견고 구외 골신장술을 이용한 구순구개열 환아의 치험례 : CASE REPORT

구순구개열 환자에서는 이른 시기에 시행된 수술로 인한 구순이나 구개부의 반흔 형성으로 섭식장애나 발음장애를 동반한 상악의 열성장이 나타나게 된다. 때로는 집중적인 교정치료 후에도 심한 상악골 저형성증을 보이며, 이러한 경우 성장이 완료된 후에 골이식을 동반한 악교정 수술로 상악골을 전방이동시켜 안모의 개선을 도모하기도 한다. 그러나 이러한 상악골의 전방이동은 연조직의 과도한 신장으로 인한 술후회귀현상, 추가적인 골이식이 필요하다는 한계를 가지고 있다. 골신장술은 이러한 한계점을 극복하는 최신 치료방법으로 구순구개열 환자, 두개골 융합증을 나타내는 환자 등에서 상악골을 포함한 두개 안면골의 개선에 많이 이용되고 있다. 특히 구외장치를 이용하는 골신장술은 골신장기간 중에 견인 방향의 조절이 보다 쉬우며, 충분한 양의 골신장이 가능하여 보다 좋은 결과를 얻을 수 있다. 본 증례는 상악 열성장을 보이는 구순구개열 6세 7개월의 여자 환아로, 변형된 구내 프로씨 1형 골전단술(modified Le Fort I osteotomy) 후 두개골을 고정원으로 이용하는 견고 구외 골신장술을 통해 상악골을 전진시켜 양호한 결과를 얻었기에 이를 보고하는 바이다. Patients with cleft lip and palate present severe maxillary hypoplasia due to scar of lip and palate, often accompanied by compromised mastication, speech abnormalities. Sometimes maxillary hypoplasia persist even though active orthodontic treatment was done. In theses cases, patients born with cleft lip and palate will be potential candidates for maxillary advancement with bone grafting after growth to correct the functional deformities and improve aesthetic facial proportions. But, maxillary advancement using standarf surgical approaches has several limitations: increased relapse tendency after maxillary advancement, necessity of additional bone graft and mandibular setback surgery. Distraction osteogenesis is current treatment modality to overcome these limitations, thus has become popular for treatment of maxillary hypoplasia associated cleft lip and palate, craniosyntosis. Especoally, rigid external distraction, contrary to internal device, has adbantages; better vector control of osteotomized segment, effective traction of the bony segments, the ease of the application and removal the distraction device. This study showed that relatively successful result could be generated by using rigid external distraction osteogenesis(RED) in the case of cleft lip and palate with severe maxillary hypoplasia, 6 years 7 months old.

In this study, we used RNA sequencing (RNA-seq) to analyze and compare bulk cell samples from wild-type (WT) dermal mesenchymal stem cells (DMSCs) (n = 3) and Prx II knockout DMSCs (n = 3). The purpose of the study was to elucidate the role of Prx II on allogeneic immune rejection of transplanted DMSCs. The results revealed differential expression of 472 genes (176 up-regulated and 296 down-regulated; p ≤ 0.05) between the PrxII+/+ (WT) and PrxII−/− sample groups. When highly regulated genes were categorized according to the Gene Ontology (GO) molecular function classification and the Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis, the PrxII−/− samples showed a robust downward trend in allograft rejection. The study identified 43 all immunologically rejected differentially expressed genes, of which 41 showed lower expression in the PrxII−/− vs. PrxII+/+ (WT) samples. These findings suggest that Prx II gene knockout may down-regulate the allograft rejection that occurs during DMSCs transplantation and improve the survival rate of DMSCs in the host. This study provides a new perspective on the clinical treatment of stem cell transplantation.

