        Vesicovaginal fistula is a distressing complication which may follow hysterectomy, prolonged labor, chemical cauterization due to uterine prolapse, radiation therapy and cervix cancer. The management of vesicovaginal fistula still remains controversial in regard to the type of approach and timing of repair but the classic opinion is to wait 3 to 6 months to allow the surgical inflammatory reaction to subside. We experienced 4 cases of vesicovaginal fistula which were as follows; Case 1: 51-year-old The vesicovaginal fistula, about 3.0cm in diameter, after laparascopic hysterectomy 15 days ago was repaired with interposition of a gracilis muscle flap via the transvaginal route. Case 2: 38-year-old The vesicovagianl fistula, about 2.5cm in diameter, after Cesarean section 5 years ago was repaired with interposition of the omental flap via the transabdominal route because she had been accompanied by an overian cyst. Case 3: 46-year-old The vesicovaginal fistula, about 2.5cm in diameter, after manual self abortion at home 30 years ago was repaired with interposition of fibro-fatty tissue in labia via the transvaginal route. Case 4: 43-year-old The vesicovaginal fistula, about 2.0cm in diameter, after prolonged labor 12 years ago was repaired with interposition of the fibro-fatty tissue in labia via the transvaginal route. There noted no complications or recurrence during the minimal 1 year follow-up.

        Breeding Hybrid Rice with Genes Resistant to Diseases and Insects Using Marker-Assisted Selection and Evaluation of Biological Assay

        Developing elite hybrid rice varieties is one important objective of rice breeding programs. Several genes related to male sterilities, restores, and pollinators have been identified through map-based gene cloning within natural variations of rice. These identified genes are good targets for introducing genetic traits in molecular breeding. This study was conducted to breed elite hybrid lines with major genes related to hybrid traits and disease/insect resistance in 240 genetic resources and F1 hybrid combinations of rice. Molecular markers were reset for three major hybrid genes (S5, Rf3, Rf4) and thirteen disease/insect resistant genes (rice bacterial blight resistance genes Xa3, Xa4, xa5, Xa7, xa13, Xa21; blast resistance genes Pita, Pib, Pi5, Pii; brown planthopper resistant genes Bph18(t) and tungro virus resistance gene tsv1). Genotypes were then analyzed using molecular marker-assisted selection (MAS). Biological assay was then performed at the Red River Delta region in Vietnam using eleven F1 hybrid combinations and two control vatieties. Results showed that nine F1 hybrid combinations were highly resistant to rice bacterial blight and blast. Finally, eight F1 hybrid rice varieties with resistance to disease/insect were selected from eleven F1 hybrid combinations. Their characteristics such as agricultural traits and yields were then investigated. These F1 hybrid rice varieties developed with major genes related to hybrid traits and disease/insect resistant genes could be useful for hybrid breeding programs to achieve high yield with biotic and abiotic resistance.

        Predictors for the need for endoscopic therapy in patients with presumed acute upper gastrointestinal bleeding

        Background/Aims: Selecting patients with an urgent need for endoscopic hemostasis is difficult based only on simple parameters of presumed acute upper gastrointestinal bleeding. This study assessed easily applicable factors to predict cases in need of urgent endoscopic hemostasis due to acute upper gastrointestinal bleeding. Methods: The consecutively included patients were divided into the endoscopic hemostasis and nonendoscopic hemostasis groups. We reviewed the enrolled patients’ medical records and analyzed various variables and parameters for acute upper gastrointestinal bleeding outcomes such as demographic factors, comorbidities, symptoms, signs, laboratory findings, rebleeding rate, and mortality to evaluate simple predictive factors for endoscopic treatment. Results: A total of 613 patients were analyzed, including 329 patients in the endoscopic hemostasis and 284 patients in the non-endoscopic hemostasis groups. In the multivariate analysis, a bloody nasogastric lavage (adjusted odds ratio [AOR], 6.786; 95% confidence interval [CI], 3.990 to 11.543; p < 0.0001) and a hemoglobin level less than 8.6 g/dL (AOR, 1.768; 95% CI, 1.028 to 3.039; p = 0.039) were independent predictors for endoscopic hemostasis. Significant differences in the morbidity rates of endoscopic hemostasis were detected between the group with no predictive factors and the group with one or more predictive factors (OR, 2.677; 95% CI, 1.920 to 3.733; p < 0.0001). Conclusions: A bloody nasogastric lavage and hemoglobin < 8.6 g/dL were independent predictors of endoscopic hemostasis in patients with acute upper gastrointestinal bleeding.

