Radial Arteriovenous Fistula after Coronary Angiography

Introduction: Transradial approach for Coronary angiography has become an increasingly popular technique since it was first introduced in 1989. Because of fewer vascular access complications and a shorter length of hospital stay, use of transradial approachis getting more popularity. We report an extremely rare case of delayed radial arteriovenous fistula formation, 3 months after transradial coronary angiography, which was treated successfully with surgical repair. Case report: A 67-year-old man presented chest pain. He underwent diagnostic coronary angiography via right radial artery, and it was found that he didn’t have coronary artery atherosclerosis. He was discharged one day after the procedure. About 3 months after the day of the diagnostic coronary angiography, he complained of a swishing sensation on the right wrist at the outpatient clinic. An upper extremity angiography revealed arteriovenous fistula between the right radial artery and the adjacent cephalic vein around the right wrist. He was referred to a vascular surgeon and received surgical ligation of the arteriovenous fistula. Discussions: Arteriovenous fistula after transradial approch is an extremely rare complication because of the absence of large veins near the radial artery. Since first noticed in 2005, a few cases were demonstrated to have arteriovenous fistula formation after transradial approch. In this case, upper extremity angiography was performed for diagnosis of AVF formation and surgical repair was done without any complication.

<P><B>Abstract</B></P> <P>Homeobox (HB) genes encode transcription factors that play important regulatory roles in plant growth, development, and plant responses to abiotic stresses, including water deficit. In pepper plant, little is known about HB genes and their functions. Here, we identified 95 putative HB genes within the pepper genome that were classified into 14 classes using phylogenetic analysis. Of the genes responsive to abiotic stresses and ABA treatment, we selected HD ZIP I gene <I>CA08g08650</I> as a dehydration-responsive homeobox 1 (DRHB1) gene. <I>CaDRHB1</I>-silenced pepper plants exhibited a dehydration-sensitive phenotype, characterized by decreased stomatal closure and high levels of transpirational water loss. <I>CaDRHB1</I>-overexpressing transgenic Arabidopsis plants showed a contrasting opposite phenotype to that of <I>CaDRHB1</I>-silenced pepper plants. Outside of the HD-ZIP domain, CaDRHB1 has a serine-rich region (amino acids 8–32) and an AHA motif (amino acids 216–243) which negatively and positively regulate transactivation activity in yeast, respectively. In addition, amino acids 139–190 interact with pepper type 2C protein phosphatases which act as a negative regulator of ABA signaling. Taken together, our findings suggest that CaDRHB1 acts as a positive factor for dehydration tolerance via regulation of ABA-mediated stomatal closure. Moreover, conserved domains may be involved in regulating CaDRHB1 function.</P> <P><B>Highlights</B></P> <P> <UL> <LI> We found 95 homeobox-containing genes in pepper genome. </LI> <LI> HD-ZIPI gene CaDRHB1 shows the highest induction in response to dehydration. </LI> <LI> CaDRHB1 regulates dehydration tolerance via ABA-mediated stomatal movement. </LI> <LI> Serine-rich region and AHA motif in CaDRHB1 control its transcriptional activity. </LI> </UL> </P>

The effect of tensile hold time on the creep-fatigue interaction in AISI 316 stainless steel was investigated. To study the fatigue characteristics of the material, strain controlled low cycle fatigue(LCF) tests were carried out under the continuous triangular waveshape with two different total strain ranges of 1.0% and 2.0%. To study the creep-fatigue interaction, 5 min.,<br/> 10 min., and 30 min. of tensile hold times were applied to the continuous triangular waveshape with the same two total strain ranges. The creep-fatigue life was found to be the longest when the 5 min. tensile hold time was applied and was the shortest when the 30 min. tensile hold time was applied. The cause for the shortest creep-fatigue life under the 30 min. tensile hold time is believed to be the effect of the increased creep damage per cycle as the hold time increases. Under the total strain range of 1%, significant reduction in life was observed as the tensile hold time was increased compared to the result of 2% total strain range. The theoretical prediction obtained by the Modified Coffin-Manson method and the experimental results of creep-fatigue life were found to be in resonable agreement with each other.<br/>

<P><B>Abstract</B></P><P>This study aims to evaluate inter‐annual and inter‐plot variation of wood biomass production (WBP) and to investigate the relationships of the WBP variations with several biotic and abiotic characteristics at a deciduous forest in complex terrain, the Gwangneung Experimental Forest, Korea. Based on field survey in the plot‐scale study area, WBP during 1991–2004 was estimated by a dendrochronological method. Our field data indicated that the inter‐annual variation of WBP was closely related to the seasonal climate of both winter air temperature and spring precipitation. The inter‐plot variation of WBP was highly associated with basal area, biomass, and frequency of <I>Quercus</I> spp. in the plots, and correlations of the inter‐plot variation with the stand characteristics of the specific species were stronger than those with slope and soil water content. Our results suggest that the annual fluctuation of forest productivity is primarily governed by severe climate in a season of the year, and the spatial distribution of a dominant species largely represent plot variation in the productivity. Our findings contribute to an enhanced understanding of climatic effects on the annual variability of forest productivity and the spatial heterogeneity of the productivity, which are extensively concerned with forested ecosystems of Korea.</P>
증례 : 순환기 ; 관상동맥조영술 후 발생한 동정맥루 1예

67세 남자 환자에서 경요골동맥 접근을 통해 CAG를 시행하고 3개월 후 시술을 시행한 손목의 박동을 주소로 외래를 방문하였다. 국소적인 압박을 통한 치료와 1회의 동정맥루결찰술 이후에도 잡음이 지속되어 상지 동맥조영술을 통해 남아있는 동정맥루를 확인하였고 외과적 동정맥루결찰술을 재시행하여 호전되었던 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. A 67-year-old man underwent coronary angiography using a transradial approach. Three months after coronary angiography, the patient complained of a thrill detected in his right wrist. Localized compression was performed in the assumption of arteriovenous fistula formation. Since thrill was still detected after localized compression, surgical revision of an arteriovenous fistula was performed. Six days later, radial bruit was still reported. It was decided to perform upper extremity angiography. Upper extremity angiography revealed the remaining arteriovenous fistula from radial artery to cephalic vein, and surgical revision was performed again. This appears to be a very unusual complication related to the transradial approach for coronary angiography. (Korean J Med 2016;90:231-233)
