003) and a larger yolk sac diameter during the scan (4.8 mm vs. 3.8 mm,P<0.001) in the pregnant women with first-trimester IUH. There was no significant differenceregard the prevalence of low birth weight (LBW; P=0.091), very LBW (P=0.370), or extremelyLBW (P=0.600) between cases with IUH and without IUH, the cesarean section rate (68% vs. 81%, P=0.130), preterm delivery (16% vs. 16%, P>0.999), or the incidence of first-trimestervaginal bleeding (31% vs. 20%, P=0.130). Moreover, heart rate (HR) was the only variable thatpredicted miscarriage with statistical significance (P=0.017). Conclusion: Women with first-trimester IUH had a higher risk of miscarriage after the ultrasoundscan. HR was the only variable that predicted miscarriage with statistical significance.

        Effect of the type of resin cement on the fracture resistance of chairside CAD-CAM materials after aging

        Purpose. The study objective was to evaluate the influence of the type of resin cement on the flexural strength and load to fracture of two chairside CAD-CAM materials after aging. Materials and Methods. A polymer-infiltrated ceramic network (PICN) and a nanoceramic resin (RNC) were used to produce the specimens. Two types of dual-cure resin cements, a self-adhesive and a universal, were investigated. Bilayer specimens were produced (n = 10) and aged for 6 months in a humid environment before the biaxial flexural strength test (σf). Bonded specimens were subjected to a mechanical aging protocol (50 N, 2 Hz, 37°C water, 500,000 cycles) before the compressive load test (Lf). σf and Lf data were analyzed using two-way ANOVA and Tukey tests (α = .05). Chi-square test was used to analyze the relationship between failure mode and experimental group (α = .05). Results. The type of resin cement and the interaction between factors had no effect on the σf and Lf of the specimens, while the type of restorative material was significant. RNC had higher σf and Lf than PICN. There was a significant association among the type of cracks identified for specimens tested in Lf and the restorative material. Conclusion. The type of resin cement had no effect on the flexural strength and load to fracture of the two investigated CAD-CAM chairside materials after aging.
