      • 3D Asthenopia in Horizontal Deviation

        <P><I>Purpose:</I> This study was conducted to investigate the asthenopic symptoms in patients with exotropia and esotropia while watching stereoscopic 3D (S3D) television (TV).</P><P><I>Methods:</I> A total 77 subjects who more than 9 years of age were enrolled in this study. We divided them into three groups; Thirty-four patients with exodeviation (Exo group), 11 patients with esodeviation (Eso group) and 32 volunteers with normal binocular vision (control group). The S3D images were shown to all patients with S3D high-definition TV for a period of 20 min. Best corrected visual acuity, refractive errors, angle of strabismus, stereopsis test and history of strabismus surgery, were evaluated. After watching S3D TV for 20 min, a survey of subjective symptoms was conducted with a questionnaire to evaluate the degree of S3D perception and asthenopic symptoms such as headache, dizziness and ocular fatigue while watching 3D TV.</P><P><I>Results:</I> The mean amounts of deviation in the Exo group and Eso group were 11.2 PD and 7.73PD, respectively. Mean stereoacuity was 102.7 arc sec in the the Exo group and 1389.1 arc sec in the Eso group. In the control group, it was 41.9 arc sec. Twenty-nine patients in the Exo group showed excellent stereopsis (≤60 arc sec at near), but all 11 subjects of the Eso group showed 140 arc sec or worse and showed more decreased 3D perception than the Exo and the control group (<I>p</I> < 0.001, Kruskal-Wallis test). The Exo group reported more eye fatigue (<I>p</I> < 0.001, Kruskal-Wallis test) than the Eso and the control group. However, the scores of ocular fatigue in the patients who had undergone corrective surgery were less than in the patients who had not in the Exo group (<I>p</I> < 0.001, Kruskal-Wallis test) and the amount of exodeviation was not correlated with the asthenopic symptoms (dizziness, <I>r</I> = 0.034, <I>p</I> = 0.33; headache, <I>r</I> = 0.320, <I>p</I> = 0.119; eye fatigue, <I>r</I> = 0.135, <I>p</I> = 0.519, Spearman rank correlation test, respectively).</P><P><I>Conclusion:</I> Symptoms of 3D asthenopia were related to the presence of exodeviation but not to esodeviation. This may indicate that S3D symptoms are closely related to the convergence demand.</P>

        Chloroform is harmful volatile organics and representatives of Trihalomethane (THM). Well-known removal methods of Chloroform are photo oxidation or OH radical oxidation. Plasma on water surface at slightly vacuum condition (45 torr) can produce OH radical and it will help chloroform removal. 81.5% of chloroform is removed by vacuum and plasma in 10 min. Plasma can totally oxidize it till 2.8% and partially oxidize chloroform up to 18.5%. Water-surface plasma is good method to remove chloroform in short time.

      • Differential Effects of Pioglitazone in the Hippocampal CA1 Region Following Transient Forebrain Ischemia in Low- and High-Fat Diet-Fed Gerbils.

        <P>In the present study, we investigated the effects of pioglitazone (PGZ) in the hippocampal CA1 region of low- or high-fat diet (LFD or HFD) fed gerbils after transient forebrain ischemia. After 8 weeks of LFD or HFD feeding, PGZ (30 mg/kg) was intraperitoneally administered to the gerbils, following which ischemia was induced by occlusion of the bilateral common carotid arteries for 5 min. Administration of PGZ significantly reduced the ischemia-induced hyperactivity 1 day after ischemia/reperfusion in both LFD- and HFD-fed gerbils. At 4 days after ischemia/reperfusion, the neurons were significantly reduced and microglial activation was observed in the hippocampal CA1 region in LFD- and HFD-fed gerbils. The microglial activation was more prominent in the HFD-fed gerbils compared to the LFD-fed gerbils. Administration of PGZ ameliorated ischemia-induced neuronal death and microglial activation in the hippocampal CA1 region 4 days after ischemia/reperfusion in the LFD-fed gerbils, but not in the HFD-gerbils. At 6 h after ischemia/reperfusion, tumor necrosis factor-α (TNF-α) and interlukin-1β (IL-1β) levels were significantly increased in the hippocampal homogenates of LFD-fed group compared to control group, and HFD feeding further increased TNF-α and IL-1β levels. PGZ treatment significantly ameliorated the increase of TNF-α and IL-1β levels in LFD-fed gerbils, not in the HFD-fed gerbils. At 12 h after ischemia/reperfusion, superoxide dismutase (SOD) and malondialdehyde (MDA) levels in hippocampal homogenates were significantly increased in the LFD-fed group compared to the control group, and HFD feeding significantly showed relatively reduction in SOD activity and increase in MDA level. PGZ administration significantly reduced the increase in MDA levels 12 h after ischemia/reperfusion in the LFD-fed gerbils, but not in the HFD-fed gerbils. These results suggest that PGZ ameliorates the neuronal damage induced by ischemia by maintaining the TNF-α, IL-1β, SOD and MDA levels in LFD-fed gerbils. In addition, HFD feeding affects the modulation of these parameters in the hippocampus after transient forebrain ischemia.</P>

        가토에서 Vecuronium bromide 의 용량에 따른 근육이완 효과

        The introduction of muscle relaxants was an epoch making event in the development of clinical anesthesia and their use has hecame essential to anesthesia practice. Vecuronium, one of the newest muscle relaxants, many advantages; it is .apid acting, non-accumulating, has minimals side reactions and is promptly antagonized by anticholinesterases. However, its pharmacological effect, like other conventional muscle relaxants depends upon the dose and is influenced br many factors including body temperature, blood flow, interaction with other drugs, antibiotics, diuretics and so forth, To obtain a dose·response relationship which is important for clinical use, the effect of the vecuronium in various doses on the neuromuscular block in rabbits was investigated. The results were as follows. 1) In the vecuronium 0.025 mg/kg group, maximal twitch depression was varied between 75 to 92% with a mean value of 84,9%. The spontaneous recovery index was 141.0 seconds (2.35 min). 2) In the vecuranium 7.75 mg/kg group, thmean maximum twitch depression and spontaneous recovery index were 98.4% and 218.6 seconds (3.64 min.) respectively. 3) In the vecuronium 7.1 mg/kg group, maximum twitch depression was 100% and the spontaneous recovery index was markedly prolonged to 627.8 seconds (10.35 min).

