Evaluation of Humic Acid and Tannic Acid Fouling in Graphene Oxide-Coated Ultrafiltration Membranes

<P>Three commercially available ultrafiltration (UF) membranes (poly(ether sulfone), PES) that have nominal molecular weight cut-offs (5, 10, and 30 kDa) were coated with graphene oxide (GO) nanosheets. Field-emission scanning electron microscopy, Fourier-transform infrared spectroscopy, confocal laser scanning microscopy, water contact angle measurements, and X-ray photoelectron spectroscopy were employed to determine the changed physicochemical properties of the membranes after GO coating. The water permeability and single-solute rejection of GO-coated (GO(c)) membranes for humic acid (HA) molecules were significantly higher by approximately 15% and S5%, respectively, compared to those of pristine UF membranes. However, the GOc membranes for single-solute tannic acid (TA) rejection showed similar trends of higher flux decline versus pristine PES membranes, because the relatively smaller TA molecules were readily adsorbed onto the membrane pores. When the mixed-solute of HA and TA rejection tests were performed, in particular, the adsorbed small TA molecules resulted in irreversible membrane fouling due to cake formation and membrane pore blocking on the membrane surface for the HA molecules. Although both membranes showed significantly higher flux declines for small molecules rejection, the GOc membranes showed better performance than the pristine UF membranes in terms of the rejection of various mixed-solute molecules, due to higher membrane recovery and antifouling capabilities.</P>

Background: The initiation of sodium-glucose cotransporter-2 inhibitors (SGLT2i) typically leads to a reversible initial dip in estimated glomerular filtration rate (eGFR). The implications of this phenomenon on clinical outcomes are not well-defined.Methods: We searched MEDLINE, Embase, and Cochrane Library from inception to March 23, 2023 to identify randomized controlled trials and cohort studies comparing kidney and cardiovascular outcomes in patients with and without initial eGFR dip after initiating SGLT2i. Pooled estimates were calculated using random-effect meta-analysis.Results: We included seven studies in our analysis, which revealed that an initial eGFR dip following the initiation of SGLT2i was associated with less annual eGFR decline (mean difference, 0.64; 95% confidence interval [CI], 0.437 to 0.843) regardless of baseline eGFR. The risk of major adverse kidney events was similar between the non-dipping and dipping groups but reduced in patients with a ≤10% eGFR dip (hazard ratio [HR], 0.915; 95% CI, 0.865 to 0.967). No significant differences were observed in the composite of hospitalized heart failure and cardiovascular death (HR, 0.824; 95% CI, 0.633 to 1.074), hospitalized heart failure (HR, 1.059; 95% CI, 0.574 to 1.952), or all-cause mortality (HR, 0.83; 95% CI, 0.589 to 1.170). The risk of serious adverse events (AEs), discontinuation of SGLT2i due to AEs, kidney-related AEs, and volume depletion were similar between the two groups. Patients with >10% eGFR dip had increased risk of hyperkalemia compared to the non-dipping group.Conclusion: Initial eGFR dip after initiating SGLT2i might be associated with less annual eGFR decline. There were no significant disparities in the risks of adverse cardiovascular outcomes between the dipping and non-dipping groups.

Palliative Care for Adult Patients Undergoing Hemodialysis in Asia: Challenges and Opportunities

This article underscores the importance of integrating comprehensive palliative care for noncancer patients who are undergoing hemodialysis, with an emphasis on the aging populations in Asian nations such as Taiwan, Japan, the Republic of Korea, and China. As the global demographic landscape shifts towards an aging society and healthcare continues to advance, a marked increase has been observed in patients undergoing hemodialysis who require palliative care. This necessitates an immediate paradigm shift to incorporate this care, addressing the intricate physical, psychosocial, and spiritual challenges faced by these individuals and their families. Numerous challenges impede the provision of effective palliative care, including difficulties in prognosis, delayed referrals, cultural misconceptions, lack of clinician confidence, and insufficient collaboration among healthcare professionals. The article proposes potential solutions, such as targeted training for clinicians, the use of telemedicine to facilitate shared decision-making, and the introduction of time-limited trials for dialysis to overcome these obstacles. The integration of palliative care into routine renal treatment and the promotion of transparent communication among healthcare professionals represent key strategies to enhance the quality of life and end-of-life care for people on hemodialysis. By embracing innovative strategies and fostering collaboration, healthcare providers can deliver more patient-centered, holistic care that meets the complex needs of seriously ill patients within an aging population undergoing hemodialysis.
조각자 추출물의 항산화 활성 및 alpha-glucosidase 저해 활성

본 연구는 항산화 활성 및 항당뇨 효과가 있는 조각자 추출물인 GSM (methanol 추출물)과 GSE (ethanol 추출물)에 대한 연구이다. 추출 용매에 대한 페놀 함량을 측정한 결과 100 g 당 GSM에서는 1.12 g이며, GSE에서는 0.6 g으로 GSM에서 함량이 높은 것으로 나타났다. DPPH 라디칼 소거능의 경우 최대 농도 1,000 ㎍/ml에서 확인한 결과 GSM에서는 68.8% 그리고 GSE에서는 70.4%로 두 종류 추출물에서 높은 결과를 확인할 수 있었다. DNA 손상 정도를 확인한 comet assay에서도 200 μM H₂O₂에 의해 나타나는 DNA 손상에 대하여 GSM과 GSE는 DNA를 보호하는 활성이 있었다. 항당뇨 효과를 알아보는 α-glucosidase 억제 실험에서 GSM은 17.9%, GSE는 10.3%이었으며, 처리시간을 증가하였을 경우에는 그 억제 정도가 GSM의 경우 25%이며, GSE의 경우 20%로 증가하여 항당뇨 효과가 있다는 것을 확인할 수 있었다. 이 결과로부터 당뇨병 치료에 대한 새로운 소재로써 조각자 추출물의 가능성을 확인하였다. This study was performed to investigate the physiological activities of stings of Gleditsia sinensis extracts. Antioxidant activity was evaluated by measuring total phenolic contents (TPC), comet assay, and 2.2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging activity (RSA). Anti-diabetic activity was measured by inhibition activities on α-glucosidase. Stings of Gleditsia sinensis extracts were prepared by extracting them with methanol and ethanol. The methanolic extracts showed the highest phenol content (1.12 g/100 g gallic acid equivalents). The α-glucosidase inhibitory activity of methanol extracts were 17.9% higher, and that of ethanol extracts were 10.3% higher at a concentration of 1 ㎎/ml. These results indicate that stings of Gleditsia sinensis might be potential candidates as antioxidant and anti-diabetic agents.

<P>LaGaO3:Er3+, Yb3+ powder with different Er3+ contents (0.01 similar to 0.10 mol) was synthesized by a conventional solid-state reaction. The structure, morphology and photoluminescence properties of the as-prepared phosphors were characterized by X-ray diffraction (XRD), scanning electron microscopy (SEM), and fluorescence spectrophotometer techniques, respectively. The photoluminescence (PL) emissions based on the green emission near 522 and 544 nm were observed and the highest emission intensity occurred for the sample LaGaO3:Yb-0.15, Er-0.07. The green and red up-conversion emissions were observed in Er3+, Yb3+ co-doped LaGaO3 phosphors under the excitation of 980 nm laser diode.</P>
