Purpose: We prospectively conducted a multi-center,open-label, randomized phase II trial to compare the efficacy and safety of docetaxel plus cisplatin (DC) and etoposideplus cisplatin (EC) for treating advanced stage non-small cell lung cancer (NSCLC).Materials and Methods: Seventy-eight previously untreated patients with locally advanced, recurrent or metastatic NSCLC were enrolled in this study. The patientsreceived cisplatin 75 mg/m2 on day 1 and either docetaxel 75 mg/m2 on day 1 or etoposide 100 mg/m2 on days 1 to 3 in the DC or EC arm, respectively, every 3 weeks.Results: The objective response rate was 39.4% (15/38) and 18.4% (7/38) (p=0.023) in the DC and EC arms, respectively. The median time to progression (TTP) was5.9 and 2.7 months (p=0.119), and the overall survival was 12.1 and 8.7 months (p=0.168) in the DC and EC arms, respectively. The prognostic factors for longer survival were an earlier disease stage (stage III, p=0.0095), the responders to DC (p=0.0174) and the adenocarcinoma histology (p=0.0454). The grades 3 and 4 toxicities were similar in both arms, with more febrile neutropenia (7.9% vs. 0%) and fatigue (7.9% vs. 0%) being noted in the DC arm.Conclusion: DC offered a superior overall response rate than does EC, along with tolerable toxicity profiles, although the DC drug combination did not show significantlyimproved survival and TTP.

CONTENTS<BR> (1) RESEARCH OBJECTIVES<BR> The officetel is the residential form which is chosen as a convenience place for commuting to work or school by single household or family members with small scale. The goal of study is to analysis the status of officetels in Busan and to explore officetel residents" evalutaion to grasp the problems about utilities and facilities and managements of officetel.<BR> (2) RESEARCH METHOD<BR> It is used by depth study through real interview method and OLB (ordinary least squares) analysis to achieve the purpose of this study.<BR> (3) RESEARCH RESULTS<BR> First, as a rule the residents of officetel showed high satisfaction with their residence. However there were cases that the tenants can be confused on accurate legal differences of officetel.<BR> Second, examining the residents! cognition about inner and outer space of officetel like bedroom, kitchen and bathroom, they are generally satisfied with the dimension and the structure of each room. But they expressed dissatisfaction about ventilation and circulation of the indoor air and about noise of outside.<BR> Third, in case of facilities and management, while most of the residents are satisfied with the secure system and maintenance of facilities, they pointed out the inconvenience in using parking lot and the matter of high management expenses.<BR> Forth, in case of the officetel"s location, most of the residents emphasized to be located within station sphere because it is the proximity to transportation facilities which is the most important factor in selecting officetel.<BR> 2. RESULTS<BR> Although the officetels are controled legally, they are classified into business facilities by the architecture law. Most of the users recognize the officetel as new form of the residence and use it as the sort of residence. Thus it is not easy to solve the officetels problem through compulsive regulations about change of use.
Endovascular Aneurysm Repair for Abdominal Aortic Aneurysm: A Comprehensive Review

Abdominal aortic aneurysm (AAA) can be defined as an abnormal, progressive dilatation of the abdominal aorta, carrying a substantial risk for fatal aneurysmal rupture. Endovascular aneurysmal repair (EVAR) for AAA is a minimally invasive endovascular procedure that involves the placement of a bifurcated or tubular stent-graft over the AAA to exclude the aneurysm from arterial circulation. In contrast to open surgical repair, EVAR only requires a stab incision, shorter procedure time, and early recovery. Although EVAR seems to be an attractive solution with many advantages for AAA repair, there are detailed requirements and many important aspects should be understood before the procedure. In this comprehensive review, fundamental information regarding AAA and EVAR is presented.

<P>Although the Cold-Heat Pattern is the most important diagnostic factor in Traditional Korean Medicine (TKM), its relationship to body temperature and subjective temperature has not been clearly revealed. In this study, based on clinical data from 551 patients, we classified patients treated with herbal medicines into a Cold-prescription group (CG) and a Heat-prescription group (HG), and we compared the ordinary symptoms between the two groups. Subjective body temperature was higher in the HG than in the CG (OR = 1.68, <I>p</I> < 0.01) and digestive ability was better in the HG than in the CG (expert's questionnaire, OR = 1.91, <I>p</I> < 0.001). However, objectively measured body temperature did not show any significant difference between the HG and CG in both gender groups (<I>p</I> = 0.383 and 0.181 for males and females, resp.). Our study suggests that the subjective body temperature and digestive ability may be the principal diagnostic elements of Cold-Heat Pattern identification by Korean Medicine Doctors. These findings may contribute to the investigation of an objective method to measure the Cold-Heat Pattern. </P>

저자들은 13년 전 피부의 악성흑색종을 완전절제한 과거력이 있는 환자에서 복부 팽만감을 주소로 내원하여 실시한 복부 전산화 단층 촬영상 복막강내 암의 파종소견이 확인되었고, 원발부위를 찾기위해 시행한 내시경에서 전이성 악성흑색종으로 진단된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Malignant melanoma may spread through the lymphatic channels or via the bloodstream. The lung, liver, bone and brain are common sites of hematogenous metastases. Malignant melanoma that has recurred to the stomach or peritoneum is not common. An 83-year old male patient was admitted to the Department of Oncology due to abdominal distension; 13 years ago, the patient had previously received a complete surgical resection for malignant melanoma of the right heel. According to a computed tomography (CT) scan of the abdomen, peritoneal carcinomatosis was suggested. Under a gastrofiberoscopic examination, two small sized polyps with central umbilication were found on the midbody of the stomach. The gastric polyps were histopathologically confirmed as metastatic melanoma. We report a rare case of metastatic malignant melanoma that has recurred to the stomach and peritoneum 13years after its initial diagnosis. (Korean J Med 72:213-216, 2007)

Pulmonary capillary hemangiomatosis (PCH) is a rare disease of unknown etiology that is characterized by nodules composed of infiltrating capillary blood vessels. Herein, we describe a case of a PCH-like lesion that was detected by chest computed tomography. Transthoracic needle aspiration resulted in life-threatening hemorrhage. The patient was followed for seven years. He remained in good health and a follow up image showed little interval change.