        Purification and Characterization of Helicobacter pylori ${\gamma}$-Glutamyltranspeptidase

        Gamma-glutamyltranspeptidase (GGT) was purified to electrophoretic homogeneity from the cell extract of H. pylori. The purified enzyme consisted of heavy and light subunits with molecular weights of 38 kDa and 21 kDa, respectively. N-terminal amino acid sequence of heavy and light subunits revealed that H. pylori GGT was processed into 3 parts for a signal peptide of 27 amino acid residues, a heavy subunit of 352 residues, and a light subunit of 188 residues during translation. The reaction rate for hydrolysis of ${\gamma}$-GpNA was 84.4 ${\mu}mol/min$ per milligram of protein, and that for the ${\gamma}$-glutamyl transfer from ${\gamma}$-GpNA to gly-gly was 23.8 ${\mu}mol/min$ per milligram of protein. The apparent Km values of H. pylori GGT for ${\gamma}$-glutamyl compounds were on the order of $10^{-3}$ to $10^{-4}$ M and those for acceptor peptides and amino acids were on the order of $10^{-1}$ to $10^{-2}$ M. The GGT protein kept approximately 80% of the initial enzymatic activity on incubation at $60^{\circ}C$ for 15 min. The optimum temperature and pH for reactions of both hydrolysis and transpeptidation were $40^{\circ}C$ and 9.0, respectively. The transpeptidation and hydrolysis reactions catalyzed by H. pylori GGT were strongly inhibited by L-Gln and moderately inhibited by L-Ala, L-Ser, ${\beta}$-chloro-L-Ala, and L-Glu. These results demonstrated that the biochemical properties of H. pylori GGT are different from those of other bacterial GGTs. Further, H. pylori GGT might degrade glutathione in the gastric mucous layer of humans if the enzyme could be secreted in the bacterial niches.

        임상증례중심 성인 전문적 심장구조술 교육과정에 대한 고찰

        Background: To describe the first experience of case-based advanced cardiac life support(ACLS) course in Korea. Methods: We have given case-based ACLS course to 13 nurses(6 emergency nurses, 4 coronary care unit nurses, 3 professors) and 17 physicians(4 emergency physicians, 12 emergency residents, 1 intern). We performed the case-based ACLS course according to 1992 American Heart Association guidelines and recommendations for advanced cardiac life support by american ACLS instructors(1 pulmonologist, 4 critical care nurses). We performed final theoretical written test and 2 times written survey (immediate and 100th day after the course) about the course. Results: On final written test, all practitioners answered at least 70% of the questions correctly. There was no significant difference between nurses and physicians(86.2±3.6 of physicians and 82.5±6.8 of nurses, p=0.06). 90% of participants considered that case-based advanced cardiac life support was acceptable. 100th day after the course, 93% of participants answered that ACLS course have been helpful on his/her job and also want retraining of ACLS course. 70% of participants considered that ideal ACLS training committee in Korea is the Korean Society of Emergency Medicine. Conclusion: Case-based ACLS course is a useful educational method for physicians and nurses in Korea. In the future, we should organize Korean resuscitation committee and then make guidelines for ACLS, and then continuously educate physicians and nurses.

      • 최대근력부하와 폭발적 근력부하에 의한 신경-근피로와 그 회복과정 비교 분석

        The result which was gained from this study is as follows. From the conclusion of this study it was confirmed that resistant motion at high level strength brings about rise in density of lactate in blood at the same time decline of muscular power and generates acute fatigue of nerve-muscle accompanied by decline in electrical activity of muscle. On the other hand explosive loading produced fatigue at body center and decline in nervine-muscular activity particularly for men, In case of women explosive load did not produce fatigue in the same level as men and they were also quicker in recovery after exercise. Based on 2 types of loading method used in this study there was clearly difference in the process of restoration from resistant motion. For greater decline in muscular power and electromyogram activity more longer period of recovery was require.

      • Epstein-Barr 바이러스 형질전환법을 이용한 파상풍 톡소이드에 대한 사람 단세포군 항체의 생산

        For production of tetanus toxoid(TT) specific human monoclonal antibodies, anti-TT antibody secreting peripheral B lymphocytes were separated by rosetting with TT-coated SRBC. And, B lymphoblastoid cell line(BLCL) was established by Epstein-Barr Virus(EBV) transformation methods. Stable BLCLs were established after three times of cloning by limiting dilution. To establish the maximal antibody producing condition, several environmental factors were tested. The optimal condition for maximum antibody prodiction was 4 day culture of 1 x 105cells/ml concentration in RPMI 1640 complete culture media supplemented with 20% fetal calf serum, and the maximum concentration of secreted antibody was about 900ng/ml. The antibody production of BLCL was decreased during long-term culture after establishment of cell line, but antibody production was maintained by repeated cloning by limiting dilution.
