        증례 : 감염 ; 결핵성 임파선염 환자에서 Rifampin 재투여로 인한 치명적 과민성 반응 1예

        저자들은 rifampin에 과민 반응이 있는 환자가 rifampin에 다시 노출된 후, 아나필락시스 및 횡문근 융해증, 급성 신부전, 파종성 혈관내응고증후군이 병별하여 결국 사망에 이르렀던 예를 경험하여 문헌고찰과 함께 보고하는 바이다. A 44-year-old patient who had been diagnosed with tuberculous Lymphadenopathy came to our hospital with a rash. He began to take antituberculous medications, but the next day, a rash appeared and persisted for one week. When he came to the hospital, he had already stopped the medication by his own decision. We stopped all drugs and reintroduced one drug at a time. After re-administration of rifampin, anaphylactic shock and subsequent acute renal failure and rhabdomyolysis occurred. Intensive care was performed, but the patient died 60 h after the first hypersensitive reaction that occurred due to rifampin. Anaphylaxis and subsequent rhabdomyolysis induced by rifampin is an extremely rare event. It is necessary to initially prescribe low dose rifampin in cases of re-administration.         수중재활운동이 편마비 아동의 대근육 운동기능에 미치는 효과

        The purpose of this study was to examine the effects on the gross motor skills of the children with hemiplegia by aquatic rehabilitation exercise. 12 children with hemiplegia were selected as subjects of this study. They were divided into two groups; one group was exercise group(n=6), and the other group was control group (n=6). The results of this study were as follows; 1. Aquatic rehabilitation exercise showed effect on sitting trunk flection of children with hemiplegia. 2. Aquatic rehabilitation exercise showed effect on standing long jump, shuttle run change of body position, sit-up of children with hemiplegia. 3. Aquatic rehabilitation exercise showed effect on opened-eyes foot balance, closed- eyes foot balance of children with hemiplegia. 4. Aquatic rehabilitation exercise showed effect on movement skill test battery`s total score of children with hemiplegia. In conclusion, aquatic rehabilitation exercise has a positive effect on the body flexibility, muscle strength, agility and balance which is contribute to improve of physical fitness and health. Therefore, further studies were suggested that various and specific measures for the practical application of this program were to be applied.

        Phenomenological and Microscopic Analysis of Elastic Scattering Reactions: 9Be+27Al New Results

        In this study, elastic scattering of the 9Be+27Al system has been re-analyzed by using phenomenologicalmodel (WS) and microscopic double-folding model (DF) potentials within the frameworkof the optical model at different incident energies. In the phenomenological and double-foldingcalculations, we have used the new parameters to explain the 9Be+27Al reaction. The real potentialsin the double-folding calculations have been obtained by using the density distribution of the9Be projectile and the 27Al target. The imaginary potentials have been taken as the Woods-Saxonvolume. The imaginary radius rw and the imaginary diffusion parameter aw have been fixed to 1.1fm and 1.85 fm, respectively. We compare the phenomenological model results with the doublefoldingmodel ones as well as the experimental data. These comparisons provide information aboutthe similarities and the differences in the models used in the calculations. The cross sections, thevolume integrals and the χ2/N values have been obtained for seven energies. Additionally we haveshown a threshold anomaly for the 9Be+27Al system in the both calculations.

        Bis (3-bromo-4,5-dihydroxybenzyl) ether, a novel bromophenol from the marine red alga <i>Polysiphonia morrowii</i> that suppresses LPS-induced inflammatory response by inhibiting ROS-mediated ERK signaling pathway in RAW 264.7 macrophages