A periodically poled device was investigated by using fourth-order quasi-phase-matched (QPM) second harmonic generation (SHG) in MgO-doped stoichiometric lithium tantalate (LiTaO3). The effective nonlinear coefficient was found be 2.4 pm/V by using fourth-order QPM SHG at the fundamental wavelength of 1064 nm. For first-order QPM SHG, the effective value of d33 could be 9.2 pm/V. Using the sensitive higher-order QPM SHG method, we investigated the relationship between the domain duty ratio and the conversion efficiency.

Development of the vascular tissues is a dynamic process that integrates extrinsic and intrinsic factors to control vascular tissue formation throughout the plant life cycle. During vascular tissue formation in Arabidopsis roots, radial and longitudinal signals, including nuclear factors and plant hormones, control the developmental processes involved in the specification, differentiation, and maintenance of the correct cell types. SHR, a GRAS transcription factor, has been known to regulate the specification of the stem cell niche and ground tissue identity in the root meristem in a non-cell-autonomous manner. However, the role of SHR in the root vasculature is relatively overlooked, despite localization of its mRNA and protein in the stele. Here, we investigated the role of SHR in the vascular system of the primary root using a reverse genetic approach and detailed phenotypic analysis. A novel, loss-of-function mutant, shr-6, was isolated in the Columbia background, and vascular patterning was characterized in detail. Our results reveal that shr mutants have developmental defects in both protophloem and protoxylem elements. Our study also suggests that SHR plays a central role in the root vascular system to control patterning processes, possibly regulated by longitudinal and radial signals.
Implications of the Family and Consumer Sciences Curriculum in the USA

This study examined the Family and Consumer Sciences (FCS) National Standards with some examples at the state level, analyzed the previous studies relevant to curriculum implementation in the USA, and explored critical success factors in moving toward the new perspective curriculum in exemplary states. The process, in which the FCS discipline struggled to clarify the identity and image as well as to find the mission and vision, produced the FCS National Standards in 1998 and 2008 in the USA. The FCS National Standards were established to fulfill the mission of the FCS based on a critical science perspective. The previous research on a state level implementation indicated that the majority of FCS state administrators agreed that the National Standards positively influenced curriculum development. The critical success factors in integrating National Standards into local programs included the dissemination of thephilosophical works of Marjorie Brown, the foundation of the FCS curriculum with a critical science perspective, the establishment of National Standards corresponding to the philosophical works and a critical science perspective, the openness of state FCS administrators to educational reform, the construction of an infrastructure to support reform, and the commitment by university professors to develop a teacher training program. The critical success factors identified can be employed as an informative guide for the future development and implementation of the Family and Consumer Sciences curriculum in Korea.

This study compared actual weight with perceived weight, described the prevalence of unhealthy weight control behavior, determined the differences in psychological and personal variables between participants that reported unhealthy weight control behavior and those who did not, and examined the relationship of eating habits to unhealthy weight control behavior for Korean adolescents. The study population consisted of a nationally representative sample of middle and high school students who completed the Fifth Korea Youth Risk Behavior Web-based Survey (KYRBWS): Fifth in 2009. Among the 75,066 participants of KYRBWS, 35,473 (n = 18,851 girls and 16,622 boys) were eligible for a research focused on unhealthy weight control behavior. The results of this research were as follows: First, there were considerable discrepancies (45.1% of girls and 32.8% of boys) between the perceived weight and the actual weight. Second, overall, unhealthy weight control behavior was more prevalent in girls and fasting was the most commonly reported behavior. Third, participants that reported unhealthy weight control behavior scored significantly lower on scaled measures of happiness, health, academic achievement, and economic status; in addition, they scored higher on stress measures. Fourth, girls and boys shared common protective factors of having breakfast and vegetables more often, perceiving their weight as underweight rather than overweight, and having a correct weight conception. Protective factors unique to girls were having lunch and dinner more often. Girls and boys shared common risk factors of the consumption of soda, fast food, instant noodles, and snacks more often, while consumption of fruit more often was a risk factor only for girls. The improvement of protective factors and minimization of risk factors through Home Economics classes (and other classes relevant to health) may mitigate unhealthy weight control behavior of adolescents.