        Clinical Heterogeneity of Anti-GM2-Ganglioside-Antibody Syndrome

        <P><B>Background and Purpose</B></P><P>Antiganglioside antibodies are known to play a pathogenic role in Guillain-Barré syndrome (GBS). Either an immunoglobulin (Ig)G- or IgM-type anti-GM2 antibody is detected in rare cases in GBS patients. However, the specific pathogenic role of these antibodies in GBS has not been reported previously. This study aimed to define and characterize the clinical spectrum of GBS with anti-GM2 positivity.</P><P><B>Methods</B></P><P>We reviewed the database of the Dong-A University Neuroimmunology Team, which has collected sera of GBS and its variants from more than 40 general and university-based hospitals in Korea. Detailed information about the involved patients was often obtained and then corrected by the charge doctor applying additional questionnaires.</P><P><B>Results</B></P><P>Four patients with acute monophasic peripheral neuropathy or cranial neuropathy with isolated IgM-type anti-GM2-antibody positivity were recruited. In addition, IgG-type anti-GM2 antibody was solely detected in the sera of another four patients. The IgM-positive group comprised heterogeneous syndromes: two cases of acute motor axonal neuropathy, one of acute inflammatory demyelinating polyneuropathy, and one of isolated facial diplegia. In contrast, all of the cases enrolled in the IgG-positive group manifested with dizziness with or without oculomotor palsy due to cranial neuropathy syndrome.</P><P><B>Conclusions</B></P><P>This study has identified that anti-GM2 antibody can be found in various subtypes of GBS and its variants in rare cases. Compared to the clinical heterogeneity of the IgM-positive group, the IgG-positive group can be characterized by cranial-dominant GBS variants presenting mainly with oculomotor and vestibular dysfunctions.</P>

        수학 기피유형의 분류와 치유 효과의 분석

        The purpose of this study is to find out effective ways to take care of the 8th and 10th graders' disposition causing math. disliking. To accomplish this goal, we proceeded as follows; First, we categorized the 11 factors recognized as the reasons of math. disliking into 4 math. disliking causes such as psychological & environmental cause, conceptual cause, relational cause and application related cause. Second, to take care of these four causes, we developed materials which are closely related with the contents of the 8th and 10th graders' school mathematics. Third, with these materials we taught the students who had proved to have the math. disliking trend, for one semester. As a consequence of this experiment we arrived at the following results. As for psychological & environmental causes, 35.7% of the 8th graders and 17% of the 10th graders proved to have been improved significantly. This result shows that the curing of the psychological & environmental causes is more effective in the 8th graders than in the 10th graders. i.e., the curing effects of the students' psychological & environmental cause for disliking math. decline as they get older. As for conceptual cause, 35% of the 8th graders and 30% of the 10th graders proved to have been improved significantly. In case of the 8th graders this ratio was similar to that of the other causes. But as for the 10th graders this ratio was a little low compared with that of the case of relation causes and application related cause. As for relational causes, 35% of the 8th graders and 49% of the 10th graders proved to have been improved significantly. Especially the 10th graders improved greatly. Among the four factors that compose this cause, especially hierarchy and connection factors were effectively cured. On application related causes, 47% of the 8th graders and 57% of the 10th graders proved to have been cured significantly. And among the four types of causes listed above, this was most successfully cured one. Of the two factors of this cause, the basic application factor appeared to have been improved in all experimental groups. In connection with teaching methods, we found out the followings two facts. First, the more teachers push students to solve their tasks with their own efforts, the higher is the ratio of cure. Second, the more teachers teach students personally, the more effective are the teaching results.

      • 수학 기피요인의 설정 및 기피성향의 분석도구 개발

        To lessen the ratio of under achievers is one of the most urgent task which recent school mathematics education is confronted with. To cope with this problem efficiently, math. teachers should know more specifically and concretely the causes that make the students dislike mathematics. But actually, there are too many reasons for these situations. So, in this paper, we tried to devise a tool to analyze and measure each student's math. disliking status. We proceeded this research via the following procedures. 1. Grasping the causes which make the students dislike mathematics as specifically as possible. To obtain this, we asked more than 300 of secondary school students to write down their thoughts about school mathematics. 2. Analyzing the responses, we abstracted 74 numbers of items which were supposed to be the causes for secondary school students' mathematics disliking. 3. With these items we made a test to measure students' aptitude for each item. 4. With this test paper, we tested over 800 of secondary school students. Through factor analysis and theoretical argument, we categorized the 74 items into 11 groups whose names were defined as factors of mathematics disliking. 5. For each of these 11 factors, we developed a norm which could serve as standard of comparison in measuring each student's mathematics disliking status. Using this tool teachers were able to describe each student's traits of mathematics disliking more specifically.

