Mid-ventricular obstructive hypertrophic cardiomyopathy (MVOHCM) is a rare type of cardiomyopathy, associated with apical aneurysm formation in some cases. We report a patient presenting with ventricular fibrillation, an ECG with an above normal ST segment, and elevated levels of cardiac enzymes but normal coronary arteries. Left ventriculography revealed a left ventricular obstruction without apical aneurysm. There was a significant pressure gradient between the apical and basal sites of the left ventricle. Cine magnetic resonance imaging (MRI), performed on the 10th hospital day, showed asymmetric septal hypertrophy, mid-ventricular obstruction, and an apical aneurysm with a thrombus. The first evaluation by contrast-enhanced imaging showed a subendocardial perfusion defect and delayed enhancement. It was speculated that the intraventricular pressure gradient, due to mid- ventricular obstruction, triggered myocardial infarction, which subsequently resulted in apical aneurysm formation.

Cadmium telluride (CdTe) thin films were grown on sapphire c-face single-crystal substrates by vacuum evaporation. Prior to growing the CdTe thin films, conducting thin films of Ni, Mo, Ti and titanium nitride (TiN) were grown on the sapphire substrates. The crystallinities and photoluminescence (PL) properties of the CdTe thin films grown on the various conducting thin films were investigated and they were compared with those of CdTe thin films grown directly on sapphire substrates without a conducting thin film. CdTe thin films with relatively high crystallinities were obtained when Ti and TiN were used as the conducting thin film. Therefore, the PL properties of the CdTe thin films on such conducting thin films were not inferior to those of the CdTe thin film grown directly on the sapphire substrate. The effect of the Cd and Te supply ratio on the properties of the CdTe thin films was also investigated. The CdTe thin films had much higher crystallinities when they were grown in Te-rich conditions than in Cd-rich conditions.
Species richness of sawfly–host plant associations at higher taxonomic levels

Insect–plant interactions are important to understanding the evolutionary mechanisms responsible for most of the diverse plant‐feeding insects. We aggregated data on sawfly–host plant associations and other resource associations from existing sources to address the following questions: (i) Is there a general correlation between host diversity and sawfly species richness? (ii) Is the pattern of host plant use consistent across sawfly lineages? and (iii) Is there a phylogenetically significant shift in species richness among sawflies? Our analysis comprised 8567 sawfly species, including 2087 species with host plant and other records. In total, there were 2126 records of host usage for sawflies, the overwhelming majority of which were sawflies using angiosperms as resources. Rosales are used by most of the species in sawfly families or subfamilies. We found that there was a strong correlation between the number of host plant orders and the species richness of sawfly families and subfamilies. To examine the points at which sawflies have experienced significant shifts in species richness, we compared sister taxon species richness. Several positive and negative shifts in species richness among sawflies were related to their range of host plant usage and specialized niche, respectively. In general, we found that most of the sawfly families and subfamilies used several orders as host plants, but mainly core eudicots, although some families or subfamilies were specialized on pteridophytes or gymnosperms.

Objectives: Rheumatoid arthritis (RA) is characterized by chronic inflammation of the synovium, progressive erosion of the articular cartilage, and joint destruction. RA also causes secondary osteoporosis and muscle wasting. We investigated the effects of ibandronate (IBN), a bisphosphonate; eldecalcitol (ELD), an active vitamin D3 derivative; and combination treatment with both agents on secondary osteoporosis and muscle wasting using adjuvant-induced arthritis rats. Methods: Arthritis was induced in 8-week-old male Lewis rats. Rats were randomized into 4 treatment groups and an untreated normal control group: IBN (subcutaneously, once every 2 weeks,10 mg/kg), ELD (orally, once daily, 30 ng/kg/day), IBN þ ELD, vehicle, and control. Paw thickness measurements were performed for evaluation of arthritis. The femur was scanned using dual-energy X-ray absorptiometry. Cross-sectional areas of left tibialis and anterior muscle fibers and the expression of MuRF1, atrogin-1, MyoD, and myogenin in the gastrocnemius muscle were measured to evaluate muscle wasting. Results: IBN and/or ELD increased bone mineral density (BMD) in the femur. In addition, there was an additiveeffect of combination treatment compared with single treatments for BMD. However, IBNand/or ELD did not inhibit muscle wasting in adjuvant-induced arthritis rats. Conclusions: Combination treatment with IBN and ELD may be effective for secondary osteoporosis associated with RA. Other treatments are necessary for muscle wasting associated with RA. Studies in humans are needed to confirm these findings.