        <P><B>Abstract</B></P> <P>Inflammation is a pathophysiological defense response against various factors for maintaining homeostasis in the body. However, when continued excessive inflammation becomes chronic, various chronic diseases can develop. Therefore, effective treatment before chronic inflammation development is essential. Bis (3-bromo-4,5-dihydroxybenzyl) ether (BBDE, C<SUB>14</SUB>H<SUB>12</SUB>Br<SUB>2</SUB>O<SUB>5</SUB>) is a novel bromophenol isolated from the red alga <I>Polysiphonia morrowii.</I> The beneficial physiological functions of various bromophenols are known, but whether BBDE has beneficial physiological functions is unknown. Therefore, we first investigated whether BBDE exerts any anti-inflammatory effect. We demonstrated that BBDE inhibits inflammation by reducing inflammatory mediators, such as nitric oxide, prostaglandin E2, iNOS, COX2, and pro-inflammatory cytokines (tumor necrosis factor-α, interleukin-1β, and interleukin-6), in LPS-induced macrophage cells. To examine the mechanism of action by which BBDE inhibits inflammation, we confirmed its effect on signal transduction and ROS generation. BBDE selectively inhibited ERK phosphorylation in the mitogen-activated protein kinase pathways. Moreover BBDE suppressed LPS-induced ROS generation in RAW 264.7 macrophage cells. Inhibition of LPS-induced ROS generation by BBDE also caused ERK inactivation and an inflammatory reaction. Therefore, BBDE inhibits LPS-induced inflammation by inhibiting the ROS-mediated ERK signaling pathway in RAW 264.7 macrophage cells and thus can be useful for treating inflammatory diseases.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Bis (3-bromo-4,5-dihydroxybenzyl) ether (BBDE) is a novel bromophenol isolated from the red alga <I>Polysiphonia morrowii</I>. </LI> <LI> First confirmed the anti-inflammatory effects of BBDE. </LI> <LI> BBDE inhibits LPS-induced inflammation by inhibiting the ROS-mediated ERK signaling pathway in RAW 264.7 macrophage cells. </LI> </UL> </P>

        Eighty-seven hundred and sixty five surgical specimens which had been diagnosed as tumor histologically from January 1953 to December 1967 at Medical General Laboratory, were studied statistcally. The most common benign tumor was neurofibroma. The most common malignancy in adult was lymphosarcoma. The next common malignency was tumor arising from stomach origin.

        Incidence, Morbidity, and Mortality of Achalasia: A Nationwide, Population-Based Cohort Study in South Korea

        Background/Aims: Although an association between achalasia and esophageal cancer has been reported, whether achalasia confers a substantial increase in mortality is unknown. Moreover, the causes of death related to achalasia have not been investigated. We performed this nationwide, population-based cohort study on achalasia because no such study has been performed since the introduction of high-resolution manometry in 2008. Methods: This study was performed using data extracted from the Korean National Health Insurance Service database, covering a 9-year period from 2009 to 2017. Control participants without a diagnostic code for achalasia were randomly selected and matched by sex and birth year at a case-to-control ratio of 1:4. Data on the cause of death from Statistics Korea were also analyzed. Results: The overall incidence of achalasia was 0.68 per 100,000 person-years, and the prevalence was 6.46 per 100,000 population. Patients with achalasia (n=3,063) had significantly higher adjusted hazard ratio (aHR) for esophageal cancer (aHR, 3.40; 95% confidence interval [CI], 1.25 to 9.22; p=0.017), pneumonia (aHR, 2.30; 95% CI, 1.89 to 2.81; p<0.001), aspiration pneumonia (aHR, 3.92; 95% CI, 2.38 to 6.48; p<0.001), and mortality (aHR, 1.68; 95% CI, 1.44 to 1.94; p<0.001). Esophageal cancer carried the highest mortality risk (aHR, 8.82; 95% CI, 2.35 to 33.16; p=0.001), while pneumonia had the highest non-cancer mortality risk (aHR, 2.28; 95% CI, 1.31 to 3.96; p=0.004). Conclusions: In this nationwide study, achalasia was associated with increased risk of mortality. Esophageal cancer and pneumonia were the most common comorbidities and the major causes of death in patients with achalasia.