        춘원당 한방병원의 성장역사와 성공요인

        본 연구의 목적은 춘원당 한방병원이 장수기업으로의 발전과정에서 나타난 성공요인을 살펴보고, 춘원당 한방병원의 성공요인이 우리에게 주는 시사점을 도출하는데 있다. 논문은 크게 두 부분으로 구성되어 있다. 첫째, 춘원당 한방병원의 발전과정과 한방의료기관의 현황을 살펴보았다. 둘째, 춘원당 한방병원이 장수기업으로 성장한 성공요인을 분석하여 시사점을 도출하고자 하였다. 이를 위하여 장수기업의 일반적인 성공요인을 살펴본 후, 춘원당 한방병원의 성공요인을 분석하고 시 사점을 찾고자 하였다. 한방병원의 발전과정과 성공요인을 파악하기 위하여 Miller and Le Breton-Miller (2005)가 제시한 연속성(Continuity), 공동체의식(Community), 관계(Connection), 지휘 (Command) 등 4C 개념을 적용하여 7대에 걸친 장수기업 춘원당 한방병원의 성공사례를 분석하였다. 본 연구를 통해 우리나라에도 100년 이상 장수하고 있는 소규모 기업이 존재한다는 사실과 우리나라의 소규모 장수기업의 성공요인을 재조명하였다. 그리고 선진국의 사례 에서처럼 이러한 기업들이 지속적으로 성장할 수 있도록 지원하는 정책들이 개발되기를 기대해 본다. 다만 춘원당 한방병원의 한 가지 사례를 분석했다는 한계점이 있어 본 연구의 결과를 일반화하는데는 주의가 필요하다. The purpose of this paper was to identify 164 years of Choonwondang Korean Medical Center s success factors. This paper was divided into two parts : first are Choonwondang s life stages and the present condition of Korean Medical institutions, second is Choonwondang Korean Medical Center s success factors. Choonwondang Korean Medical Cent s life stages were divided into three time periods : opening small medical house, theorized Choonwondang medical system, 7 generations devoted to Korean Medicine get international fame. And Choonwondang s success factors were examined Miller and Le Breton-Miller(2005) 4C concept : Continuity and Community and Connection and Command. This study tries to highlight success factors for Choonwondang s 164years. And the implications is needs to provide sufficient support policies for small companies to grow into long life enterprise.

      • MIC Distribution of Isoniazid, Rifampicin, Ethambutol, Pyrazinamide, and Levofloxacin in cPMTb Cohort Mycobacterium Tuberculosis Isolates

        Background Minimum inhibitory concentration (MIC) of antibiotics has been widely conducted for drug susceptibility tests (DST) of most bacterial infectious diseases, except tuberculosis (TB). Current phenotypic DST for TB uses one or two critical concentrations to determine the resistance of anti-TB drugs. The MIC test would be necessary not only for therapeutic drug monitoring-based precision medicine but also for detecting borderline resistance of anti-TB drugs. Here, we present the distribution of MIC of 4 first-line drugs and levofloxacin in cPMTb cohort Mycobacterium tuberculosis (Mtb) isolates. Methods From 2019-2020, 108 Mtb strains were collected in the cPMTb cohort. Broth microdilution Method in 96 well plates was used for the MIC test of anti-TB drugs, except for pyrazinamide (PZA). MGIT960 system was used for PZA MIC determination. Results The proportions of drug-resistant (DR) and drug-susceptible (DS) strains were 15.7% and 84.3%, respectively. Most of the DS MIC were distributed below the critical concentration of the drug. Three discordances between phenotypic DST and MIC were found in isoniazid, which is 2.8% in total. We observed 2 cases (1.9%) of low-level resistance (MIC 0.5 ug/ml), determined as DS for rifampicin in phenotypic DST but DR in genotypic DST. Conclusion MIC of cPMTb cohort 108 strains were distributed mostly below the critical concentration of all drugs. There are 6 cases of discordance between MIC and DST (phenotypic or genotypic). The MIC test could be utilized to overcome the limitation of phenotypic DST using critical concentration and fill the gap between genotypic and phenotypic DST.
