The Role of Ballooning in Patients with Post-tuberculosis Bronchial Stenosis

Background: In order to access the role of ballooning in patients with post-tuberculosis bronchial stenosis (PTBS), medical records of patients who underwent the procedure were reviewed. Methods: Twenty-nine PTBS patients underwent balloon dilatation between May 1999 and November 2000 at Samsung Medical Center. The median age was 28 (range 16~62 year) and most patients were female (n=27, 93%). The mean number of ballooning procedures was 2.4 (range 1~8) and the interval between ballooning procedures was 76.2±69.7 days. Results: In general, the FEV1 was improved after ballooning (from 66.2.±11.9% predicted to 73.5±13.0% predicted, p=0.0004). Among 29 patients who underwent ballooning, a clinically successful outcome was observed in 16 patients (55%). Comparison between the successful and unsuccessful groups showed that favorable factors for a successful outcome were a higher pre-ballooning FEV1 (71.1±8.1 vs. 60.2±13.3% predicted), higher post-ballooning FEV1 (89.2±7.8 vs. 63.4±9.2% predicted) and absence of left upper lobe collapse. The clinical outcome was unsuccessful in all eight patients with a pre-ballooning FEV1 ≤57% of predicted or with complete left upper lobe collapse. Conclusion: In conclusion, ballooning appears to be helpful in patients with PTBS, especially when the pre-ballooning FEV1 >57% of predicted and there is no complete left upper lobe lung collapse.

Tubuolointerstitial inflammation and tubular injury account for most types of glomerulonephritis. The injury is characterized by an infiltration of mononuclear cells with atrophy and dilation of tubules and increased deposition of collagen in the interstitium. Despite the fact that the degree of tubulointerstitial injury in glomerular diseases may be the best predictor of overall outcome, the pathogenic mechanism by which the tubular injury develops remains unknown. Osteopontin, a highly acidic, phosphorylated, secreted glycoprotein, is up-regulated in renal cortex in many experimental models of tubulointerstitial fibrosis. In this study, we examined the expression of osteopontin in tubulointerstitium in experimental renal failure mouse, FGS/KIST. Mice were assigned three groups and sacrificed at 1 month, 2 months, and 3 months, in each. Proteinuria, GFR, the degree of tubulointerstitial inflammation, tubular atrophy, glomerulosclerosis and osteopontin expression were measured. Three-month-old group showed severely decreased GFR and marked tubulointerstitial inflammation and glomerulosclerosis compared with other groups. The expression of osteopontin increased with the severity of tubulointerstitial injury. These data suggest that osteopontin may act as a chemotactic or adhesive factor in the recruitment of the monocyte/macrophages and have a role in the pathogenesis of the tubulointerstitial injury.

The discharge started at lower voltage for the cathode corona than the anode corona. In contract Spark occurred higher voltage for the cathode corona. The anode corona showed higher NO to NO₂ conversion rate. The cone-hole type showed more stable discharge than the cone-plate type. As O₂ concentration increases, the NO to NO₂ conversion increases. It can be seen that the onset of combustion occurs at approximately 280℃ when using NO₂, while temperatures of around 480℃ are required to combust soot using 0₂
In vitro에서 미색류 껍질로부터 추출한 콘드로이틴황산의 기능성 화장품 소재로서의 가능성

CCD 1108Sk 를 이용한 세포 활성을 조사한 결과 Sigma사 제품(Bovine Trachea에서 얻어진 콘드로이틴황산)은 80 ㎍/mL에서 높은 활성을 보였으나, 미더덕 및 멍게로부터 추출한 콘드로이틴황산은 400~600 ㎍/mL에서 높은 활성을 보였다. 이러한 활성의 차이는 순도의 차이에 기인하거나 서로 다른 구조에 기인한다고 여겨졌다. 그러나 MDD F1은 1,000㎍/mL에서 활성이 증가하는 모습이 보였지만 MDD F2는 미약하게 증가하는 모습을 보여 fraction들간에 차이가 있음을 보였다. A 431 주화세포를 이용한 실험에서는 Sigma사 제품과 미더덕과 멍게에서 추출한 crude 콘드로이틴황산의 경우는 CCD 1108Sk에서 보여준 결과와 비슷하였으나, MDD F1과 F2는 모두 600~1,000 ㎍/mL 사이에서 최고의 활성을 보였다. Collagen 합성에 있어서는 bovine trachea, MDD F1과 F2는 조사 범위에서는 특이 활성을 보이지 않았다. 그러나 멍게에서 추출한 콘드로이틴황산의 경우는 100 ㎍/mL에서, 미더덕에서 추출한 콘드로이틴황산의 경우는 50 ㎍/mL에서 CCD 1108Sk 세포에서 각각 최고의 활성을 보였다. 그러나, A 431 세포에서는 검사한 모든 물질에 대하여 특이 반응을 확인할 수 없었다. 자외선 손상에 대한 회복능 조사는 세포 상층액과 세포 자체가 생성한 LDH를 조사한 결과, 멍게와 미더덕에서 추출한 콘드로이틴황산의 경우는 400 ㎍/mL에서 CCD 1108Sk 세포의 상층액에서 높은 LDH를 보였으나, 다른 물질의 경우 조사범위에서는 반응을 보이지 않았고, 세포 자체에서도 LDH는 조사 물질간 큰 차이를 나타내지는 않았다. 멍게와 미더덕에서 추출한 crude 콘드로이틴황산의 경우는 400 ㎍/mL에서 최고의 활성을 보였다. 전체적으로 멍게에서 추출한 콘드로이틴황산이 미더덕에서 추출한 콘드로이틴황산의 경우보다 세포활성이 높게 나타났다. 그렇지만 두 해양생물로부터 추출한 콘드로이틴황산은 화장품 원료로서의 사용이 가능함이 판명되었다. With the aim of using a cosmetic material, chondroitin sulfates extracted from midduck tunics (Styela clava) and munggae tunics (Halocynthia roretzi) were examined in vitro with two cell lines for cell toxicity, collagen synthesis, cell growth and recovery ability after U.V. irradiation. Cell toxicity test with A 431 and CCD 1108Sk was able to observe high activity between 400 and 600 ㎍/m while standard chondroitin sulfate (CS) purchased from Sigma was showed at 80 ㎍/mL. Even fraction 1 and 2 collected from chondroitin sulfates originated from midduck appeared having the highest activity between 600 and 1000 ㎍/mL, but slightly lower compared to crude chondroitin sulfates from both mideduck and munggae. In cell growth examination, it was not able to find significant differences between chondroitin sulfates used. Both crude chondroitin sulfates were exhibited the highest activity for two cell lines except that of mideduck which was showed activity for CCD 1108Sk. CS, fraction 1 and 2 from midduck were not able to demonstrate a significant activity in collagen synthesis. On the contrary, crude chondroitin sulfates from both munggae and midduck were showed the highest activity at 100 and 50 ㎍/mL with only CCD 1108Sk. The recovery ability after U.V. irradiation with crude chondroitin sulfates from both munggae and midduck were showed high activity at 400 ㎍/mL with CCD 1108Sk and A 431. But there were no activity observed in fractions examined. As a consequence, the crude chondroitin sulfates from both munggae and midduck might not only be available as a cosmetic material but also useful for increasing some activity by blending properly.

This paper describes controller development for a dynamic voltage compensator using a shunt diode converter and series inverter. The control system was designed using 1/4 period integrator and vector relationship between the supply voltage and load voltage. A simulation model and scaled hardware model were developed for analyzing performance of the controller and the whole system Both results confirm that the dynamic compensator can restore the load voltage under the fault of the distribution system