<P>Bosentan is a substrate of hepatic uptake transporter organic anion-transporting polypeptides (OATPs), and undergoes extensive hepatic metabolism by cytochrome P450 (P450), namely, CYP3A4 and CYP2C9. Several clinical investigations have reported a non-linear relationship between bosentan doses and its systemic exposure, which likely involves the saturation of OATP-mediated uptake, P450-mediated metabolism, or both in the liver. Yet, the underlying causes for the nonlinear bosentan pharmacokinetics are not fully delineated. To address this, we performed physiologically based pharmacokinetic (PBPK) modeling analyses for bosentan after its intravenous administration at different doses. As a bottom-up approach, PBPK modeling analyses were performed using in vitro kinetic parameters, other relevant parameters, and scaling factors. As top-down approaches, three different types of PBPK models that incorporate the saturation of hepatic uptake, metabolism, or both were compared. The prediction from the bottom-up approach (models 1 and 2) yielded blood bosentan concentration-time profiles and their systemic clearance values that were not in good agreement with the clinically observed data. From top-down approaches (models 3, 4, 5-1, and 5-2), the prediction accuracy was best only with the incorporation of the saturable hepatic uptake for bosentan. Taken together, the PBPK models for bosentan were successfully established, and the comparison of different PBPK-models identified the saturation of the hepatic uptake process as a major contributing factor for the nonlinear pharmacokinetics of bosentan.</P>
Bond-Strengthening Hooks for RC Members with High Strength Spirals

This paper presents an experimental investigation of bond-strengthening hooks as a new method to increase bond strength along flexural reinforcing bars in reinforced concrete (RC) beams and columns. The RC members, which consisted of 1,300 MPa-class spirals as shear reinforcement, often suffered from bond splitting failure. The proposed method attempts to increase confining stiffness around the flexural bars by placing U-shaped hooks and to prevent premature bond splitting failure. Twelve specimens with varied amounts and sizes of the hooks were prepared to verify the strengthening effectiveness under monotonic and cyclic loading conditions. The test result indicated that the hooks increased the bond strength along the flexural bars although the strengthening effectiveness was limited by effective reinforcement ratio pbe. This limit is determined by size of stress-transmitting zones of concrete around anchors of the hooks. Anchors of the hooks are recommended to be longer than twelve times the hook diameter and inserted deeper than a quarter of the member depth (D/4). Proposed design equations provide modest estimates of the shear strengths.
Clinicopathological Significance of S100A10 Expression in Lung Adenocarcinomas

Background: S100A10, of the S100 protein family, is reported to be involved in cancer cell invasion and metastasis. The aims of the present study were to immunohistochemically examine S100A10 expression in surgically resected lung adenocarcinomas, and evaluate any relationships with clinicopathological parameters and prognosis of patients. Materials and Methods: S100A10 expression was immunohistochemically studied in 202 consecutive resected lung adenocarcinomas, and its associations with clinicopathological parameters were evaluated. Kaplan-Meier survival analysis and Cox proportional hazards models were used to estimate the effect of S100A10 expression on survival. Results: S100A10 expression was detected in 65 of the 202 (32.2%) lung adenocarcinomas, being significantly correlated with poorer differentiation (P =0.015), a higher pathological TNM stage (stages II and III) (P=0.004), more frequent and severe intratumoral vascular invasion (P=0.001), and a poorer prognosis (P=0.030). However, S100A10 expression was not an independent predictor of survival after controlling for clinicopathological factors. Conclusions: The present study reveals that S100A10 is expressed in a subset of lung adenocarcinomas, and this is related to some clinicopathological parameters, although further studies are required to confirm the correlation between S100A10 expression and prognosis of lung adenocarcinoma patients.

A 50-year-old woman had a percutaneous endoscopic gastrostomy (PEG) tube placed after surgery for pharyngeal cancer. After 21 months, the PEG tube was removed due to improvement of per-oral ingestion. She had taken prednisolone for 31 years for systemic lupus erythematosus. The post-PEG fistula did not close spontaneously. The cause of the fistula was slow wound healing and gastrostomy site inflammation due to long-term steroid therapy. We were able to close the fistula with an over-the-scope clipping (OTSC) system. This case suggests that OTSC is useful for closing persistent post-PEG fistulas in patients receiving long-term prednisolone therapy.

Background: Pemetrexed monotherapy has come to be recognized as one of the standard second-line therapies for advanced non-squamous non-small cell lung cancer (NSCLC). However, there have been no reports of studies that have evaluated the efficacy of pemetrexed according to type of active EGFR mutation, i.e., an exon 19 deletion or an L858R point mutation. Materials and Methods: The records of non-squamous NSCLC patients harboring an EGFR mutation who received pemetrexed monotherapy as a second or later line of chemotherapy at Kitasato University Hospital between March 2010 and October 2015 were retrospectively reviewed, and the treatment outcomes were evaluated. Results: The overall response rate and progression-free survival time (PFS) of the 53 patients with non-squamous NSCLC were 15.1% and 2.3 months, respectively. There were significant differences between the disease control rate (37.5% vs. 76.2%) and PFS time (1.8 months vs. 3.3 months) of the exon 19 deletion group and the L858R point mutation group, and a multivariate analysis identified type of EGFR mutation as well as performance status (PS) as independent predictors of PFS. Conclusions: The clinical data obtained in this study provided a valuable rationale for considering type of EGFR mutation as well as non-squamous histology as predictors of the efficacy of pemetrexed monotherapy.
